Draft initial individualized Service Plans and update as needed. Review service plans with responsible parties and Executive Director as requested by DHS. Ensure continuity of the assisted living residents' total care regimen. Under the direction of the DHS, provide training and education as needed on a range of essential topics including competent medication delivery, acceptable treatments, safety protocols, emergency procedures, accurate record, and state requirements to provide the best possible resident care. Perform all assigned duties accurately and timely including required documentation. Serve as a medication technician, if needed. Provide coverage of job duties within the department during absences, either through assistance in finding coverage, or personal completion of duties. Check vital signs as directed and look for signs that health is deteriorating or improving. Perform basic nursing functions such as treatments, medication delivery and managing resident emergencies ensuring residents are comfortable, well-fed, and hydrated. Maintain adequate inventory of resident care supplies and demonstrate a commitment to minimizing waste of supplies and equipment. The Health Services Coordinator preserves dignity and promotes independence for each resident while providing care and services according to each individual service plan and in accordance with MBK policy and procedure. Duties & Responsibilities: Conduct and coordinate assessments / evaluations of potential residents and make recommendations for admission in accordance with current rules, regulations, and community policies and procedures that govern resident assessment. Draft initial individualized Service Plans and update as needed. Review service plans with responsible parties and Executive Director as requested by DHS. Ensure continuity of the assisted living residents' total care regimen. Under the direction of the DHS, provide training and education as needed on a range of essential topics including competent medication delivery, acceptable treatments, safety protocols, emergency procedures, accurate record, and state requirements to provide the best possible resident care. Perform all assigned duties accurately and timely including required documentation. Serve as a medication technician, if needed. Provide coverage of job duties within the department during absences, either through assistance in finding coverage, or personal completion of duties. Check vital signs as directed and look for signs that health is deteriorating or improving. Perform basic nursing functions such as treatments, medication delivery and managing resident emergencies ensuring residents are comfortable, well-fed, and hydrated. Maintain adequate inventory of resident care supplies and demonstrate a commitment to minimizing waste of supplies and equipment. Coordinate prescription orders with doctors' offices and manage pharmacy delivery of medications including oversight of the central storage, tracking and delivery of medications, and ensuring Medication Administration Records are completed according to company policy and State regulations. Aid in the community marketing effort through positive interactions, acting as a liaison between the community, and families/outside health service providers. Maintain a safe and secure environment for all staff, residents, and guests following established safety standards, policies, and procedures. Understand and comply with all Federal, State, and local regulations, and all company policies and procedures. Promote a spirit of teamwork and open communication in accordance with the MBK principles and core values. Perform other job duties or special projects as assigned or requested by the Supervisor or Executive Director. Education Requirements: RN, LVN or LPN License that is active and in good standing is essential; adherence to all requirements to maintain license including CEU completion and timely renewal. Experience Requirements (in years): 2+ years of prior related work experience functioning in a similar healthcare environment. Required Competencies/Licenses/Certifications: Must complete required Background clearances, health screening and provide negative TB test results within 7 days of employment (must be within the last 6 months). Valid state driver's license and valid insurance or reliable method of transportation required. Must have competent and current technical and computer skills, including familiarity with Microsoft Office Suite (e.g., Word, Excel, Outlook, etc.) and office equipment. Must have excellent communication skills including the ability to speak, write and read English. Must possess the ability to make sound, independent decisions when circumstances warrant, and remain calm during stressful or emergency situations. Must possess the ability to deal tactfully and professionally at all times with personnel, residents, family members, and guests. Must possess strong organizational, problem solving and time management skills. Must maintain a neat and organized work environment to promote safe coordination of resident care. Physical Demands & Work Environment: Must be able to work a flexible schedule, opposite of the Director of Health Services, including weekends and holidays. Must be mobile and able to perform the physical requirements of the job, including walking, bending, kneeling, squatting, pulling, reaching overhead, and repetitive motion. Ability to move intermittently throughout the workday, in the community and between neighborhoods. Ability to lift and carry up to 50 pounds and push up to 250 pounds. Ability to assist in the physical movement of residents during routine transfers or in emergency situations.
