Summary/Objective:

Summary/Objective:
The Post-Acute Director of Care Coordination is responsible for supporting highquality, coordinated transitions of care from the acute care setting to skilled nursing facilities (SNFs), home health, hospice, outpatient therapy, or other post-acute care environments. The Care Coordinator collaborates with interdisciplinary teams, patients, families, and post-acute providers to ensure safe, timely, and appropriate transitions that reduce avoidable readmissions and improve patient outcomes.
Essential Functions:

* Provide referral / Intake support and education to post-acute providers to ensure continuity of care and adherence to treatment plans.
* Track patient progress post-discharge and follow up on care milestones, including therapy participation, medication reconciliation, and follow-up appointments
* Monitor for early signs of deterioration or gaps in care and intervene to prevent unnecessary readmissions.
* Assist with advanced care planning discussions and documentation, when appropriate.
* Participate in performance improvement initiatives related to transitions of care, quality metrics, and value-based care programs (e.g., ACOs, BPCI-A).
* Collect and analyze post-acute care data to identify trends, gaps, and opportunities for improvement.
* Serve as a resource and clinical educator for SNF partners on post-hospitalization care standards, documentation best practices, and care coordination.

Summary of position
The primary purpose of your job position is to plan, organize, develop, coordinate, and direct our infection prevention and control program and its activities in accordance with current federal, state, and local standards, guidelines, and regulations that govern such programs, and as may be directed by the Executive Director, the Director of Nursing, and the Infection Prevention and Control Committee to ensure that an effective infection prevention and control program is maintained at all times.

Summary of position
Responsible for the direction and administration of the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA and other payor program requirements. Ensures assessments accurately reflect the physical, mental and psychosocial status of each resident; ensures accuracy of RAI to reflect optimal reimbursement for services provided; ensures appropriate documentation to report and support services provided and assessment accuracy.
Requirements of the position
Required Education and Experience
* Registered Nurse (RN) in good standing with respective state licensing board
* Clinical experience, education or specialty skills specific to geriatrics
* Demonstrated competence in PPS assessment as well as OBRA compliance
* Excellent clinical assessment skills
* Excellent organization and independent decision-making skills
* Knowledgeable and understands state and federal regulations and guidelines governing the practices of the center
* Excellent interpersonal skills
* Computer proficiency with Microsoft products
Preferred Education and Experience
* 2+ years of previous experience

* Post-acute care or health care experience preferred

Summary of Position:
Under the supervision of the Unit Manager/ADON/DON, the RN assumes responsibility and accountability for a group of residents/patients for a shift of duty. Nursing care is provided through assessment, implementations, and evaluation of the plan of care. The RN adheres to the standards of care for the area, manages the environment to maintain resident/patient safety, and supervises the resident/patient care activity performance by nursing assistants and helpers. Follows all North Shore's policies and procedures. Performs duties as defined by the State Nurse Practice Act. Follows where applicable JCAHO standards.
Who We're Looking For
* An RN with an active license and a heart for elder care
* Someone who values teamwork, accountability, and pride in their work
* Must have and maintain current CPR certification
* Able to use necessary supplies and equipment (feeding pumps, oxygen, treatment trays, medication cart, etc.).
* Able to calculate drug dosage
* Meets all health requirements imposed by law
* Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other center staff
* Able to read, write, speak and understand English
* Demonstrated basic computer knowledge and ability with an aptitude to learn company applications
