This person will be the clinical partner within the wellness clinic you will play a vital role in delivering high-quality nursing care in a supportive and collaborative setting. ESSENTIAL DUTIES: -Respond swiftly to rapid response calls and assess the patient's condition upon arrival. -Perform a thorough evaluation, including taking vital signs, reviewing patient history, and identifying signs of deterioration. -Implement appropriate emergency interventions, such as advanced airway management, intravenous access, and medication administration. -Provide critical care procedures, such as chest compressions and defibrillation, if necessary. -Draw blood and process lab test -Set-up and monitor EKGs -Prepare and administer injections -Supervise direct care of members. -Manage medical records -Patient Scheduling -Provides direct care as necessary. -Provides basic wound care including cleaning and dressing wounds. -Provides and receives inter-shift reports timely and efficiently to ensure good communication about the needs of members and the community. -Monitor and record patient vital signs including temperature, blood pressure, pulse and restoration rates. -Keeps physicians and families aware of changes in member's condition. -Participates in health teaching and counseling to promote and maintain optimum health of member. -Maintains accurate documentation of all care provided. -Transcribes and provides orders as necessary. -Review all the medications for availability, expiration and proper documentation and notifies the appropriate guide if discrepancies are noted. -Works with members, family, and pharmacy to maintain adequate medication levels. -Maintain the self-respect, personal dignity, and physical safety of each member. -Respond to independent living member emergency calls if indicated. -Participates in family /member counseling as necessary. -Completes chart check weekly to ensure compliance with written physician orders. COORDINATION AND COMMUNICATION: Communicate effectively with the primary care team, including nurses, physicians, and other healthcare professionals, to coordinate care and ensure continuity. Provide clear and concise handoff reports to the attending physician or primary care team regarding the patient's condition and any interventions performed. Educate and involve family members or caregivers in the situation, providing updates and addressing concerns as appropriate. SKILLS AND QUALIFICATIONS: -Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) or equivalent. -Strong clinical assessment skills and ability to make rapid, accurate decisions in high-pressure situations. -Excellent communication and teamwork skills, with the ability to collaborate effectively with diverse healthcare professionals. -Ability to handle stressful situations with composure and professionalism. REQUIRED BASIC QUALIFICATIONS: -LPN is currently licensed by the State of Georgia. -At least 6 months of nursing experience with geriatric involvement preferable. -Must be able to work in self-directed work groups -Passion for working with elderly people.
