Position Summary Under the general supervision of the LifeSpring Program Manager, the Case Manager plays a critical role in assisting youth and young adults who are at risk of homelessness or experiencing homelessness. This role combines housing navigation with intensive case management and skill development, ensuring that services are delivered through a youth-driven, whole person, trauma-informed model. The position supports participants in achieving independence, obtaining safe housing, and accessing necessary resources, all while adhering to a strength-based, culturally relevant, trauma-informed, and outcome-based approach. Specific Activities: 1. Client Support and Engagement: Create a welcoming, supportive environment to engage youth effectively and to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence. Provide basic needs assistance, including food, clothing, hygiene products, and access to essential services. Conduct intakes, eligibility screenings, basic needs assessments, and administer tools like the TAY-VI-SPDAT. Offer crisis management, safety planning, and emergency support as needed. Empower youth to make informed decisions by providing options and supporting their autonomy. 2. Housing Navigation: Create a housing plan with tangible goals and milestones to work towards housing and self-sufficiency. Assist and partner with youth to finding housing options and securing stable, inclusive housing. Assist youth with understanding and signing leases. Also provide support with housing applications, security deposits and other documents related to securing housing. Build and maintain relationships with local housing providers, such as landlords, property managers, and other entities. Utilize the Coordinated Entry System to connect youth with appropriate housing resources. 3. Case Management and Skill Development: Provide intensive case management, including goal setting, life skills training, economic literacy, and employment readiness. Help build and support participants' relationships with family and community with an emphasis on permanent connections. Facilitate skill development opportunities such as meditation, public benefits advocacy, tenancy, and employment training. Coordinate care with mental health professionals, primary care providers, outside social service organizations, and other supportive services. Assist with job search, educational goals, and development of life plans supporting independence. 4. Community Engagement and Outreach: Initiate and organize outreach efforts to engage youth and identify service gaps. Participate in county collaborative meetings and community partner discussions to create best practices. Provide training and outreach to enhance program visibility and effectiveness as needed. 5. Administrative Duties and Compliance: o Maintain accurate and timely documentation of client progress and interactions in Efforts to Outcomes (ETO). o Track goals, compile data, and complete reports for funding sources. o Uphold confidentiality and privacy standards in accordance with HIPAA. o Perform other administrative tasks, including document storage, auditing, and work order submissions. 6. Program Development and Coordination: Collaborate with other LifeSpring staff to screen and interview prospective participants. Assist in program development and resource identification to enhance service delivery. Participate in program meetings, group supervision, trainings, and other collaborative activities as needed. Required Education and Experience: Bachelor's degree in social work or a related field, with at least 3 years of experience, or Master's degree and registered intern status with the BBS, preferred. Experience working with transitional age youth, particularly those experiencing homelessness or at risk, and familiarity with former foster youth and CSEC/HT populations. Strong skills in engagement, crisis intervention, case management, and goal setting. Excellent verbal and written communication skills, computer literacy, and organizational abilities. Knowledge, Skills, and Abilities: Advanced knowledge of trauma-informed, client-driven, and culturally competent care. Must have ability to exercise discretion, independent judgment, and critical thinking skills. Maintain effective working relationships and engage in conflict resolution, as needed. Enthusiasm for the work, creative problem-solving skills, and a spirit of honesty and self-awareness. Knowledge of mental/behavioral health issues as they relate to transitional age youth. Other Requirements: Reliable transportation, a valid California driver's license, and a good driving record. Use of a personal, properly insured vehicle with mileage reimbursement.     Job Description Case Manager, Youth Development Services Under the supervision of the Youth Development Program Manager, utilize a team approach to provide comprehensive case management, counseling and educational services to youth on probation and their families participating in the YDS program. Services include, but are not limited to, in-home support, parent training, crisis intervention, counseling, case management, conducting placement assessments, transporting participating youth, and community education and development. The Case Manager is expected to function as part of a multi-disciplinary team while making sound, independent decisions for the benefit of the clients and their families. Due to the nature of the workflow and client needs, the YDS Case Manager must be available to work schedules that include days, evenings, and weekends to best meet needs of the population served. Position Summary The Program Manager is responsible for overseeing the Enhanced Care Management Team, Community Supports provider, and Case Managers. Under the supervision of the Program Director, the Program Manager is responsible for managing the daily operations of North County Lifeline's CalAIM and CinA programs. This position is responsible for program oversight, daily operations, monthly billing, budgets and supervision of all service staff. The Program Manager is responsible for providing leadership and guidance to the staff in the Youth Development department and to serve as a liaison between the Youth Development department and other departments within Lifeline, and between Lifeline and the community. Must demonstrate knowledge of trauma informed, client driven, and culturally competent care. The Program Manager will assure that all services provided to our clients and families are implemented through a client driven wellness and recovery model that is strength and outcome based, culturally relevant, and trauma informed. Specific Activities Assures that the program consistently provides high quality, trauma informed, professional services. Assures timely, accurate and appropriate documentation. Provides administrative daily/weekly supervision to program staff. Assists with the recruitment, hiring, training and scheduling of program staff. Monitors staff productivity and provides constructive feedback and coaching. Prepares and conducts staff performance reviews annually or as needed and ensures adherence to agency policies and procedures including disciplinary protocols if the need arises. Engages in component and program development, implementation, and evaluation. Sets parameters for documentation requirements. Responsible for the onboarding of new staff, training and compliance of HIPPA laws Facilitates weekly or bi-weekly Multi-disciplinary Team Meetings and provides individual supervision to staff on a consistent basis. Maintains statistics and meets reporting requirements for CinA, and CalAIM programs on the specified database, including program-specific ETO database. Under the direction of the Program Director, coordinates or provides substance abuse and mental health services training, and other related staff development activities. Works closely with the fiscal department to ensure accurate and timely submission of all staff Absence Request forms, Mileage Sheets, Timesheets and any other contract/budget requirements. Develops, supervises and maintains systems and procedures that encourage and support a multi-disciplinary wraparound service model which ensures timely and appropriate critical core services for youth, adults and families, including information and outreach to the community, assessment referral and follow up, case coordination and management, home visiting, rapid response to crisis, drug and alcohol counseling and gender-specific services for females. Oversees and operates within program budgets and monitors the use of wraparound fund accounts. Provides Quality Assurance including file reviews, and oversight of documentation of assessment and intake, case management and progress notes, case planning and goal setting, completion of all necessary documents. Participates in and facilitates agency and component meetings. Stays informed of and disseminates to staff information related to the Enhanced Care Management Field. Resolves programmatic issues as identified by staff. Attend required meetings and training as appropriate. Participates in other duties as assigned. Requirements/Qualifications Licensed LMFT, LCSW required. Bilingual Spanish preferred. Must possess the ability to make sound clinical decisions.     Clinician - Human Trafficking Prevention Specialist, Project LIFE Lifeline Community Services is hiring a Clinician (Human Trafficking Prevention Specialist) to support our growing team. You will be responsible for the following: providing human trafficking prevention education (groups and workshops) to high risk youth; clinical services to youth and young adults that have experienced human trafficking; and facilitate prevention education and support to parents of high risk youth and those that have experienced victimization. Prevention Specialist will also participate in outreach activities to high-risk girls and young women and participate in the rotation for 24 hour emergency response to the San Diego Human Trafficking Task Force. This is a master's level and BBS registered clinical role that will help our team provide trauma informed, evidence based interventions to survivors of human trafficking and those at significant risk. This role requires positive attitude, integrity, confidence, creativity and reliability, since your day-to-day work will be managing a caseload and working with the team to further develop the program and ensure high quality services to clients. Specific Activities Provides culturally competent, trauma informed assessment services for CSF families referred by Child Welfare Services and community referred families. Position Overview Lifeline Services is hiring a mental health clinician to support our growing team. The mental health will be responsible for providing comprehensive trauma-informed school-based suicide Prevention and Early Intervention (PEI) services in the North Inland and North Coastal regions. The HERE Now Program Clinician is responsible for providing education and trainings to school staff and gatekeepers to include outreach and school-wide suicide prevention education, risk assessments, individual intervention services, and care coordination with area mental health support and services. Specifically, will work with district/school employees, youth, and parents in creating an overall school environment conducive to appropriate learning for students, promoting positive mental health, stigma reduction, and suicide prevention. This is a BBS registered role that will help our team implement suicide prevention and early intervention services to schools throughout inland and coastal North County area. We're looking for a candidate who is able to connect with youth and families using culturally sensitive and trauma informed practices. This role requires creativity, flexibility, and teamwork, since your day-to-day work will be as part of a team at schools and will require the ability to make sound clinical decisions. Key Responsibilities Implementation, and delivery of culturally-competent services on middle and high school campuses and in community locations that serve children, youth and transition aged youth (TAY), with suicide prevention and early intervention services in a culturally-sensitive and culturally-competent manner. Present an evidence-based curriculum, Signs of Suicide (including bullying prevention), to 7th- 12th grade students with the purpose of reducing risk and enhancing protective factors as well as facilitate mental health and safety trainings for parents/caregivers, district staff and community members. Plan, coordinate and conduct trainings to school staff and gatekeepers. Provide one-on-one safety assessments and crisis management to at-risk students and their families, while utilizing a trauma-informed care philosophy and recovery principles. Work with the school to connect high-risk youth to selected level (psychosocial group) and individual linkages. As needed, provide individual intervention services to youth identified as "at-risk" based upon assessment. Care coordination with area mental health support and services as well as other needed services for youth/ families with depression, other mental health problems, substance abuse or whose history of trauma would indicate a high risk for suicide or self-harm. Work in and through the County of San Diego CSOC Principles. Work collaboratively and cooperatively with staff and volunteers within the program, the agency, and the community. Participate in all required trainings and consultation/supervisions. Attend all required agency and county trainings and meetings. Maintain accurate and timely uniform record documentation. Maintain data collection and contract compliance of records as required. Maintain clear, effective and open communication. Work with school personnel and families to reduce disparities and stigma in accessing mental health services and to provide high quality services to youth and families.
