Under supervision of the I/DD RN provides nursing and medical related services to individuals, and consultation to staff regarding health care related matters, and other Individual Program Plan (IPP) components for which responsible; functioning as an Interdisciplinary Treatment Team (ITT) member, and assuring compliance with all applicable standards.
Essential Job Functions:
Assure and carry out all medical treatments prescribed by physician/dentist
Administer and monitor the security of medications to individuals per established policy and procedure
Assure and monitor health care services in accordance with Agency and program policy and procedure. Serve as information resource to staff, parents, and programs
Handle medical emergencies in accordance with established protocols; accompany individuals to Emergency Department or medical appointments, if needed, and complete documentation of injury/visit in individual's chart
Follow through immediately on medication changes and newly written orders, calling MD to clarify if necessary
Assure physician documents on medically related progress notes as necessary
Ensure orders on pharmacy prescriptions from designated pharmacy store, monitor prescription renewals, and assure medications are in supply at all times
Observe each individual for health status and hygiene and take appropriate action if called for
Monitor and submit requests for medical/nursing equipment; maintain medication storage area and medical supply cabinet
Alert all staff and I/DD RN regarding pertinent medical information via designated communication system.
Ensure treatments are completed and recorded with full date and time in the individual's chart
Compare doctor's orders to the individual's Medication and Treatment sheets, note any changes, and notify the appropriate staff of any discrepancies. Secure physician review and sign off orders quarterly
Fulfill all required documentation including but not limited to medical related progress notes, seizure charts, menstrual chart updates, formulary, immunization records, etc.; maintain medical files and records under supervision of I/DD RN.
Acts as a liaison with other community service providers
Participate in reviews and weekly clinical meetings as required
Perform all other necessary duties relevant to the position as requested by the supervisor
Knowledge, Skills, and Abilities
Ability to work independently and motivate others
Ability to communicate effectively, both orally and in writing
Education and Experience:
Must be licensed and currently registered as a Licensed Practical Nurse by the State of New York State Education Department
Specialized training or one year of professional experience in treating the developmentally disabled preferred
Physical Requirements/Working Conditions:
Ability to stand/walk frequently
Ability to reach above shoulder level
Ability to turn/twist upper body
Ability to lift at least 25 lbs.

Our programs are spread across 25 locations and include over 1200 units of supportive housing in three boroughs, the Howie the Harp Advocacy Center, Peer-driven Crisis Respite Center, Adult Home Initiative, Assertive Community Treatment (ACT) and the Intensive Mobile Team (IMT).
Program/Department Description:
Intensive Mobile Treatment (IMT) is an innovative, creative, trauma-informed mobile practice model designed to provide treatment, rehabilitation and support services to individuals whose needs have not been met by traditional services. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served.
Community Access' IMT Teams serve individuals who are street homeless or living between institutional settings and housing. Team members work collaboratively with settings in which the participant is living and use assertive engagement strategies to proactively engage individuals in services. IMT team members share responsibility for the people they serve, helping individuals attain housing and other personal and safety goals that may be of a health, social, work or education nature.
IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant's choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week.
Position Overview
The IMT RegisteredNurse is responsible for overseeing the health interventions for all program participants, in coordination with the team's prescriber and other staff. The RegisteredNurse works collaboratively with program participants and team members to ensure healthcare is coordinated and delivered within the framework of participant self-determination, recovery, and cultural competency. The Registered Nure is responsible for developing, implementing, and maintaining IMT's program nursing standards all while guiding the development of policies and procedures and identification of resources essential to delivering nursing services. The RegisteredNurse will also conduct psychiatric, physical health and other assessments with a special focus on assessments and evaluations with individuals using substances or experiencing street homelessness.
Key Performance Indicators
All key performance indicators below are expected to be met fully.
Update face sheets for new diagnostic and prescription information within 2 business days of any hospital contact.
Complete progress notes for all contacts, collaterals, and attempts within 2 business days.
Complete assigned nursing evaluations on time.
Job Qualifications
Have an understanding, appreciation, and commitment to the philosophy and mission of Community Access.
Commitment to person-centered treatment strategies, upholding participants' rights, and self-determination in service provision.
Currently licensed as a registered professional nurse by the New York State Education Department.
Minimum two (2) years' experience working as an RN.
Minimum two (2) years' experience working with persons with mental health condition in health or social services setting.
Excellent written, verbal and interpersonal communication skills.
Ability to synthesize and summarize information and make judgments regarding care.
Eligible for full and unconditional participation in the Medicaid and Medicare programs.
Computer proficiency in Windows operating systems and programs, such as MS Word, Excel, PowerPoint, as well as comfort with learning new electronic systems.
Comfort providing treatment in non-traditional settings, including but not limited to the street, community spaces, and shelters.
Must be able to work in the community, including use of public transportation throughout the boroughs
Must be able to lift and carry 25 pounds
Bilingual Spanish-speaking, preferred
Must be fingerprinted and cleared by the New York State Justice Center

We are seeking a Full-Time RegisteredNurse (RN) in Manhattan, NY to deliver care to individuals with disabilities in a home health setting, utilizing the nursing process of assessment, planning intervention, implementation, and evaluation.
Starting rate is $90,000-$110,000/ annually, depending upon experience
Responsibilities
Participate in the admission of new residents to assigned homes.
Develop plans to guarantee that the medical and healthcare needs of the residents are met and ensures the appropriate medical follow-up.
Review lab work and coordinates annual physical exams and care with clinics, outside physicians, and hospitals.
Provide special instructions to resident and home staff related to medical diagnosis, allergies, diet, etc., and is available for ongoing consultation to staff regarding medical problems and healthcare.
Clarify medication orders and contact physician to review consumer's status and medication plans.
Participate in quarterly and Annual Individual Plan (IP) meetings for assigned participants.
Create and distribute Individual Plan Medical/Health Reports detailing participants' general health, health care goals, and physician recommendations for each participant.
Provide documentation regarding individual's health status for various support people.
Make referrals for needed medical services and ensure that follow-up care is pursued.
Provide quarterly in-service training for support staff.
Advises supervisor of all concerns and/or issues related to the provision of nursing services.
Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
Follows strict OSHA procedures regarding blood borne pathogens.
Minimum Requirements
Current Nursing License required.
OPWDD experience preferred.
Minimum 2 years' experience as an RN.
Bachelor's Degree preferred.
Diabetes Education Certification preferred.
Ability to calculate figures related to medical dosages.
Ability to establish a comfortable and supportive relationship with a person receiving supports.
Valid Driver's License
