Position Overview: RN Night Supervisor (Every Weekend)
The RN Night Supervisor provides clinical leadership and oversight during overnight weekend shifts. This role is responsible for supervising nursing staff, ensuring quality patient care, maintaining compliance with state and federal regulations, and serving as a trusted leader during night operations. The RN Night Supervisor acts as a key liaison between residents, families, on-call providers, and facility leadership.
Key Responsibilities of the RN Night Supervisor:
Provide clinical leadership and supervision for nursing staff during overnight weekend shifts
Oversee day-to-day resident care operations to ensure quality, safety, and compliance
Supervise, direct, and support nursing staff to maintain performance and accountability
Ensure units comply with federal and state regulations and facility policies and procedures
Support quality assurance, infection control, and utilization review activities during night operations
Receive, communicate, and implement physician orders as needed
Oversee delegation of nursing and therapeutic procedures to licensed and unlicensed staff
Conduct regular patient rounds to assess resident conditions, evaluate quality of care, and ensure accurate documentation
Serve as the on-site nursing leader and liaison during overnight weekend hours
Qualifications for the RN Night Supervisor:
Current and active Registered Nurse (RN) license in the state of employment
Minimum of two (2) years of nursing experience
At least one (1) year of supervisory or leadership experience
Experience in long-term care, skilled nursing, or geriatric care preferred
Strong leadership, organizational, and decision-making skills
Ability to read reports, analyze data, and utilize information accurately
Ability to work independently and confidently during overnight hours

Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes
Participates in on-call and weekend rotation as needed to meet patient needs

Position Overview: Licensed Practical Nurse (LPN)
We are actively seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our interdisciplinary healthcare team. In this role, the LPN provides hands-on nursing care, supports rehabilitation goals, and works closely with RNs, physicians, and therapists to ensure residents receive safe, dignified, and high-quality care.
Key Responsibilities of the LPN:
Provide direct patient-centered nursing care in a long-term care and rehabilitation setting
Administer medications and treatments as prescribed by physicians
Monitor residents' conditions, including vital signs, and promptly report changes to the supervising RN or physician
Assist residents with activities of daily living (ADLs) such as bathing, dressing, and feeding when needed
Collaborate with the interdisciplinary care team to support rehabilitation and recovery plans
Maintain accurate, timely, and compliant documentation in the electronic medical record (EMR)
Provide emotional support and education to residents and their families
Promote a safe, clean, and therapeutic environment by following infection control protocols
Qualifications for the Licensed Practical Nurse (LPN):
Current and valid Licensed Practical Nurse (LPN) license

Strong clinical skills, including medication administration and basic wound care
Excellent communication, documentation, and teamwork skills

Position Overview: LPN Weekend Supervisor
The LPN Weekend Supervisor is responsible for overseeing day-to-day nursing operations on assigned weekends. This role provides leadership, supervision, and guidance to nursing staff while partnering with the Director of Nursing to ensure high-quality resident care. The Weekend Supervisor also serves as a professional and approachable liaison between residents, families, providers, and staff.
Key Responsibilities of the LPN Weekend Supervisor:
Oversee daily resident care operations during weekend shifts
Supervise, direct, and support nursing staff to ensure safe and effective care delivery
Assist in maintaining resident care standards through oversight of quality assurance, infection control, and utilization review activities
Receive and communicate physician instructions related to resident care and ensure orders are implemented appropriately
Oversee delegation of nursing and therapeutic procedures within LPN scope of practice
Conduct regular resident rounds to assess physical condition, behaviors, and overall care quality
Ensure accurate and timely documentation of nursing observations and treatments
Ensure compliance with federal and state regulations and facility policies and procedures

Qualifications for the LPN Weekend Supervisor:
Current and active Licensed Practical Nurse (LPN) license in the state of employment
Minimum of two (2) years of nursing experience
At least one (1) year of supervisory or leadership experience preferred
Experience in long-term care or working with the geriatric population preferred
Strong organizational, planning, and personnel management skills
Ability to read reports, interpret data, and apply information accurately
Ability to plan, coordinate, and direct nursing services in collaboration with other departments
