We are nestled in a historic district called Norwood Park on Chicago's North side.
Our Life Enrichment Associate must ensure that all activity procedures are followed in accordance with the established policies. He/She is responsible for carrying out planned activity programs and must have the ability to observe the response of persons participating in the group to determine modifications appropriate to the activity.

DUTIES
Duties include but are not limited to:
Assume the authority, responsibility and accountability of Life Enrichment Associate/ Activity Coordinator by planning, developing, organizing, implementing and evaluating Activity programs.
Observe resident attendance, mood, behavior, and degree of involvement so that facility activities and resident progress are evaluated by noting, reporting and charting resident behavior.
Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident's response to the service.
Maintain the confidentiality of all resident care information (HIPAA).
Maintain a neat and clean work environment.
Develop and maintain a good working rapport with inter-departmental personnel.
Coordinate activities with other departments as necessary.
Involve the resident/family in planning activity programs, objectives and goals when possible.
Participate in the community planning related to the interest of the facility and the services and needs of the resident and family.

Position Summary:
Provides social services support to the community while functioning as an advocate for the residents, family members, and continued success of the organization.
Social Services Essential Duties and Responsibilities:
1. Completes documentation in the resident's charts regarding pertinent information to conform to organizational policy, industry standards, along with required state and federal regulations and protocol.

Discharge Planner Essential Duties and Responsibilities:
1. Complete all required social service documentation in a timely manner.
2. Responsible for timely completion of MDS 3.0.
3. Complete a discharge plan and assessment within a 24 hour guideline.
4. Competent in written clinical documentation.
5. Able to care plan/ service plan behavior/mood or other issues with admissions.
6. Meet Medicare/Medicaid standards of Care Plans meetings and Notice of Medicare provider non-coverage.
7. Able to work well with families and resident during crisis and LTC adjustments.
8. Address and explain resident rights.
9. Able to assist and facilitate advance directives
10. Ability to make referrals and communicate with physicians and hospitals.
11. Able to complete paperwork as needed for completion of discharge.
12. Facilitate the facility Bed hold policy and Care Plan letters.
13. Able to intervene and liaison between staff, family, and resident.
14. Able to problem solve and defuse issues with residents.
15. A willingness to support and assist Social Services, Admissions, and other related disciplines.

21. Set goals for resident along with IDT for re-entering the community and home
22. Refer and work alongside of home health/ ancillaries/ equipment companies and hospices.
23. Ability to have work flexibility, especially during survey or IDPH to work longer hours if required.
24. Must interact on a professional level with the interdisciplinary team as well as residents, families, vendors, and physicians.
Qualifications:
· Bachelor's degree in psychology or related field.

Assists in the orientation of new residents' adjustment to community living
Maintains appropriate and complete reports, records, physicians orders, care plans, and progress notes for residents. Assures confidentiality of resident information
Maintains communication with physicians, families, and appropriate staff regarding resident status
Assists residents and families by
Offering support in dealing logistically and emotionally with the aging process and illness
Provides accurate information and timely updates to residents and families
Addresses questions and concerns promptly, or identifies appropriate parties/resources to do so
Assist and stimulates residents to participate in social and creative activities
Assists in coordinating services for resident being admitted to and discharged from Assisted Living, Health Center, and Hospital
Monitors infections and accidents of residents and notifies families and physicians when appropriate
Monitors private duty personnel in accordance with community policies and procedure, coordinates care with agency representatives
Responds with urgency to emergent health issues of our residents
Here are a few of the qualifications we need you to have:
Minimum two years clinical or case management experience with elderly population strongly preferred
Current Registered Nurse or Licensed Practical Nurse with active license by State of Illinois
Current CPR/BLS certificate
Comprehensive understanding of the English Language; must be able to read, write, and communicate in English
Physical Requirements
Able to walk long distances 50% of the work day
Must be able to lift and carry up to 30 lbs
Must be able to push a wheelchair occupied by a resident weighing up to 200 lbs. independently
Must be able to safely respond to emergent situations throughout the entire Independent Living building
