The RN Transplant Coordinator is a licensed professional nurse responsible for managing and coordinating all aspects of care for solid organ transplant patients and living donor candidates across the continuum-from referral and evaluation through waitlist management, inpatient care, surgery, and post-transplant/post-donation follow-up. The coordinator ensures patient-centered, regulatory-compliant, and efficient delivery of care by serving as a central liaison among patients, living donors, families, referring providers, and the multidisciplinary transplant team. Coordinators are assigned a focus area (Pre-Transplant, Waitlist, Post-Transplant, Inpatient, Outreach, or Living Donor) while maintaining accountability to the broader transplant program's quality goals and compliance standards (UNOS/OPTN, CMS, Joint Commission, and institutional policies). This role supports high-quality, safe, and ethical care for both transplant recipients and living donor candidates.
Outreach Coordinator
* Organize and manage outreach evaluation clinics across multiple locations.
* Facilitate pre-transplant evaluation processes, record reviews, testing, and referrals.
* Serve as liaison with dialysis centers, community providers, and outreach partners.
* Provide education and support for patients, families, and dialysis staff.
* Present outreach cases to the multidisciplinary selection committee.
* Support marketing, education, and community engagement initiatives.

Interdisciplinary Collaboration & Communication:

* Communicate care plans, testing needs, and clinical updates across inpatient and outpatient settings and ensure seamless transitions throughout the transplant and donation processes.

* Collaborate with the HLA laboratory on crossmatching, donor preselects, match reviews, and NKR-related logistics.
* Facilitate clear communication regarding candidacy, closure of referrals, and evaluation outcomes with transparency and consistency.
* Promote continuity, safety, and consistency of care across all phases of transplantation and living donation.
Education & Counseling:
* Provide individualized and group education regarding the transplant process, testing, surgery, medications, and long-term expectations.
* Facilitate informed consent discussions and reinforce patient and caregiver understanding.
* Support education on medication adherence, infection prevention, and self-management strategies.
* Assist in developing educational materials and participate in community or outreach education efforts as assigned.

This is due to the special care and education necessary for transplant patients. Nursing care is concentrated on recovery from transplantation, focusing on mobility, monitoring of I & O's, pain management and medication administration and education. Transplant patients are always recovered and monitored in the SICU prior to coming to 8CCP. Transplant patients can also be admitted from TACU, clinic, or directly from home. This unit consists of patient populations from many specialized surgical teams. Our patient population is admitted from the ER, SICU and PACU. Nursing care for the Trauma patient is concentrated around recovering from traumatic injury with focus on mobility, patient education, monitoring intake and outputs and pain management. Our surgical specialties include ENT and urology who are consistently nationally ranked by U.S. News & World Report.
Trauma: UCMC is the area's only Adult Level 1 Trauma Center in the region. Our patients arrive from injuries sustained by motor vehicle crashes, gunshot wounds and unforeseen accidents.
Ear Nose & Throat (ENT): UCMC has a large head & neck program that includes patients with head & neck cancer or other diseases. We see many different types of procedures including reconstruction of the airway, neck and face.
Urology: Surgeries and procedures related to urological cancer or other diseases, which may include treatment of prostate issues, urostomies, urinary diversions, among many other procedures.

Responsibilities
The RegisteredNurse (RN) is an individual who holds a current state license to practice nursing. The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. The RN utilizes knowledge derived from the principles of biological, physical, behavioral, social and nursing sciences to assess, plan, implement and evaluate patient care. The care is culturally based and age-specific.
Prepares, implements, evaluates, revises and coordinates interdisciplinary plan of care for each assigned patient in a collaborative effort with the other healthcare team members and patient/family within the time frame indicated by unit's standards of practice.
Communicates formulated plan of care, as appropriate, to all levels of associates involved in the patient's care, and demonstrates ability to establish priorities of care based on patient's assessment and needs.
Gives clear instructions to patients/family regarding treatment, and involves family/guardian in the assessment, initial treatment, and continuing care of the patient.
Identifies and responds appropriately to different patient needs resulting from behavioral health problems, unique psychological needs or those associated with religious/cultural norms.
Demonstrates critical thinking, accountability, flexibility, and a cooperative attitude in meeting the needs of the unit.
Nursing process is accurately and concisely documented, including evaluation of treatment and in accordance to hospital policy and procedure.
Acts as a resource person and mentor for new personnel, floater, and/or supplemental staff working on the unit.
Assumes responsibility for personal professional development and contributes to the professional development of peers, colleagues, and others.
Qualifications
Graduate from an accredited school of nursing
Bachelor of Science in Nursing, preferred
Current OH RN License
BLS/ACLS/PALS per department requirements based on patient population needs.
For employees working in UCMC Psychiatric Services, NCI-APS (Nonviolent Crisis Intervention with Advanced Physical Skills) Certification must be obtained within 90 days of hire.

Solid Organ Transplant/Renal is 24 bed unit. (1 bed designated as a pre-op bed for those patients being called in for a transplant) UCMC performs Liver, Kidney and Pancreas transplants. The kidney program has a living donor transplant program. All transplant patients, regardless of when their transplant was performed are admitted to 8CCP. This is due to the special care and education necessary for transplant patients. Nursing care is concentrated on recovery from transplantation, focusing on mobility, monitoring of I & O's, pain management and medication administration and education. Transplant patients are always recovered and monitored in the SICU prior to coming to 8CCP. Transplant patients can also be admitted from TACU, clinic, or directly from home. The patient ratio is 1:4 or 1:6 patients. This position will train. New graduates are welcome to apply!

This 28-bed unit works with the patient population admitted from the ER, PACU, and SICU. Nursing care is concentrated around recovering from traumatic injury with focus on mobility, patient education, monitoring intake and outputs and pain management. This unit will also be working with ortho patients recovering from total knee, hip, and shoulder replacements as well as spinal surgery cases. UCMC is the area's only Adult Level 1 Trauma Center in the region. Our patients arrive from injuries sustained by motor vehicle crashes, gunshot wounds and unforeseen accidents. We provide treatment of patients with severe and multiple fractures throughout the body, non-healing fractures, bone infections and other complex conditions.

Nursing care is concentrated on recovery from transplantation, focusing on mobility, monitoring of I & O's, pain management and medication administration and education. Transplant patients are always recovered and monitored in the SICU prior to coming to 8CCP. Transplant patients can also be admitted from TACU, clinic, or directly from home. Trauma: UCMC is the area's only Adult Level 1 Trauma Center in the region. Our patients arrive from injuries sustained by motor vehicle crashes, gunshot wounds and unforeseen accidents.
Ear Nose & Throat (ENT): UCMC has a large head & neck program that includes patients with head & neck cancer or other diseases. We see many different types of procedures including reconstruction of the airway, neck and face.

The Neuroscience Acute Care unit teamwork model allows close work with all members of the team to provide the highest level of acuity and care for all our patients.
UCMC has been named the Preferred Neuroscience institution in the area, and the Neuroscience Acute Care unit is also a Level 4 Epilepsy Center.

Job Responsibilities
The RegisteredNurse (RN) is an individual who holds a current state license to practice nursing. The practice of nursing requires specialized knowledge, judgment, and skills to provide care to groups and individuals. The RN utilizes knowledge derived from the principles of biological, physical, behavioral, social, and nursing sciences to assess, plan, implement, and evaluate patient care. The care is culturally based and age-specific.
Prepares, implements evaluates, revises, and coordinates interdisciplinary plan of care for each assigned patient in a collaborative effort with the other healthcare team members and patient/family within the time-frame indicated by the unit's standards of practice.
Communicates formulated plan of care, as appropriate, to all levels of associates involved in the patient's care, and demonstrates the ability to establish priorities of care based on the patient's assessment and needs.
Gives clear instructions to patients/family regarding treatment, and involves family/guardian in the assessment, initial treatment, and continuing care of the patient.
Identifies and responds appropriately to different patient needs resulting from behavioral health problems, unique psychological needs, or those associated with religious/cultural norms.
Demonstrates critical thinking, accountability, flexibility, and a cooperative attitude in meeting the needs of the unit.
The nursing process is accurately and concisely documented, including evaluation of treatment and in accordance with hospital policy and procedure.
Acts as a resource person and mentor for new personnel, floater, and/or supplemental staff working on the unit.
Assumes responsibility for personal and professional development and contributes to the professional development of peers, colleagues, and others.
