Position Summary Under the general supervision of the LifeSpring Program Manager, the Case Manager plays a critical role in assisting youth and young adults who are at risk of homelessness or experiencing homelessness. This role combines housing navigation with intensive case management and skill development, ensuring that services are delivered through a youth-driven, whole person, trauma-informed model. The position supports participants in achieving independence, obtaining safe housing, and accessing necessary resources, all while adhering to a strength-based, culturally relevant, trauma-informed, and outcome-based approach. Specific Activities: 1. Client Support and Engagement: Create a welcoming, supportive environment to engage youth effectively and to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence. Provide basic needs assistance, including food, clothing, hygiene products, and access to essential services. Conduct intakes, eligibility screenings, basic needs assessments, and administer tools like the TAY-VI-SPDAT. Offer crisis management, safety planning, and emergency support as needed. Empower youth to make informed decisions by providing options and supporting their autonomy. 2. Housing Navigation: Create a housing plan with tangible goals and milestones to work towards housing and self-sufficiency. Assist and partner with youth to finding housing options and securing stable, inclusive housing. Assist youth with understanding and signing leases. Also provide support with housing applications, security deposits and other documents related to securing housing. Build and maintain relationships with local housing providers, such as landlords, property managers, and other entities. Utilize the Coordinated Entry System to connect youth with appropriate housing resources. 3. Case Management and Skill Development: Provide intensive case management, including goal setting, life skills training, economic literacy, and employment readiness. Help build and support participants' relationships with family and community with an emphasis on permanent connections. Facilitate skill development opportunities such as meditation, public benefits advocacy, tenancy, and employment training. Coordinate care with mental health professionals, primary care providers, outside social service organizations, and other supportive services. Assist with job search, educational goals, and development of life plans supporting independence. 4. Community Engagement and Outreach: Initiate and organize outreach efforts to engage youth and identify service gaps. Participate in county collaborative meetings and community partner discussions to create best practices. Provide training and outreach to enhance program visibility and effectiveness as needed. 5. Administrative Duties and Compliance: o Maintain accurate and timely documentation of client progress and interactions in Efforts to Outcomes (ETO). o Track goals, compile data, and complete reports for funding sources. o Uphold confidentiality and privacy standards in accordance with HIPAA. o Perform other administrative tasks, including document storage, auditing, and work order submissions. 6. Program Development and Coordination: Collaborate with other LifeSpring staff to screen and interview prospective participants. Assist in program development and resource identification to enhance service delivery. Participate in program meetings, group supervision, trainings, and other collaborative activities as needed. Required Education and Experience: Bachelor's degree in social work or a related field, with at least 3 years of experience, or Master's degree and registered intern status with the BBS, preferred. Experience working with transitional age youth, particularly those experiencing homelessness or at risk, and familiarity with former foster youth and CSEC/HT populations. Strong skills in engagement, crisis intervention, case management, and goal setting. Excellent verbal and written communication skills, computer literacy, and organizational abilities. Knowledge, Skills, and Abilities: Advanced knowledge of trauma-informed, client-driven, and culturally competent care. Must have ability to exercise discretion, independent judgment, and critical thinking skills. Maintain effective working relationships and engage in conflict resolution, as needed. Enthusiasm for the work, creative problem-solving skills, and a spirit of honesty and self-awareness. Knowledge of mental/behavioral health issues as they relate to transitional age youth. Other Requirements: Reliable transportation, a valid California driver's license, and a good driving record. Use of a personal, properly insured vehicle with mileage reimbursement. Position Summary The Program Manager is responsible for overseeing the Enhanced Care Management Team, Community Supports provider, and Case Managers. Under the supervision of the Program Director, the Program Manager is responsible for managing the daily operations of North County Lifeline's CalAIM and CinA programs. This position is responsible for program oversight, daily operations, monthly billing, budgets and supervision of all service staff. The Program Manager is responsible for providing leadership and guidance to the staff in the Youth Development department and to serve as a liaison between the Youth Development department and other departments within Lifeline, and between Lifeline and the community. Must demonstrate knowledge of trauma informed, client driven, and culturally competent care. The Program Manager will assure that all services provided to our clients and families are implemented through a client driven wellness and recovery model that is strength and outcome based, culturally relevant, and trauma informed. Specific Activities Assures that the program consistently provides high quality, trauma informed, professional services. Assures timely, accurate and appropriate documentation. Provides administrative daily/weekly supervision to program staff. Assists with the recruitment, hiring, training and scheduling of program staff. Monitors staff productivity and provides constructive feedback and coaching. Prepares and conducts staff performance reviews annually or as needed and ensures adherence to agency policies and procedures including disciplinary protocols if the need arises. Engages in component and program development, implementation, and evaluation. Sets parameters for documentation requirements. Works closely with the fiscal department to ensure accurate and timely submission of all staff Absence Request forms, Mileage Sheets, Timesheets and any other contract/budget requirements. Develops, supervises and maintains systems and procedures that encourage and support a multi-disciplinary wraparound service model which ensures timely and appropriate critical core services for youth, adults and families, including information and outreach to the community, assessment referral and follow up, case coordination and management, home visiting, rapid response to crisis, drug and alcohol counseling and gender-specific services for females. Oversees and operates within program budgets and monitors the use of wraparound fund accounts. Provides Quality Assurance including file reviews, and oversight of documentation of assessment and intake, case management and progress notes, case planning and goal setting, completion of all necessary documents. Participates in and facilitates agency and component meetings. Stays informed of and disseminates to staff information related to the Enhanced Care Management Field. Resolves programmatic issues as identified by staff. Attend required meetings and training as appropriate. Participates in other duties as assigned. Specific Activities Duties include but are not limited to: Creates and monitors client's charts, assures that administrative documentation is complete and correct. Prepares official reports that are submitted to San Diego County Behavioral Health Services. Utilizes SmartCare, Optum, MHoms and DCR systems to run reports, analyzes and enters data to prepare contract documents Maintains office files and establishes, develops, and maintains filing system. Maintains and organizes HIPAA rooms in compliance with industry standards Screens program participants for Medi-Cal eligibility and other Health Insurance Assists in referral and intake tracking process. Coordinates design of data tracking systems to respond to changing contract requirements. Works with BH Admin Manager and CCC Program Manager to integrate contract-required administrative and financial documentation into the program's assessment process. Prepares brochures, letters, enrollment and resources packets and mailings accurately Data input/management utilizing Microsoft Access/Excel or other data management software Completes work order requests, provides some office management Trains new clinical staff on program administrative processes, including assessment and discharge administrative documentation. Assists with orientation for new staff and arranging Lifeline password accesses, equipment, and all staff registrations required by the BBS, BHS, and HHS. May supervise and train Administrative Volunteer Serves as program's IT liaison. Participate in all required meetings and trainings Attend agency, collaboration, and community meetings as needed Other duties as assigned Requirements BA, AA or HS diploma + 2 years directly related experience. Bilingual (English-Spanish) preferred. Nonprofit experience preferred. Knowledge of and respect for diverse cultures. Accurate spelling, grammatical usage and excellent communication skills. Knowledge of MS Word, including strong formatting skills. Good working knowledge of MS Excel, Outlook, and other current software as necessary. Knowledge of SmartCare, Optum, MHoms and DCR systems are a plus. Requires strong time management skills, attention to detail, and organizational skills. Ability to perform tasks in an organized manner with accuracy and attention to detail. Required proficiency in basic word processing, spreadsheet and database programs. Reliable transportation.
