This will be an hourly position with wage up to $45/hr (based on experience); Monday-Friday 1st shift. #NSHCRN Summary/Objective: Responsible for planning, organizing, developing, and directing the nursing care of the hospice patient in accordance with current federal, state, and local standards, guidelines, and regulations. Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions: * Assisting the admission nurse in the coordination of timely admissions * Provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient's care. * Assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. * Assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues * Educate patient and family on the disease process and expectation related to the disease progression * Provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual, and financial comfort. * Participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs * Ensure patient care is delivered at the appropriate level of care * With coordination of PCP and medical director, provide management of medication profile to prevent polypharmacy and ensure optimal symptom management * Manage medication profile to ensure hospice is providing medications related to disease process and symptom management * Ensure all services provided are in accordance with the patient's Plan of Care * Ensure the POC involves the active participation of the patient, family, Primary Care Physician, and the Interdisciplinary Team * Provide team members with appropriate information to coordinate all services related to hospice care for assigned patients. Coordination of services include communicating and making assignments for visiting nurses, home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as, ordering medications and Durable Medical Equipment, and supplies * Provide consultation and supervision to all CNAs and LPNs involved in the care of the patient * Participate in the performance evaluation of clinical members of LPN(s) and CNA(s) involved with the care of case load. * Assist with the orientation of clinical members participating in the care of the assigned caseload * Ensure cost effective practices by self and others * Assist with QAPI (Quality Assurance and Performance Improvements) activities * Work as a team member at all times and uphold the Hospice Code of Ethics * Provide care to manage patient * Utilize the medication formulary whenever possible for the management of patient symptoms * Triage scheduling and symptom management calls from members of IDT * Reports to the Clinical Director as patient condition changes or change in the patient condition that require change in level of care Work Environment/Physical Requirements The responsibilities of this position involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling and twisting. The employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements. The employee may experience a change in work conditions related to weather issues, traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs. Required Education and Experience * Registered Nurse graduated from an accredited program currently licensed and in good standing with the Wisconsin State Board of Nursing. Preferred Education and Experience * One to two years of experience in nursing in an acute care setting and 1 year experience in home health, hospice, or comparable experience is preferred. * Desire to discover and demonstration of aptitude for the hospice philosophy and comfort care. * Must have the ability to work cooperatively with other disciplines including social workers, chaplains, CNA's, and volunteers. Key Responsibilities: Resident Care and Satisfaction: Promotes quality of care and services for residents in assigned communities, including their physical, emotional, and social well-being. Monitors resident and family concerns to validate prompt and effective resolution. Staff Development, Training, and Delegation: Collaborates with senior leadership and community Executive Directors to promote the alignment of nursing practices with organizational goals and standards. Identifies training needs and coordinates ongoing education and professional development programs for assisted living staff. Mentors and develops a team of clinical staff, fostering a culture of excellence and continuous improvement. Supervises, orients and competency tests direct care staff to assisted living and to each resident's individual needs. Supervises medication practices and medical equipment being used by assisted living tenants/residents. Summary of position Nurse will assume the Medicare technical oversight and managed care responsibilities onsite and or in a remote capacity in a cluster of centers. They will be accountable for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process. This process includes managing the completion of the comprehensive resident assessments; care coordination and planning; staff teaching; and adherence to the RAI guidelines. Conducting meetings, tracking payors, reviewing skilled criteria and handling the technical areas of Medicare and Managed Care are core job components. This is a manager position overseeing multiple local Centers.
