Activity Assistant Job Summary As an Activity Assistant, you'll work with the Engagement Director to make our residents' lives more enjoyable with fun and engaging activities. Your role involves creating experiences that cater to their physical, intellectual, and emotional well-being, making our community vibrant and lively. Responsibilities:
Work with the Engagement Director to plan and run a variety of activities, like exercise programs, interesting sessions, art projects, trips, spiritual gatherings, and events with speakers and music.
Connect with residents personally, understanding what they like and need to make sure activities are enjoyable and meaningful.
Help plan trips outside the community, organize details, and make sure residents are safe and happy.

The Senior Clinical Advisor (SCA), under the general supervision of the Nurse Manager collaborates to provide continuity of care by ensuring adequate resources are available on a shift to shift basis.
The SCA collaborates with the healthcare team to plan and coordinate timely admission, transfer and discharge of patients.
Creates an environment that ensures the delivery of safe and high quality patient care , high patient satisfaction, positive patient outcomes.

Experience: ICU/Critical Care, Telemetry, or Intermediate/Step Down nursing experience from an Acute Care Hospital. Progressive intermediate/step-down or critical care nursing experience including charge nurse experience and supervision. Knowledge of national standards, i.e. DPH, JC, CMS. Knowledge of current nursing, medical and health trends/practices.

Responsibilities:
Assists the manager in planning and developing programs and activities that support habilitation, wellness and recreation. Coordinates delivery of daily activities with fellow Activities staff.
Delivers daily activities directly to participants at ADH, in participants' homes and via telehealth.
Daily travel between sites and visits to participants' homes. Assists with transportation safety.
Provides coverage at other ADH sites as needed.
Implements appropriate interventions at ADH and participants' homes.
Develops and writes periodic participant assessments, progress notes, and care plans according to organizational policies. Documentation is completed timely.
Creates and maintains tracking spreadsheets. Compiles information for inclusion in monthly calendars, newsletters, and other reports.

Qualifications:
Must have successfully completed an O. T. A. curriculum approved by the Accreditation Council for Occupational Therapy Education, Inc. with a minimum of an Associate's Degree.
Maintains current Occupational Therapy Assistant licensure in the Commonwealth of MA
Maintains current CPR Certification
Minimum of 1-2 years of experience working with a geriatric population.
Minimum of 1-2 years of experience in program development.

Activities Assistant Qualifications:
High school diploma or equivalent.
Knowledge of arts and crafts, preferably in a geriatric setting.
Activities Assistant Certification or willingness to be certified (at St. Patrick's Manor expense) within 6 months.
Computer literate with training on entering resident care plan codes (MDS), preferred.
Must be able to speak, write and understand English.
Activities Assistant Working Conditions:
Bending, standing, sitting, lifting, walking and moving intermittently during working hours.
Must assist in the evacuation of residents as necessary.
May need to lift serving trays of up to 30 pounds.
Activities Assistant - Life Enrichment Job Summary:
Assist in planning, organizing and implementing the activities programs of the facility.

Position Summary:
The Activities Assistant delivers the recreational and social programing, in accordance with our residents' individual assessment and care plans. Plans and coordinates group social activities/events for the residents at the facility.
Primary Responsibilities:
Assist in planning, developing, organizing, implementing, and evaluating the activity program
Interview residents or family members to obtain activity information
Involve the resident/family in planning activity programs when possible
Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident's response to the service
Must be able to drive the community vans and assist residents with transfers on and off the vans.
Transport residents in the facility's van to and from recreational outings and shopping trips.
Assist in the development of and participate in regularly scheduled orientation and in-service training programs that relate to the activity department
Assist in developing a written plan of care (preliminary and comprehensive) for each resident's activity program that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified
Schedule and coordinate FaceTime and Zoom virtual visits with residents and their families
Monitor and escort groups during Music Concerts, and to Activities on their appropriate floor.
Other responsibilities as deemed necessary and appropriate, or as may be directed by the Activity Director
May be required to help run special programs on occasional evenings or weekends, as needed
Minimum Qualifications:
Must possess, as a minimum, a high school diploma.
Therapeutic Recreation Certification preferred.
Minimum of one (1) year experience in long-term care or assisted living working directly with a geriatric population.
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, procedures, etc. of the department.
Computer literacy in MS Office Suite including Word, Excel, Outlook, Internet and other programs utilized to research, track and create materials for use in resident activities
Must be able to drive the community vans and assist residents with transfers on and off the vans.
Must possess excellent communication skills and be able to relate professionally and positively to residents, family members, volunteers, and facility staff.
Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies. Demonstrates organizational skills and ability to coordinate and plan.

Responsibilities
ESSENTIAL FUNCTIONS:
Complete activity assessment through patient observation and patient/family interview, obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan.
Plan, organize, direct and implement the activities within the treatment program to facility patient progress with treatment goals.
Plan and implement an activities program including but not limited to therapeutic leisure skills and activities, leisure education and leisure awareness.
Facilitate and lead activity sessions to improve patient mental and physical well-being.
Instruct patients in activities and techniques, such are sports, dance, music, art or relaxation techniques designed to meet their specific needs.
Develop treatment plans and implement activity interventions that meet patient needs and interests and helps the patient achieve his/her treatment goals.
Engage patients in therapeutic activities, such as exercise, games and community outings.
Help patients learn social skills needed to become or remain independent.
Modify activities to suit the needs of specific groups.
Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
OTHER FUNCTIONS:

Qualifications
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
Bachelor's Degree in Therapeutic Recreation, Recreational Therapy or related field required.
Master's degree preferred.
Three or more years' experience in recreation in a healthcare setting with knowledge of patient population served by the facility required.
Previous experience in recreation in a healthcare setting is preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
Certified Therapeutic Recreation Specialist (CTRS) or currently in the process of obtaining national certification; OR certified in the area of specialty (Art, Dance, Music, etc.) OR clinical professional licensure (LCPC, LPC, etc.) with certification in expressive therapy area required.
CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
First aid may be required based on state or facility requirements.
Pay Range: $30.44 to $36.35/hr.
