OUR REQUIREMENTS

Required

Position Overview
The Mobile Team SocialWorker is an integral part of Community Access' mobile multi-disciplinary team that supports tenants with complex mental health needs across our supportive housing programs in NYC. The SocialWorker provides engagement and supports program participants using a person-centered model that supports individuals in working through crisis, developing new coping strategies, assessing risk, maintaining their housing and developing and reaching goals that could include but are not limited to furthering their recovery, accessing community-based treatment and support services, connecting to community, family and social support networks, and exploring educational or vocational opportunities. The SocialWorker supports other team members and housing staff by acting as a bridge between participants and resources, ultimately contributing to more holistic support. Consistently document 40 hours of direct service work each month.
Job Qualifications
Have an understanding, appreciation, and commitment to the philosophy and mission of Community Access, with particular expertise in recovery-oriented service delivery.
Minimum of a Bachelor's Degree in Social Work or related field required; LMSW or LCSW preferred.
Minimum of two years working with people who have a history of homelessness, substance use, and/or mental health diagnosis. required.

Qualifications
Experience & Background:
Lived experience or a strong personal understanding of the challenges faced by marginalized or unhoused populations is highly valued.
Prior experience in community outreach, peer advocacy, or social services is preferred.
Skills & Competencies:
Excellent communication and interpersonal skills, with the ability to navigate sensitive situations with empathy and confidence.
Strong problem-solving skills, crisis intervention experience, and the ability to work independently in dynamic urban environments.
Familiarity with local community resources and service networks is a plus.

To provide a safe, nurturing and stimulating environment in which the children can grow physically, socially, emotionally and intellectually using therapeutic intervention and goals of the program as guidelines.
ESSENTIAL DUTIES AND RESPONSIBILITES
Evaluate children in area of expertise for initial / requested / annual evaluation as per CPSE (SocialWorker- Complete social history intakes.)
Complete IEPs for children evaluated.
Develop a weekly therapy schedule for children on caseload to meet each child's IEP therapeutic mandates.
Follow schedule for individual/small group therapy sessions.
Provide and implement well planned therapeutic activities that encompass using a multitude of learning opportunities for children requiring services as recommended on the IEP.
Review therapeutic goals and objectives for children to ensure continuity in therapy activities.
Maintain accurate attendance records, files and paperwork (session notes, progress reports, etc.) for each child on caseload.
Complete required paperwork in a timely manner (evaluations, IEP's, quarterly progress reports, session notes, etc.).
Participate in team meetings to make recommendations for development of IEP.
Participate in team case conference meetings to review and update student intervention strategies.
Establish and maintain professional communication with parents (notebooks, phone, etc.) keeping them informed of their child's progress, activities, and needs.
Participate in parent-teacher conferences twice yearly, or as needed, to review IEP goals/objectives and school performance.
Provide and ensure appropriate supervision of children at all times.
Provide and ensure appropriate, language and positive methods of disciplinary intervention as defined by our staff policy.
Prepare and maintain a safe clean, and well-organized learning environment.
Ensure that the Department of Health, Bureau of Day Care regulations is followed.
Assist in planning school wide events.
Maintain and respect confidentiality of child, parent, staff and other center issues.
Maintain a high standard of classroom intervention by training classroom staff in area of expertise.
Attend weekly staff meetings, in-service training and on and offsite professional development workshops, as determined by the school calendar.
Keep Director informed of incidents relating to accidents, concerns about child's progress, or extra ordinary circumstances.
Appropriately represent the Center in all forums.
Report to the Director or in his/her absence the Supervising Teacher.
ADDITIONAL DUTIES AND RESPONSIBILITIES FOR SOCIALWORKER
Provide behavior management strategies when deemed appropriate.
Organize and participate in parent support groups.
Provide family/individual intervention when deemed appropriate.
