DEPT. SCOPE OF SERVICE:
The Department of Radiology at Brattleboro Memorial Hospital is a consultative service that will provide services in the following categories: diagnostic radiology, nuclear medicine, ultrasound, computed tomography, interventional radiography, MRI, bone density and mammography. The department offers services 24 hours a day, seven days a week to inpatients, outpatients and emergency room patients of all ages. Whereas x-ray procedures constitute the majority of our service, we also provide related quality control (including protecting patients and staff from harmful radiation), quality assurance monitoring and evaluation, marketing and continuing education of staff and patients. Radiology is comprises of licensed technologists, certified and licensed nuclear medicine technologists, secretarial staff and licensed radiologists. We render reports of all of our imaging studies to referring physicians within 24 to 36 hours of completion of the exam.

* Formal AMA approved school of Radiologic Technology required.
Experience:
* Previous, recent experience in all areas of radiologic technology, in a hospital setting required.

Responsible for coordinating home care and hospice services on weekend days and managing a home care and hospice caseload on weekdays. Coordinates weekend nursing schedules, patient care and intake of referrals for home care and hospice. Plans, organizes and directs home care and hospice services and is experienced in nursing, with emphasis on community health education/experience. Builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Supervisory Responsibilities:
This position has no direct supervisory responsibilities.
Essential Job Functions/Responsibilities:
* COORDINATION
* Communicates with the Referral Source to facilitate the process of admission of the person into the agency.
* Obtains all necessary referral information, including, but not limited to, physician ordered medications and treatments (signed by provider) and enters information into electronic medical record.
* Ensures patient meets eligibility requirement for certain admission types and if does not, assists referral source with exploring alternative options.
* Communicates with the patient or family when necessary to give an accurate description of the services the patient is referred to.
* Establishes and maintains positive working relationships with customers and referral sources and responds to customer requests and concerns.
* Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Northern Counties Health Care.
* Monitors and responds to weekend tele-monitoring data.
* Responds to calls from the answering service on Saturday and Sunday, 8am to 4pm.
* Coordinates patient care and communicates scheduling changes to nurses.
* DIRECT CARE
* Completes an initial assessment of patient and family to determine home care needs and regularly re-evaluates patient nursing needs.
* Initiates the plan of care and makes necessary revisions as patient status and needs change.
* Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
* Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act.
* Counsels and educates the patient and family in meeting nursing and related needs.
* Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
* Acts as Case Manager when assigned by Nurse Manager and assumes responsibility to coordinate patient care for assigned caseload.
* Participates in care conferences and in-services.
* COMMUNICATION
* Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
* Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
* Communicates with community health related persons to coordinate the care plan.
* ADDITIONAL DUTIES
* Participates in on-call and holiday duties.
* Ensures that arrangements for equipment and other necessary items and services are available.
* Instructs, supervises and evaluates home health aide care provided in accordance with state and federal regulations.

Position Qualifications:
* Registered nurse currently licensed in the state of Vermont. Home Care and Hospice experience preferred.

* Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
* May occasionally be in a position to lift and/or carry an item weighing up to 50 pounds.

The BMH medical staff includes more than 130 board-certified providers in primary care and many other specialties, with a shared mission of providing exceptional health care delivered with compassion and respect. The Brattleboro Memorial Hospital Health System includes the BMH Medical Group, a multispecialty group practice of primary care and specialty care physicians, nurse practitioners, and physician assistants. Over 110 active volunteers assist BMH's 700 employees in caring for our community.
A Hospitalist Physician is responsible for the general medical care of hospitalized patients. The role includes diagnosing, treating, and coordinating care for patients during their hospital stay, working closely with specialists, nurses, and support staff to ensure high-quality, evidence-based care. The candidate should have excellent clinical judgment and diagnostic skills, strong communication and interpersonal skills. The candidate is committed to patient-centered, evidence-based care.
Specialty backup includes surgeons to perform central line insertions, respiratory therapists, Emergency Medicine physicians and CRNAs. Brattleboro Memorial Hospital will consider J1-Visa candidates.
KEY RESPONSIBILITIES:
* Provide direct care and management for hospitalized patients, including admissions, daily rounds, and discharges.
* Evaluate, diagnose, and treat a wide range of acute and chronic medical conditions.
* Coordinate and collaborate with specialists, primary care physicians, and ancillary services.
* Order and interpret diagnostic tests, imaging, and laboratory work.
* Document patient encounters, treatment plans, and outcomes in the electronic medical record (EMR) in a timely and thorough manner.
* Respond to emergencies and provide resuscitative care as needed (e.g., codes, rapid response).
* Participate in quality improvement initiatives, hospital committees, and performance evaluations.
* Provide education and guidance to patients and their families regarding medical conditions and care plans.
* Ensure compliance with hospital policies, medical ethics, and regulatory standards.

Requirements
QUALIFICATION
EDUCATION:
* Medical degree (MD or DO) from an accredited medical school.
* Board Certification or Board Eligibility in Internal Medicine or Family Medicine.
EXPERIENCE/SKILL SETS:
* 2-3 years' experience as a Hospitalist preferred
* Experience in rural health preferred but not required.
* Vent management experience required.
* Experience running codes.
LICENSURE/CERTIFICATIONS:
* Active an unrestricted Vermont State medical license.
* Valid Vermont DEA license
* BLS, ACLS certification
