Requirements
· College Degree from an accredited school of nursing, current license to practice as an RN in the Commonwealth of Massachusetts

Job Description
Employee Health Nurse (20 hours)
Responsible for coordination and management of employee health program to include: assessment of employee's health status, ensuring prospective employees, volunteers, students and interns are able to carry out their proposed functions and are appropriately protected from major communicable diseases. Major focus is coordinating and conducting the pre and ongoing health screening to include N95 Fit Testing, TB screening and ensuring all applicable immunizations are up to date and/or verification of immunity demonstrated. Monitoring and tracking employee flu vaccine status and encouraging vaccination.
Access Nurse (20 hours)
The Access Nurse assesses clinically new patients and integrates the patient into Lowell CHC primary care and/or other services to ensure high quality, cost effective care along the continuum. The goal of access nurse is to facilitate access and to promote/maintain optimal care for patients, to assist linking the patient with the next available provider and to enhance revenues by appropriate management of resources. The access nurse will facilitate access for high volume, high-risk patients, i.e., diabetics, asthmatics, and those seen in Emergency Room and/or recently discharged from the hospital.
Qualifications
Graduate of an accredited School of NursingRegisteredNurse License in the Commonwealth of Massachusetts
Associates of Science in Nursing required. Bachelor of Science in Nursing strongly preferred.

Job Description
SUMMARY:
Full Time (40 hours per week).
Some evenings and weekends required, inclusive in the 40-hour work week.
The Care Coordinator/Community Health Worker (CHW) will be an integral member of an interdisciplinary Behavioral Health and Primary Care Integration team in Pediatrics and Family Medicine Departments. The CHW will work closely with patients, care managers, other care management team members, health care providers, behavioral health and social services providers, and community partners to effectively manage the care of designated patients.
REQUIREMENTS:
Care Management
Assist Children's Mental Health Initiative (CMHI) team in monitoring and evaluating patients' needs, including for prevention, wellness, medical, specialist, and behavioral health treatment; care transitions; and social and community service needs. Support the development and execution of patients' care plans, including assisting patients in understanding care plans and instructions and tailoring communications to appropriate health literacy levels.
Conduct initial and periodic needs assessments, including assessing barriers and assets (e.g., transportation, community barriers, social supports); patient and family or caregiver preferences; and language, literacy, and cultural preferences.
Promote patient treatment adherence through assessing patient readiness to make changes; assisting patients and families in making changes to daily routines; identifying barriers; and assisting patients with developing strategies to address barriers.
Provide informal counseling, behavioral change support, and assistance with goal setting and action planning.
Provide support for chronic disease self-management to patients and their families.
Provide information on patients to care managers, other care team members, and providers.
Manually and/or electronically document activities and patient information and interventions in patient-tracking systems, care management software programs, and other program systems.

POSITION SUMMARY:
The Triage RegisteredNurse (RN) plays a key role in supporting providers and delivering direct patient care within a patient-centered, team-based environment. Job Description
The Access Nurse assesses clinically new patients and integrates the patient into primary care and/or other services to ensure high quality, cost effective care along the continuum. The goal of access nurse is to facilitate access and to promote/maintain optimal care for patients, to assist linking the patient with the next available provider and to enhance revenues by appropriate management of resources. The access nurse will facilitate access for high volume, high-risk patients, i.e., diabetics, asthmatics, and those seen in Emergency Room and/or recently discharged from the hospital.

Essential Functions:
· Assesses an individual's basic health status and related patient care needs for patients who call the clinic or who present for scheduled or unscheduled care

· Provides health education to individuals and families to further patient self- management and self-empowerment and to help patients maintain/sustain an optimal level of health
· Communicates with members of the patient's care team outside of the health center as needed, for instance, with specialists or visiting nurses, to help with the coordination of care
