HOURS:
Full-time, 37.5 hours per week
* Thursday-Saturday 8am-8:30pm
QUALIFICATIONS:
State RN licensure, CPR-BLS required; Justice Center background check, including finger printing required; Certification in addictions or psychiatric nursing a plus. Previous relevant work experience in detoxification programs or emergency departments is preferred. Basic computer literacy required. CPR training certification or willingness to take training class in CPR.

We are seeking a Full-Time Registered Nurse (RN) in Manhattan, NY to deliver care to individuals with disabilities in a home health setting, utilizing the nursing process of assessment, planning intervention, implementation, and evaluation.
Starting rate is $90,000-$110,000/ annually, depending upon experience
Responsibilities
Participate in the admission of new residents to assigned homes.
Develop plans to guarantee that the medical and healthcare needs of the residents are met and ensures the appropriate medical follow-up.
Review lab work and coordinates annual physical exams and care with clinics, outside physicians, and hospitals.
Provide special instructions to resident and home staff related to medical diagnosis, allergies, diet, etc., and is available for ongoing consultation to staff regarding medical problems and healthcare.
Clarify medication orders and contact physician to review consumer's status and medication plans.
Participate in quarterly and Annual Individual Plan (IP) meetings for assigned participants.
Create and distribute Individual Plan Medical/Health Reports detailing participants' general health, health care goals, and physician recommendations for each participant.
Provide documentation regarding individual's health status for various support people.
Make referrals for needed medical services and ensure that follow-up care is pursued.
Provide quarterly in-service training for support staff.
Advises supervisor of all concerns and/or issues related to the provision of nursing services.
Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
Follows strict OSHA procedures regarding blood borne pathogens.
Minimum Requirements
Current Nursing License required.
OPWDD experience preferred.
Minimum 2 years' experience as an RN.
Bachelor's Degree preferred.
Diabetes Education Certification preferred.
Ability to calculate figures related to medical dosages.
Ability to establish a comfortable and supportive relationship with a person receiving supports.
Valid Driver's License

Job Responsibilities:
Administrative Nursing Duties
· Oversees nursing/ lab staff
· Maintains patient flow along with clinical coordinators.
· Reviews Schedules of Nursing staff prepared by Clinical Coordinator
· Maintains and keeps employee Health files, training, Licenses, and registrations up to date
· Evaluates nurses' performance and administers performance reviews, coaching, counseling, and employee discipline. Shared with Nursing Coordinator
· Teaches and mentors new staff
· Identifies staff developmental and training needs. Training of new products and procedures as needed
· Shares in maintaining immunization supply, Private and VFC
· Conducts bi-monthly staff meetings with Nursing staff.
· Attends all Administrative, Provider, and Lab meetings
· Monitors for and communicates any changes with the Nursing staff
· Prepares and conducts morning Huddles along with the Clinical Coordinator on assigned days.
· Monitors staff at the end of the day to make sure that the staff who have completed work have punched out and are leaving the facility
· Liaison between Providers and Nursing Staff
· Follows and enforces all the company's policies and procedures
· Coordinates Nursing Student activities
· Reviews charts of high-risk patients for care management
· Maintains current knowledge of nursing care practices and participates in research and educational programs
· Works with HR to complete reviews, monitor overtime, and any disciplinary actions that need to be documented and turned into HR.
