FlexStaff is seeking an experienced Registered Nurse for a Field RN position for our client, a leader in home health care. This position will cover cases in Westchester County, NY.
This position will manage all aspects of patient care related to services provided in the home.
Responsibilities:
Conduct homecare assessments
Perform home visits
Complete all required documentation
Communicate effectively with Primary Care Physicians (PCPs)
Update medication profiles
Coordinate care with other providers
Review and complete case communication notes
Participate in discharge planning activities
Other duties as assigned
Requirements:
Active New York State (NYS) RN License
Home Health Care experience

The STAT Nurse makes independent decisions/judgments and takes action regarding a wide range of routine and complex patient problems and situations based on protocol. The STAT Nurse acts to support patient care needs in any acute medical/surgical unit, intensive care unit, and Emergency Department.
The STAT team is a group of critical care trained professional nurses who are available 24/7 for any issue within the hospital and medical college. STAT nurses provide specialized care to all patients, employees, and visitors requiring acute medical attention at Albany Medical Center and Albany Medical College. The team members also contribute their knowledge to educate staff, patients, and families to recognize clinical deterioration and how to make a proactive assessment and observation for patients at risk for deterioration. STAT team members give MOCK code training for nurses and ancillary staff to help facilitate better outcome in code situations.
STAT Nurses respond to:
Code stroke, Code Thrombectomy, Code STEMI, Code 10, Anesthesia Emergency, Code Blue, Code Malignant Hyperthermia, Code Pink, and Rapid Response pages. Any employee with questions about hospital policies, procedures, or patient care.
STAT Nurses will:
Use problem focused physical exams to evaluate and treat patients according to protocols.
Stabilize decompensating patients on the floor and in ICUs.
Manage ICU patients on the floor.
Facilitate movement of patients to higher level of care when needed.
Use the chain of command when resolving issues.
Assist visitors and staff to ED after a Code Gray.
Administer IV push medications on applicable floors (ex. Cardizem, Amiodarone, Hydralazine).
Education Requirements:
Graduate of a professional academic nursing program. Holds current RN Registration to practice in the State of New York.
Experience:
A minimum of three (3) years recent Intensive Care clinical experience in specific patient populations served. BLS/ACLS certified.
Communication Skills:
Communicates effectively with patients, family members, employees, and others. Listens actively and verifies and clarifies messages. Seeks the input of others to achieve consensus. The STAT Nurse makes independent decisions/judgments and takes action regarding a wide range of routine and complex patient problems and situations based on protocol. The STAT Nurse acts to support patient care needs in any acute medical/surgical unit, intensive care unit, and Emergency Department.
The STAT team is a group of critical care trained professional nurses who are available 24/7 for any issue within the hospital and medical college. STAT nurses provide specialized care to all patients, employees, and visitors requiring acute medical attention at Albany Medical Center and Albany Medical College. The team members also contribute their knowledge to educate staff, patients, and families to recognize clinical deterioration and how to make a proactive assessment and observation for patients at risk for deterioration. STAT team members give MOCK code training for nurses and ancillary staff to help facilitate better outcome in code situations.
STAT Nurses respond to:
Code stroke, Code Thrombectomy, Code STEMI, Code 10, Anesthesia Emergency, Code Blue, Code Malignant Hyperthermia, Code Pink, and Rapid Response pages. Any employee with questions about hospital policies, procedures, or patient care.
STAT Nurses will:
Use problem focused physical exams to evaluate and treat patients according to protocols.
Stabilize decompensating patients on the floor and in ICUs.
Manage ICU patients on the floor.
Facilitate movement of patients to higher level of care when needed.
Use the chain of command when resolving issues.
Assist visitors and staff to ED after a Code Gray.
Administer IV push medications on applicable floors (ex. Cardizem, Amiodarone, Hydralazine).
Education Requirements:
Graduate of a professional academic nursing program. Holds current RN Registration to practice in the State of New York.
Experience:
A minimum of three (3) years recent Intensive Care clinical experience in specific patient populations served. BLS/ACLS certified.
Communication Skills:
Communicates effectively with patients, family members, employees, and others. Listens actively and verifies and clarifies messages. Seeks the input of others to achieve consensus. Applies problem solving, negotiating, and conflict resolution skills into communication. Utilizes Chain of Command, as needed, to reach optimum outcomes. Job Description
Implements physician orders, administers medications, starts IVs, performs treatments, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations.
Orders, interprets, and evaluates diagnostic tests to identify and assess patients' conditions.
Assesses and evaluates patient needs for, and responses to, care rendered.
Applies sound nursing judgment in patient care management decisions.
Provides primary and emergency care for occupational and non-occupational injuries and illnesses.
Administers over-the-counter and prescription medications as ordered.
Collaborates with the nursing team to create a Plan of Care for all patients.
Directs and guides ancillary personnel and maintain standards of professional nursing.
Minimum Qualifications
Graduate of a professional academic nursing program in which a Diploma, Associate Degree or Baccalaureate Degree is conferred
Must hold current NYS Registered Nurse license
Obtains and maintains certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)
Minimum of 1-3 years of previous clinical RN experience in Med-Surg or Progressive Care
Excellent communication, prioritization, organizational and time-management skills
Possesses knowledge of quality concepts, principles, and problem-solving tools and techniques and demonstrates ability to apply in support of departmental and patient care quality improvement.

Job Description:
Assist patients in reaching outcomes as established by the RPN in the plan of care. 2026 Student Nurse Externship Program

Home care nurse job at Northwell Health
Northwell Health's Summer Nurse Externship program provides Junior BSN students with an 8-week paid preceptorship. Eligible nursing students will have a graduation date between December 2026- May 2027, and must be available for the entirety of the 8-week program. Externships are available in a variety of inpatient nursing units and specialties across the health system; preferred areas of interest will be considered, with final determination of placement made by the host site.
Multiple locations available system wide.
Responsibilities
1. Assists Registered Nurse by performing selected procedures and treatments.
May document in the patient's medical record under the supervision of designated RN.
Reports any abnormal findings, related to patient, to RN for further action.
Takes and records temperature, pulse, respirations, weight and blood pressure.
Measures and records fluid intake and output.
Obtains specimens, i.e. urine, stool, sputum, blood, documents collection.
Tests finger sticks for glucose and records results.
Administers enemas/high colonies and records results.
Performs treatments, i.e. dressing changes, pressure ulcer care, trach care, suctioning and records results under supervision of RN.
Checks potency of drainage tubes (foley catheter, NG tubes) and records results.
Assist with tube feedings after validation of tube placement by RN.
Monitors I.V. fluids and tubing.
2. Maintains patient care units in a neat and orderly condition.
Changes and makes beds.
Strips patient units and equipment upon discharge of patient, in preparation for cleaning by Housekeeping Department.
Prepares room for new admission and postoperative patients.
Maintains equipment in clean and orderly condition.
3. Assists Registered Nurse in preparing patient for meals, and feeds or assists patients with meals.
4. Performs a variety of duties including, but not limited to …
Assisting patient in and out of bed; lifts, turns, and positions patients.
Transporting and accompanying patients to various departments, as directed.
Assisting with post-mortem care and in transporting body to morgue.
Attending educational programs to augment clinical experiences.

Required Experience and Qualifications:
High School Diploma or equivalent, required.

Basic Life Support certification through American Heart Association
Must be available for the entirety of the 8-week program, from June 4 through July 31.
