Master's of Divinity or graduate theological degree ordination or equivalent and endorsement by denominations or faith groups endorsing body.
Completion of four units of CPE from a nationally accredited body.
Experience: Minimum of two years Spiritual Care experience in a healthcare setting, preferred.
Roles requiring communication across disciplines, preferred.
Licensure/Certification: Eligible for certification by APC, CPSP, or other equivalent national certifying body, required.
Board certification completed within 3 years of hire date, required Responsibilities Deliver Spiritual Care: Respond to referrals across inpatient/outpatient settings; collaborate with chaplain team; use spiritual assessments to guide care; offer prayer, sacraments, and rituals as requested.
Provide Supportive Counseling: Offer emotional and spiritual support in person or via phone/video platforms.
Document Care: Record interactions and interventions in the electronic medical record and other designated tools.
Support End-of-Life Needs: Assist with Advance Directives; provide grief support; help plan funerals/memorials; conduct services when appropriate.
Consult on Psychosocial Issues: Serve on teams/committees; advise staff on complex emotional/spiritual concerns.
Support Staff Wellbeing: Respond to crises (e.

Qualifications
On an Average Day You Will:
(includes but not limited to)
Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
Provide direct spiritual care to patients and families/caregivers
Serve as liaison and support to community chaplains and spiritual counselors
Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision
Document direct services and ongoing communication with community chaplains and spiritual counselors
Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group
Provide consultation, education and support to interdisciplinary group on spiritual care
Recruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care
Provide for funeral or memorial services for patients as requested
Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team
Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers
Actively participate in quality assessment performance improvement teams and activities
Accept and perform other related duties and responsibilities, as required
Other duties as assigned
Requirements:
Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred
Must have experience working with death and dying, individuals/family/caregiver
Hospice experience preferred
Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.
Must be willing to travel to hospice locations as assigned and be flexible with working hours
