We are in search of experienced clinicians to join a UAS Home Care Project. In this role, you will be responsible for independent review and evaluation of completed UAS reassessments for appropriateness of long-term care services for consumers, in accordance with Medicare and Medicaid guidelines.
These are contractor positions ? which allow for flexibility in scheduling, at your own pace training, and pay at $100/hr.
In this role, you will be scheduled for in person appointments with members of the long term care population, who are applying for Managed Long Term Care services (MLTC). Your clients will have already completed a UAS evaluation with a Registered Nurse, and your role will be to determine whether MLTC and home care services are adequate for the patient?s care, or if they should instead be admitted to a supportive facility. Each appointment will last approximately 1.5 hours ? 30 minutes of preparation time, 30 minutes meeting with the member, and 30 minutes completing and submitting your determination. Additional compensation is offered for longer appointments.
Schedules are arranged according to your availability; however hours are not guaranteed. You must be willing to work at least 12 hours weekly, within operation hours which run Monday ? Friday 8AM ? 7PM, and Saturday between 10AM-6PM. Candidates who are unable to work during these operation hours, or who cannot commit to 12 hours weekly will not be considered.
Expect to travel! Appointments will be scheduled anywhere within the county or counties of your choice, based on your availability. Candidates covering multiple counties will receive more hours. Please note, you must choose one full county to be considered!

Qualifications
Graduate of an accredited nursing school with a current RN state license.
5+ years of experience in home infusion and infusion centers.
3+ Experience with IV Immunoglobulins and IV Biologics. CRNI certification(Certified Registered Nurse Infusion) is preferred.
Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred

Participation in the on-call rotation is a requirement of this position. It is anticipated that an incumbent in the position would be on-call approx. once every eight (8) weeks for two (2) weekdays per month and one (1) weekend.
Job Description:
A Registered Nurse for Home Infusion inserts, monitors and removes an IV used to give medications, blood products and nutrition to a patient. During the IV therapy, the nurse monitors the patient for adverse reactions and ensures that the tubing, bandages and needle stay in place. When a patient needs blood levels checked, an IV nurse performs a veinous phlebotomy. During the entire IV therapy process, the nurse obtains sets of vital signs, completes the physical assessment and review of systems, documents all patient-related activities and records the medications they use.
Provides and coordinates all aspects of staffing services to the patient with the assistance of the Company Directors, Nursing Coordinator and the Director of Nursing. Communicates with the patients to determine problems and/or solutions regarding their treatment. * Serve on, participate in, and attend various other committees of the facility (e.g., Policy Advisory) as required, and as appointed by the Administrator.

* Evaluate and implement recommendations from established committees as they may pertain to the medical activities of the facility and the skin care of residents

* Identify, manage, and treat specific skin disorders and primary and secondary lesions, such as skin abrasions, foot problems such as corns and callouses, decubitus ulcers, bacterial, parasitic and viral skin infections, scaling popular diseases, and benign tumors.
* Provide assessment and diagnostic services to residents. Perform an assessment evaluation using techniques including observation, inspection, and palpation.
* Complete a quarterly physical dermatologic examination on of the resident and record the results with the assessment.
* Provide therapeutic services. Confer with the Care Planning Team in the development of the care plan.
* Provide, within established protocols and under the supervision of the physician, treatment for uncomplicated dermatologic disorders as directed by the attending physician or Medical Director.
* Ensure that residents with decubitus ulcers receive appropriate prophylaxis and treatment, such as daily inspection, turning and activity, a well-planned diet, and maintaining a clean, dry bed.
* Give direct physical and psychological nursing care in emergencies, as well as in the presence of illness or disability in order to maintain life, provide comfort, reduce stress, and enhance the resident's ability to cope.
* Assist in the admission or discharge of residents.
* Work with other health agencies and family members in the physical and emotional transitions of admission, care, and discharge of residents as necessary
* Assist in resident rehabilitation activities. Where required, perform range of motion, assist in ambulation, provide emotional support. Monitor the results of physical therapy activities.
* Participate as requested by the Medical Director or Director of Nursing Services in the planning and conducting of in-service training classes concerning dermatologic nursing skills and procedures used within the facility
* Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.
* Assist in monitoring the inventory of medications, medical supplies, and equipment to ensure an adequate supply of skin care products are on hand to meet the needs of residents.
* Be familiar with and use as appropriate all items of personal protective equipment offered by the facility, including, but not limited to, masks, gowns, gloves, and back brace to be worn when lifting.
* Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
* Report any complaints or grievances made by residents regarding nursing or medical care to the Director of Nursing Services. Maintain a written record of any resident's complaints or grievances communicated to you with a notation of actions taken
* Ensure that nursing staff personnel honor the resident's refusal of treatment request. Ensure that such requests are in accordance with the facility's policies governing advance directives.

The Registered Nurse for Day Services will ensure that nursing services are provided for those individuals needing services during the hours of program attendance. The Registered Nurse will supervise the adequate completion of delegated nursing tasks to Direct Support Staff and LPNs. The Registered Nurse will provide/participate in Agency training.
ESSENTIAL FUNCTIONS / RESPONSIBILITIES:
• Participates as a member of the Interdisciplinary Team to provide nursing recommendations related to the individuals overall plan of care.
• Monitors and coordinates the delivery of preventive and routine health care for individuals with developmental disabilities.
• Adheres to and ensures facility compliance with all Physicians, NP, PA, etc. orders.
• Completes a physical health and development on individuals every 6 months.
• In coordination with the Senior Day Supervisor, ensure safeguards are updated as needed.
• Within each month, completes a monthly breast exam on female individuals if specifically ordered at day program by PMD.
• Communicates issues and carries out all directives from Nursing Administration.
• Ensures communication with residential site/family regarding diagnostic testing as necessary for appropriate nursing care or regulation.
• Accompanies individuals on appointments, if deemed necessary by RN Supervisor.
• Assists the site management in the coordination of medical program needs with community services, medical providers and other related agencies.
• Coordinates and provides health services training for staff.
• Completes nursing responsibilities as indicated on the Nursing Responsibility Checklist for Day Programs.
• Monitors for completion delegated health related tasks to Direct Support Staff and LPNs.
• Based upon the independent judgment of the RN, the RN has the authority to suspend and/or revoke the responsibility of a Direct Support Staff or LPN to perform or complete any delegated task.
• Monitors medication administration in the facility.
• Supervises medication administration responsibilities completed by Direct Support Staff and LPNs, and ensures that staff who are passing medications are current in their certification.
• Evaluates staff medication errors. Ensures that the plan of correction and retraining follows the Medication Error Point System.
• Based upon the independent judgment of the RN, the RN has the authority to suspend and/or revoke medication certifications and medication administration of the Direct Support Staff or LPN.
• Participates in various nursing workgroups as assigned.
• Maintains compliance with all applicable State/Federal regulations and agency policy and procedure.
• Communicates issues and carries out all directives from Nursing Administration.
• Participates in any plan of correction related to medical citations resulting from any internal or external survey.
• Maintains communication with LPN (as applicable), Senior Day Supervisor, residential managers, Service Coordination, Nursing Supervisor, and other staff as necessary.
• Provides nursing coverage to all People Inc programs as directed by Nursing Administration.
• Maintains CPR certification and performs CPR when necessary.
• Provides agency training related to nursing services as required.
• Complies with all agency policies and procedures.
• Other duties as assigned by Nursing Administration.
MINIMUM QUALIFICATIONS:
• Current NYS license to practice as a Registered Nurse.
• Drivers License that meets agency policy.
• CPR certification.
• Physical agility and ability to react to emergency situations, including Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines.
• Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
