Empath Health is currently seeking a Licensed Practical Nurse (LPN) to join our Continuous Care (CC) team. Three 12 hour shifts with alternating weekends - days to be determined. 8pm - 8:30am. This position covers our 4 county service area including Manatee, Sarasota, Charlotte and DeSoto.
Under physician's orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.
Primary tasks, duties, and responsibilities:
1. Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
2. In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.
Requirements:
Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.

Operational
