This first-of-its kind model removes barriers to accessing care for our underserved communities, providing walk-in/urgent care and evidence-based outpatient treatment for mental health and substance use disorders.
CHA's Department of Psychiatry is one of the primary departments of the Harvard Medical School (HMS) Department of Psychiatry. Faculty are eligible for HMS appointment at the Instructor, Assistant or Associate Professor level, commensurate with HMS appointment criteria, candidate qualifications and commitment to teaching responsibilities for HMS students, residents, and fellows. The department also has an active program of research.

· Development of enhanced clinical care of patients experiencing homelessness starting with infectious disease, STI, and HIV services and expanding to other clinical areas of opportunity
QUALITY STANDARDS:
· Attend at least 2/3 of provider and staff meetings.
· Active participation in 1 Performance Improvement team or 1 community outreach project per year. Active participation means minimum meeting attendance of 6 meetings each year and making concrete, constructive contribution to the team or community group. Alternatively, provider may do an independent quality improvement project, approved by the CMO and CEO in advance.
· Encounters correctly completed and submitted electronically by next business day. All medical record documentation is complete by next business day. All health center documentation is electronic.
· At least 90% compliance with electronic problem list, medication list, prescribing and pain assessment standards based on chart review or electronic reports.
· Compliance with UDS and ACO quality metrics.
· Cooperates with efforts of the Patient Care Committee to monitor and improve the quality of patient care provided.
· Compliance with regulatory guidance.
· In collaboration with the Homeless Services LPN/MA/Phlebotomist, maintain clinic logs and documentation including:
o Refrigerator temperature logs for lab area and medication room
o Equipment cleaning logs
o Code cart and code bag checks in collaboration with the NP
o Oxygen log
o AED log
COMMUNICATIONS:
· Demonstrates sensitivity and responds appropriately to verbal and non-verbal communications of patients, families, co-workers, and the Father Bill's and Mainspring team.
· Maintains patient's right to privacy, assuring dignity in the provision of care and is discreet in all communications, written and oral, about the patient's care.

MINIMUM SKILL AND KNOWLEDGE REQUIREMENTS:
· Current Massachusetts independent licensure for RN in expanded role. Certification eligible as Primary Care Practitioner. Federal and State licensure for prescriptive writing privileges.
· Ability to develop and work with a multi-disciplinary team, including providers, social workers and case managers.
· High comfort level with prescribing buprenorphine and other medications for addiction treatment
· Fluency in Spanish, Portuguese, French, or Haitian Creole is highly desirable. Previous experience in ambulatory care and/or public health preferred.
· Ability to suture, perform incision and drainage, give immunizations and start intravenous lines beneficial. Ability to administer nasal naloxone necessary.
· Ambulatory care experience in an urban environment serving diverse, low-income populations experiencing complex medical and psychosocial challenges
· Understanding of how systemic racism, white supremacy, and other forms of bias affect patient engagement, retention, and health outcomes
· Experience working in multi-disciplinary team-based care and one or more of the following: trauma-informed care, health care for the homeless, harm reduction, or chronic care management models

Key Responsibilities

* Infusion/day treatment- Deliver clinical coverage in an ambulatory clinic serving over 8,000 patients annually, including treatments such as biologic infusions, blood products, food challenges, and day treatment for newly diagnosed diabetes, migraine management, sickle cell fever and pain, and central line complications
* Independently managing infusion and acute reactions
* Serve as a primary provider, facilitating patient care including medical histories, physical examinations, diagnostic evaluation, and development and implementation of evidence-based treatment plans for both new and return patients
* Collaborate closely with physicians, nursing teams, and interdisciplinary colleagues to promote optimal clinical outcomes and patient safety.
