Join the growing Occupational Therapy team in this fast-paced level 1 trauma hospital. OTs provide evaluations, treatments, and discharge planning to patients with a variety of medical conditions and injuries. The practice of Occupational Therapy shall include assessment, treatment, planning, implementing, education, communication and demonstration of leadership to address the needs of patients and their families as well as to foster a positive working relationship among professionals and ancillary staff. This person may be caring for infants, children, adolescents and/or senior patients and will have the knowledge and skills to care for the physical and developmental needs of these populations. The OccupationalTherapist will practice within defined standards of care and direct the practice of support staff in order to achieve the patient's functional goals. S/he is expected to communicate effectively with other professional and support staff in order to promote a positive, problem-solving environment.
Responsibilities
Care Management* Provides appropriate supervision to supportive personnel (OTA's, techs, students) while assuming responsibility for care of patient.
* Attends and actively participates in patient team conferences and family conferences.
* Determines needs for follow up outpatient therapy and makes post-discharge recommendations.
Documentation* Documents within EMR and charting accurately reflects a logical flow between data collected, action taken, and patients response to the action.
* Plans specific measures to meet assessed learning needs and documents in patient record.
* Recognizes and addresses patient, family, and appropriate others in the learning needs of the individual.
* Presents information clearly and in a manner appropriate to the level of patient and family understanding.
* Implements the treatment plan and evaluates and records the patient responses to the treatment sessions.
* Participates in the development of individual discharge plans that are responsive to patient/family concerns.
* Able to revise/update plan based on assessment.
* Completes documentation per department policy.
Education* Acts as a resource person to peers within the rehabilitation department, fielding questions, mentoring, or providing educational opportunities.
* Acts as a resource person to other departments and community, fielding questions, mentoring, or providing educational opportunities.
Patient Care* Reviews patient's history and appropriate data prior to initiating evaluation.
* Evaluate and treat patients without a physician order or referral as established by Board of Medical Directors Rule.
* Assists patients and families in planning for discharge.
Compliance/Safety* Responsible for reporting any safety-related incident in a timely fashion through the Midas/RDE tool; attends all safety-related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.
* Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.
* If required for the position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.
* Completes all company mandatory modules and required job-specific training in the specified time frame.
* Completes charges in a timely manner and according to proper procedures.
* Meets productivity goals according to departmental standards.
* Attends 75% of department monthly meetings in a 12-month period.
* Responds to new orders promptly in compliance with department policy.
* Adheres to regulatory and organizational standards affecting patient care and safety as well as all hospital and departmental.
* Responsible for 100% of department meeting content including reviewing minutes, asking questions as needed, and reviewing handouts.

Essential Functions & Responsibilities:
Promotes the mission, vision and values of BMHD.
Assists the physician in the evaluation, diagnosis, and prognosis of patient by applying muscle, nerve, joint, and functional ability testing. Develops the occupational therapy treatment plan based on these results and revises as necessary, assisting the physician in developing the plan of care for the patient.
Treats patients safely to relieve pain, develop, or restore function to optimal ability. Directs and aids patients in active and passive exercises, muscle re-education, activities of daily living, transfer activities, functional training, prosthetic training, safety and education. Provides education for safe and effective participation in I/ADL's implementing adaptations and modifications when appropriate.
Provides ongoing assessment and analysis of patient functioning and progress. This therapy involves providing treatment which takes into account the risks and benefits of that treatment regime relevant to the patient's medical condition, rehabilitation potential and prognosis.
Provides timely and complete documentation following department guidelines and compliance requirements. Follows any indicated HIPPA, Medicare, Medicaid as well as state, organizational and department rules.
Coordinates therapy activities with other health care disciplines. This includes coordinating services through consultation with appropriate agencies to ensure follow-up and continuing care as needed.
Participates in quality and performance improvement activities within the department including but not limited to measurement and continuous improvement of clinical competency, regulatory compliance, outcome achievement, resource management, work team performance and customer satisfaction.
Education Requirements:
Bachelor's and Master's Degree
Graduate from an Accredited School of Occupational Therapy
Certifications/Licensures:
Basic Life Support (BLS) from American Heart Association (AHA)

POSITION SUMMARY
This position is responsible for the evaluation, planning, administration and supervision of Occupational Therapy treatment in accordance with physician referral, state and local organizational standards, regulatory agencies, and established departmental policies and procedures. This position is onsite at Banner University Medical Center, South Tucson campus and will float to Banner University Medical Center, Tucson. It is regular full-time 40 hours per week. It is 5 days a week from 8:00am-4:30pm.

Banner - University Medical Center South is a comprehensive academic medical center that includes an Emergency department, a state-designated trauma center and a Behavioral Health Pavilion. We are an Arizona Department of Health Services-accredited Cardiac Receiving Center and a Nurses Improving Care for Health system Elders-designated senior-friendly hospital. The hospital is staffed by physicians who are full-time faculty of the University of Arizona College of Medicine - Tucson and is managed by Banner Health under an operating agreement with Pima County. Our specialty services include inpatient and outpatient behavioral health, treatment and education for diabetes, innovative geriatrics care and comprehensive orthopedics.
POSITION SUMMARY
This position is responsible for the evaluation, planning, administration and supervision of Occupational Therapy treatment in accordance with physician referral, state and local organizational standards, regulatory agencies, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served.
CORE FUNCTIONS
1. Evaluates patient's functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals.. Completes comprehensive evaluation within the established time frame set by the department. Identifies and prioritizes key limiting factors. Establishes treatment goals that are functional, measurable, patient related and reflect the key limiting factors. Collaborates with patient and family/caregiver when setting goal Initiates discharge planning. Recommends additions to or modifications of referring orders.
2. Implements individualized treatment plan with consideration to the patient's physical, social, spiritual, cultural, educational and age specific needs. Provides skilled treatment in a safe manner.. Assesses ongoing effectiveness of treatment and modifies plan of care as indicated. Involves patient and family/caregiver in the treatment and decision making process. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team.
3. Documents according to standards set by the department and regulatory agencies. Documents legibly. Completes documentation within the established time frames of the department. Documents innovative and individualized treatment techniques/plan. Documentation reflects comprehensive assessment of patient's progress or lack of progress. Treatment goals are reassessed according to department policies
4. Communicates pertinent patient information to ensure the best possible follow through of care. Communicates evaluation/treatment results and patient progress to physicians, nurses, patient, family/caregiver, co-workers and the interdisciplinary team. Communicates in a clear and timely manner. Accepts responsibility to contact physicians for routine or difficult problem solving to ensure optimal patient outcome. Defends treatment rationale with standard practices and/or current literature
5. Exhibits adaptability in assignments as it relates to caseload, scheduling, and staffing shifts. Takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs.
6. Demonstrates appropriate direction and delegation of personnel to achieve safe delivery of quality cost effective patient care. direction and delegation of the COTA, Therapy Technician and student complies with department policies and regulatory agencies. Participates in the training and orientation of new personnel. Assures competency of personnel, prior to delegation of task.
7. Department operational activities. Participates in department/facility process improvements. Meets productivity standards set by the department.
8.This position has responsibility for interacting with all levels of staff in a variety of departments, physicians, patients, families, and external contacts such as employees of other health care institutions, community providers and agencies concerning the health care of the patient. Also interacts with the physician offices in order to report, requires or clarify information
MINIMUM QUALIFICATIONS
Must have successfully completed the academic requirements of an educational program in occupational therapy approved by the appropriate governing board. Must possess a current Occupational Therapy license from the State of practice.
Current license from state of practice. BLS certification required.
Relates throughout the interview process their clinical competency, experience, training and education they have received that is required to perform the job. Demonstrates through behaviors and answers to job related experiential questions.
Must have the ability to become proficient with computer software applications commonly used in an automated patient care environment.
Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment.

POSITION SUMMARY
This position provides assessment, treatment, and education to cardiac and pulmonary patients and their families in an outpatient cardiac, pulmonary rehabilitation or inpatient setting. Writes exercise prescriptions to progress patients through phase two and three outpatient rehabilitation.
CORE FUNCTIONS
1. Evaluates patient's functional abilities and limitations to determine appropriate intervention. Develops a plan of care and exercise goals, as ordered by physician or other referral source. Works alongside and receives guidance from other more experienced team members when working in phase I and II cardiac or pulmonary rehabilitation. May oversee phase III outpatient cardiac and pulmonary rehab independently.
2. Implements individualized exercise plans based on patients' physical and age-specific needs and abilities. Ensures that treatment plans are supported by standard practices and/or current literature. Reevaluates treatment plans, as needed, to ensure safety and maximum patient benefit.
3. Educates patients, family, and other care providers post cardiac to promote patient participation and attainment of exercise program goals. Provides individualized home programs with clear instructions. Instructs patients and families in pathology, discharge guidelines, risk factor modification, and exercise individually and through classes for both cardiac and pulmonary patients.
4. Under general supervision, documents patient treatment according to professional and regulatory standards.
5. Follows plan of care to accomplish functions within established procedures and scope of practice. Confers with lead or supervisor on any unusual situations.
6. Utilizes basic understanding of medical devices, equipment function and quality control measurements. Adheres to safety policies. Demonstrates the ability to trouble shoot and notifies appropriate personnel to maintain equipment performance. Maintains a safe environment for patients, staff and visitors.
7. Complies with all ethical, legal and professional responsibilities. Maintains confidentiality of all patient information.
MINIMUM QUALIFICATIONS
Requires a level of clinical knowledge as normally obtained through Bachelor's degree in Exercise Physiology or related field.
Minimum experience of a 300 + hour internship in cardiac rehabilitation or equivalent clinical/patient hours.
Requires knowledge in cardiac/pulmonary physiology, management and treatment of chronic disease, health promotion, ECG interpretation.
Requires the ability to work collaboratively with physicians and other healthcare providers. Must have strong interpersonal, communication and human relation skills.
Basic Life Support (BLS) certification is required.
PREFERRED QUALIFICATIONS
Master's degree in Exercise Physiology or related field
Advanced Cardiac Life Support (ACLS) certification
600+ hour internship in outpatient cardiac and pulmonary rehabilitation
Additional related education and/or experience preferred.

Documentation* Documents within EMR and charting accurately reflects a logical flow between data collected, action taken, and patients response to the action.
* Plans specific measures to meet assessed learning needs and documents in patient record.
* Recognizes and addresses patient, family, and appropriate others in the learning needs of the individual.
* Presents information clearly and in a manner appropriate to the level of patient and family understanding.
* Implements the treatment plan and evaluates and records the patient responses to the treatment sessions.
* Participates in the development of individual discharge plans that are responsive to patient/family concerns.
* Able to revise/update plan based on assessment.
* Completes documentation per department policy.
