Provides respite and/or continuous care for patients/caregivers when applicable and as directed by the RN/Patient Care Manager.
Documents care rendered according to service procedures and forwards to designated personnel within 24 hours of visit with 95% accuracy per record review.
Completes 12 hours HHA in-service/education programs plus additional In-services yearly.
Drives vehicle throughout designated service area, adhering to standards of safe driving practices, as evidenced by report of DMV.
Participates in Performance Improvement activities as directed by the Patient Care Manager.
Reports problems/concerns regarding patients to RN/Patient Care Manager.
Assists patients in achieving their ADLS goals.
Contributes information to the development of the POC by reporting changes in patients and caregivers functional level and abilities.

Elective orthopedic joint replacement surgeries (hip, knee, shoulder)
Neurosurgical surgeries (neck, back, brain)
Orthopedic trauma in times of high census
Medical overflow in times of high census Patients receive physical, emotional, spiritual and psychosocial support for themselves and their families. N4G collaborates with a Joints in Motion program which provides comprehensive pre and post op education for the total joint population.

Other medical and surgical patients in times of high census
Nurses provide care and monitoring for patients with IV patient-controlled analgesia. Nurses are competent in the care of patients with the following: nephrostomy tubes, continuous bladder irrigations, biliary drains, Jackson Pratt drains, Constavac drains, epidurals, chest tubes, tracheostomy tubes, and peritoneal dialysis. Nurses who have completed competencies may access and de-access Portacaths, and perform negative pressure wound dressing changes.
