As part of the Interdisciplinary Team, the Hospice Nurse Case Manager provides care to terminally ill patients and their families at home on the west side of Baltimore County including Catonsville, Halethorpe, Pikesville and Owings Mills. He/she is responsible for identifying patient/family care needs, and for providing nursing and supportive care. The Home Care Nurse reports to the Team Manager. Education: * Diploma, ADN and/or BSN from an accredited institution Licensures/Certifications: * Current Maryland State license as a Registered Nurse Experience: * Two years of nursing experience, or equivalent combination or education and experience.
