Job Description
The Geriatric Nurse Coordinator is responsible to maintain the NICHE program in Jersey City Medical Center by using the Age Friendly Health System s 4Ms Framework. This framework focuses on What Matters to the patient and their family, Medications, Mentation, and Mobility. The Geriatric Nurse Coordinator will establish evidence-based standards of care in collaboration with an interdisciplinary team. The Geriatric Nurse Coordinator will also work in partnership with the interdisciplinary staff to optimize the older adult s plan of care throughout the continuum of care and provide evidence base geriatric education to staff, patients, and their family through Geriatric Nurse Referrals. The Geriatric Nurse Coordinator is responsible for guiding and serving as a mentor to other Geriatric Resource Nurses (GRNs) and Geriatric Patient Care Associates (GPCAs) within the organization. The Geriatric Nurse Coordinator will co-chair the Geriatric Steering Committee with the organization s geriatrician; co-chair the Geriatric Resource Nurse Council. The Geriatric Nurse Coordinator is the disseminator of information about NICHE within his / her organization and is the primary contact for the NICHE national office.
Additional Requirements
Graduate from an accredited school of nursing, Bachelor s Degree required. Additional experience required based upon specific department needs. Current New Jersey RN licensure required. Inpatient geriatric experience preferred. Geriatric Nurse Resource training and Gerontological Board Certification required within 1 year of hire. Geriatric nursing assessment of referred patients, collaborating with interdisciplinary team including geriatrician to create a plan of care. Consultant to nursing staff to provide delirium assessment and interventions with emphasis on prevention of cognitive and functional decline. Collaborate with nursing staff in assessing patient, family, and age-specific nursing needs; help develop the interdisciplinary plan of care for complex patients. Participate in interdisciplinary team rounds and make recommendations as needed. Monitor patients' interventions and outcomes. Serve as a resource for nursing staff and assure coordination of NICHE Program goals among the NICHE designated units. Establish and address reasons for non-adherence and implement changes to improve services. Monitor recent literature and appropriately implement changes to improve services. Serve as a liaison between the NICHE Program, physicians, and other health care specialties in the hospital. Support GRNs and GPCAs in application of knowledge from training to bedside practice, and develop a plan of care for older adult patients. Ensures that GRNs receive 6 hours per year of continuing education in geriatrics. Design and implement educational programs with the education team to ensure the quality of staff knowledge and skills. Co-chairs the interdisciplinary Geriatric Steering Committee and assures continued support for the committee and regular communications with the committee to build expertise in care of the older adult throughout the organization. Co-chairs the Geriatric Resource Nurse Council and participation of GRNs in this shared governance council.

Qualifications:Degrees: Associates Licenses & Certifications: Pediatric Advanced Life Support Neonatal Resuscitation Program Basic Life Support Advanced Cardiovascular Life Support Registered Nurse Additional Qualifications: Associate degree in Nursing or Bachelors degree in Nursing. RNs hired prior to 2/2012 are not required to have a BSN to continue in their non-leadership role as an RN. RNs hired after 2/2012 with an Associate degree will have 5 years from their date of hire to complete the BSN degree. Life Support Certificate (BLS, NRP, PALS, ACLS) as per department standard. RN must complete a preceptor class within six months of hire. Minimum Required Experience: 2
