Position Summary
VIVA Counseling is an outpatient child, youth and family therapy program that is part of North County Lifeline. The Lead Clinician serves both in a clinical and leadership role. The position provides individual family and group therapy to youth up to age twenty 21 and their families both in- person and via telehealth. The Lead Clinician also serves an important role as team's primary trainer of clinical documentation and administrative processes. If you have experience with Medi-Cal documentation, are highly organized and enjoy coaching others, this position is a great opportunity to get leadership experience while still serving clients. All clinicians receive supervision and associates earn hours towards BBS licensure. Required Education and Experience
Masters Degree in counseling, social work or related field, and BBS-registered AMFT, LMFT, ACSW, LCSW, APCC or LPCC.
Experience with EHR preferred, but will consider candidate with experience in other EHRs.
Bilingual in English/Spanish preferred but not required. Bilingual pay available.

You will be responsible for providing individual and family therapy to youth ages 12-17 who are participating in substance abuse services. The Metro Mental Health Specialist works as a team member of a multi-disciplinary homeless street outreach team (MDT), in collaboration with social service professionals, ensuring the delivery of quality care in accordance with the rules and regulations established by the Los Angeles Department of Health Services (DHS).
The Metro Mental Health Specialist as part of the DHS ICMS Street Based Engagement Outreach (SBE) Team, will work in a variety of settings that will involve traveling on public transportation and on HYC designated vehicles. The Metro Mental Health Specialist will travel with the MDT Outreach team to a variety of sites including but not limited to transit stops on bus and rail line stops/stations, homeless encampments, shelters, drop-in centers, mobile outreach teams, and other areas as designated by DHS.
The Mental Health Specialist will use the whatever it takes approach to help people experiencing homelessness with mental health support and interventions on the field.
Essential Duties and Responsibilities:
Facilitate connections with local mental health providers to establish a network of resources.
Provides mental health assessments, counseling, and interventions when necessary.
Provide crisis intervention implementation, especially for clients experiencing the effects of mental health crisis.
Facilitate training among the MDT team regarding helpful practices, methods, and interventions for addressing mental health among people experiencing homelessness.
Collaborate with the MDT outreach team to engage with people experiencing homelessness on the Metro transit system and help connect them to local resources to help them achieve their goals and improve their quality of life.

Meet with established clients on a regular basis to review, evaluate, and support goal completion.

Complete all necessary paperwork to track client progress (daily logs, ISP, etc.) as well as enter information into the appropriate data management system (HMIS).

DUTIES AND RESPONSIBILITIES:
Conducts family-centered, strengths-based, culturally competent individualized intakes/assessments, gathering information from family, consumer, significant other, and involved agencies in the office or the field as appropriate.

Performs intake assessments and formulates diagnosis and case conceptualization.

DUTIES AND RESPONSIBILITIES:
Under the supervision of the Family Preservation Program Manager, the In-Home Outreach Counselor will:
Conducts comprehensive intake and assessment with each family to evaluate specific needs and coordinate all systems of care.
Complete assessments every 90 days.
Provide one-hour, in-home counseling and clinical case management sessions to families in and around Service Area 7 & Service Area 8 (LA County Cities: Santa Fe Springs, Commerce, Long Beach and surrounding Communities) once per week, at minimum.
Conduct unannounced visits and child follow-up sessions, as needed, to ensure child safety.
Schedule a Multidisciplinary Case Planning Committee (MCPC) Meeting every 60 days with parents, FP manager, and Children's Social Worker (CSW) for each family.
Create and develop goals for each family by completing an MCPC service plan for every MCPC meeting.
Provide appropriate resources/referrals to families to assist with court mandates and/or assist with each family's needs, as well as follow-up on progress.
Collaborate and maintain effective communication with the assigned CSW to assist in accomplishing case plan goals and outcomes for families.
Complete and submit monthly reports for all cases to the assigned CSW on the case.
Maintain an average caseload of approximately 10-12 families per week.

You will be responsible for providing individual and family therapy to youth ages 12-17 who are participating in substance abuse services. Lifeline Services is hiring a Bilingual Mental Health Therapist to support our growing team. You will be responsible for providing assessment, treatment planning, individual and family therapy and case management to youth experiencing homelessness and their families. Services are offered at clients' homes, in the community, at Lifeline's clinic sites and via Zoom. Our clinicians use a wide variety of counseling modalities and work in a culturally sensitive manner. The position is supervised by the Program Manager, and clinicians receive weekly group and individual supervision. Associate level clinicians gain hours towards licensure. Most positions are based in San Diego but there are North County positions available too. This is a direct service role that will help our team support vulnerable youth in reaching their goals!
We're looking for a candidate who is able to connect with vulnerable youth, especially teenagers and young adults. This role requires organizational skills, flexibility and teamwork, as the Therapist manages a complex schedule and works as part of the treatment team.

Position Overview
General Responsibilities
Under the supervision of the Community Services for Families Program Supervisor, the Mental Health Therapist will provide assessment services for CSF families with children 0-18 and have domestic violence as the reason for the referral. Assessment services may include every family member (parents and/or children) living in the home and will be conducted in the family home or at other locations as directed by client or County CWS staff.
Specific Activities
Provides culturally competent, trauma informed assessment services for CSF families referred by Child Welfare Services and community referred families.
Provide assessments within 3 business days of receipt of referral
Prioritize cases by case type in as stated in the CSF contract
Conduct assessments both in person and occasional virtual sessions in the office and in the community
Evaluate all members of the family living in the home for domestic violence, substance abuse, mental health, trauma, and other significant needs.
Make recommendations on the basis of assessment findings for preventive or treatment services to mitigate exposure to trauma (for children) and to mitigate/treat issues negatively impacting child safety (for parent/caregivers).
Provide initial recommendations for treatment, mental health and/or other services to County Social Worker within 3 business days of completing assessment.
Complete and submit a written follow up report of assessment findings to referring party within 10 business days including assessment tools used, outcomes for each family member, type and level of services recommended, likelihood of a successful treatment outcomes, formal and informal interventions to strengthen family functioning to ensure child safety, prevention services to mitigate future effects of current child trauma, Family Centered Meetings attended, CSF referrals, and other information.
Participates in Child and Family Team Meetings to discuss client progress in services and support them in achieving their case plan goals.
Participates in Multi-Disciplinary Teams with County staff as needed.
Participates in quarterly CSF countywide peer review process to discuss service delivery and improve upon best practices.
Maintains ongoing communication with the CSF team to ensure quality of service delivery.
Maintains detailed case files and documentation utilizing County approved forms and formats.
Produces monthly reports of activities and submits to the CSF Supervisor.
Remains culturally sensitive and respectful in all client and staff interactions.
Participates in required staff meetings, collaborative meetings, trainings, County meetings, supervision, QA processes, and all other meetings and events as requested.
Completes other duties as assigned.

Qualifications and Requirements
LMFT/LCSW/LPCC eligible by the California Board of Behavioral Science plus 2 years of related experience in direct client service. Bilingual Required. Training in and mastery of family systems theory such as Structural Family Therapy and application of diagnostic criteria. Strong skills in interviewing, rapport building, and assessment. Certification in Domestic Violence Assessment and Treatment provided once hired. Ability to make sound clinical decisions independently and to apply clinical judgment to developing effective treatment recommendations. Ability to focus on multiple priorities and meet deadlines. Must be compatible with Lifeline's purpose and philosophy. Must have reliable transportation current auto insurance, and a valid California Driver's license. Must be flexible to accommodate varying client schedules. Hire is contingent upon passing required clearances, including criminal background checks, TB and drug test.
