Position Summary Under the general supervision of the LifeSpring Program Manager, the Case Manager plays a critical role in assisting youth and young adults who are at risk of homelessness or experiencing homelessness. This role combines housing navigation with intensive case management and skill development, ensuring that services are delivered through a youth-driven, whole person, trauma-informed model. The position supports participants in achieving independence, obtaining safe housing, and accessing necessary resources, all while adhering to a strength-based, culturally relevant, trauma-informed, and outcome-based approach. Specific Activities: 1. Client Support and Engagement: Create a welcoming, supportive environment to engage youth effectively and to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence. Provide basic needs assistance, including food, clothing, hygiene products, and access to essential services. Conduct intakes, eligibility screenings, basic needs assessments, and administer tools like the TAY-VI-SPDAT. Offer crisis management, safety planning, and emergency support as needed. Empower youth to make informed decisions by providing options and supporting their autonomy. 2. Housing Navigation: Create a housing plan with tangible goals and milestones to work towards housing and self-sufficiency. Assist and partner with youth to finding housing options and securing stable, inclusive housing. Assist youth with understanding and signing leases. Also provide support with housing applications, security deposits and other documents related to securing housing. Build and maintain relationships with local housing providers, such as landlords, property managers, and other entities. Utilize the Coordinated Entry System to connect youth with appropriate housing resources. 3. Case Management and Skill Development: Provide intensive case management, including goal setting, life skills training, economic literacy, and employment readiness. Help build and support participants' relationships with family and community with an emphasis on permanent connections. Facilitate skill development opportunities such as meditation, public benefits advocacy, tenancy, and employment training. Coordinate care with mental health professionals, primary care providers, outside social service organizations, and other supportive services. Assist with job search, educational goals, and development of life plans supporting independence. 4. Community Engagement and Outreach: Initiate and organize outreach efforts to engage youth and identify service gaps. Participate in county collaborative meetings and community partner discussions to create best practices. Provide training and outreach to enhance program visibility and effectiveness as needed. 5. Administrative Duties and Compliance: o Maintain accurate and timely documentation of client progress and interactions in Efforts to Outcomes (ETO). o Track goals, compile data, and complete reports for funding sources. o Uphold confidentiality and privacy standards in accordance with HIPAA. o Perform other administrative tasks, including document storage, auditing, and work order submissions. 6. Program Development and Coordination: Collaborate with other LifeSpring staff to screen and interview prospective participants. Assist in program development and resource identification to enhance service delivery. Participate in program meetings, group supervision, trainings, and other collaborative activities as needed. Required Education and Experience: Bachelor's degree in social work or a related field, with at least 3 years of experience, or Master's degree and registered intern status with the BBS, preferred. Experience working with transitional age youth, particularly those experiencing homelessness or at risk, and familiarity with former foster youth and CSEC/HT populations. Strong skills in engagement, crisis intervention, case management, and goal setting. Excellent verbal and written communication skills, computer literacy, and organizational abilities. Knowledge, Skills, and Abilities: Advanced knowledge of trauma-informed, client-driven, and culturally competent care. Must have ability to exercise discretion, independent judgment, and critical thinking skills. Maintain effective working relationships and engage in conflict resolution, as needed. Enthusiasm for the work, creative problem-solving skills, and a spirit of honesty and self-awareness. Knowledge of mental/behavioral health issues as they relate to transitional age youth. Other Requirements: Reliable transportation, a valid California driver's license, and a good driving record. Use of a personal, properly insured vehicle with mileage reimbursement. Position Summary The Program Manager is responsible for overseeing the Enhanced Care Management Team, Community Supports provider, and Case Managers. Under the supervision of the Program Director, the Program Manager is responsible for managing the daily operations of North County Lifeline's CalAIM and CinA programs. This position is responsible for program oversight, daily operations, monthly billing, budgets and supervision of all service staff. The Program Manager is responsible for providing leadership and guidance to the staff in the Youth Development department and to serve as a liaison between the Youth Development department and other departments within Lifeline, and between Lifeline and the community. Oversees and operates within program budgets and monitors the use of wraparound fund accounts. Provides Quality Assurance including file reviews, and oversight of documentation of assessment and intake, case management and progress notes, case planning and goal setting, completion of all necessary documents. Participates in and facilitates agency and component meetings. Stays informed of and disseminates to staff information related to the Enhanced Care Management Field. Serves as liaison between Youth Development programs and other Lifeline programs (i.e S4S, IMPACT, CinA, VIVA, Connections, CCH) to promote in-house program coordination. Assists with the recruitment, hiring, training and scheduling of program staff. Monitors staff productivity and provides constructive feedback and coaching. Prepares and conducts staff performance reviews annually or as needed and ensures adherence to agency policies and procedures including disciplinary protocols if the need arises. Qualifications/Requirements Master's degree in counseling, social work, or related field. Licensed LMFT/LCSW preferred. Two years experience supervising counseling and/or case management staff. Assessment services may include every family member (parents and/or children) living in the home and will be conducted in the family home or at other locations as directed by client or County CWS staff. Specific Activities Provides culturally competent, trauma informed assessment services for CSF families referred by Child Welfare Services and community referred families. Provide assessments within 3 business days of receipt of referral Prioritize cases by case type in as stated in the CSF contract Conduct assessments both in person and occasional virtual sessions in the office and in the community Evaluate all members of the family living in the home for domestic violence, substance abuse, mental health, trauma, and other significant needs. Make recommendations on the basis of assessment findings for preventive or treatment services to mitigate exposure to trauma (for children) and to mitigate/treat issues negatively impacting child safety (for parent/caregivers). Provide initial recommendations for treatment, mental health and/or other services to County Social Worker within 3 business days of completing assessment. Complete and submit a written follow up report of assessment findings to referring party within 10 business days including assessment tools used, outcomes for each family member, type and level of services recommended, likelihood of a successful treatment outcomes, formal and informal interventions to strengthen family functioning to ensure child safety, prevention services to mitigate future effects of current child trauma, Family Centered Meetings attended, CSF referrals, and other information. Participates in Child and Family Team Meetings to discuss client progress in services and support them in achieving their case plan goals. Participates in Multi-Disciplinary Teams with County staff as needed. Participates in quarterly CSF countywide peer review process to discuss service delivery and improve upon best practices. Maintains ongoing communication with the CSF team to ensure quality of service delivery. Maintains detailed case files and documentation utilizing County approved forms and formats. Produces monthly reports of activities and submits to the CSF Supervisor. Remains culturally sensitive and respectful in all client and staff interactions. Participates in required staff meetings, collaborative meetings, trainings, County meetings, supervision, QA processes, and all other meetings and events as requested. Completes other duties as assigned. Training provided Qualifications and Requirements LMFT/LCSW/LPCC eligible by the California Board of Behavioral Science plus 2 years of related experience in direct client service. Training in and mastery of family systems theory such as Structural Family Therapy and application of diagnostic criteria. Strong skills in interviewing, rapport building, and assessment. Certification in Domestic Violence Assessment and Treatment provided once hired. Ability to make sound clinical decisions independently and to apply clinical judgment to developing effective treatment recommendations. Ability to focus on multiple priorities and meet deadlines.
