Summary:
A Registered Nurse (RN) in a long-term care (LTC) facility provides skilled nursing care to residents, ensuring their health, comfort, and well-being. RNs oversee patient care plans, administer treatments, monitor for changes in condition, and collaborate with healthcare teams to provide high-quality, resident-centered care. They also play a key role in supervising nursing staff, educating residents and families, and ensuring compliance with healthcare regulations and facility policies.
Essential Work Functions:
Assess patient or resident health, monitor for changes, and take appropriate action to ensure their well-being
Develop, update, and implement personalized care plans in collaboration with residents, families, and the patient care team
Administer medications and treatments safely and accurately
Observe residents for any medication reactions or changes in condition and report concerns to the healthcare provider
Provide treatments including wound care, IV therapy, catheter management, and other skilled procedures within scope of practice
Supervise and support LPNs and CNAs, delegating appropriately and ensuring residents receive quality care
Educate residents and families on medications, treatments, and managing chronic conditions
Document care provided, resident progress, and any health changes in a timely manner
Respond quickly to emergencies, delivering critical care and coordinating with medical teams when needed
Work closely with physicians, therapists, and other healthcare professionals to ensure coordinated, comprehensive care
Perform other duties as assigned within the scope of practice
Adhere to facility safety protocols, infection control guidelines, and regulatory standards
Required Essential Skills:
Critical thinking, service excellence, and good interpersonal communication skills
The ability to read, write, and communicate in the English language
Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend
Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment
Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail
Sensory - Must possess visual acuity and ability to effectively communicate

The Ambulatory Surgery Center Registered Nurse (ASCRN) is responsible for the
provision of comprehensive nursing care throughout the patients' surgical encounter.
This role involves a highly collaborative relationship with the multidisciplinary team, and
fulfillment of high-quality patient care without compromise to every patient, every time.
CORE RESPONSIBILITIES: Possesses the clinical competencies necessary to perform
the essential tasks of a Registered Nurse (RN) in the outpatient surgical setting.
Preoperative and Pre-anesthesia Testing (PAT) Responsibilities:
• Conduct thorough evaluations including review of medical history, current
medications, and medical comorbidities; perform preoperative assessment of
patient that aligns with the procedure/surgery and anesthesia-type scheduled;
collects and interprets vital signs and point of care testing
• Educate patients about the procedure/surgery, anticipated anesthesia-type, and
expectations for recovery; ensure compliance with pre-operative instructions,
including knowledge of NPO and medication status pre- and post-surgery.
• Maintain accurate and complete records by ensuring all required documentation
is completed and signed by physicians.
• Conduct phone interview and review of records for each patient scheduled;
identify patients requiring anesthesia consultation in accordance with current
guidelines.
• Responsible for preoperative procedures including IV insertion and assistance
with regional nerve blocks; adheres to aseptic technique and infection control
guidelines per policy.
Intraoperative (circulator/scrub nurse) Responsibilities:
• Assist in maintenance of the sterile field, including monitoring for contamination;
monitors traffic/noise patterns in the OR, ensuring adherence to facility policy and
procedure.
• Management of surgical instruments; ensuring the availability of surgical
supplies, equipment, and medications for the indicated procedure/surgery.
• Adheres to all required components of Safe Surgical Checklist.
• Accurately documents all necessary components of the operative encounter,
including implantable devices or tissues.
Postoperative & Recovery Responsibilities:
• Evaluate patient recovery from anesthesia; monitor for adverse reactions;
management of acute postoperative pain.
• Provide instructions for discharge, including home care, medication changes, and
follow-up appointments; ensuring a safe discharge plan is in place.
• Follow up with patients to assess recovery and address any post-operative
concerns or questions; communicate any issues identified to the indicated
provider.
Additional Clinical Responsibilities:
• : Administer and chart medications per physician order; perform medication
calculations for accurate medication dosing.
• Responds to and provides treatment for urgent or acute patient situations during
unanticipated events per policy and in alignment with BLS, ACLS, MHAUS and
PALS guidelines and algorithms.
• Maintains patient safety throughout perioperative encounter; works in
accordance with current employee safety guidelines
• Works respectfully and collaboratively with all team members; fosters an
environment that promotes staff growth and development
• Monitors medical supplies and medication inventory levels; report to appropriate

cleaning; ongoing monitoring of expiration dates

• Completes mandatory educational requirements and maintains required BLS,
ACLS, and PALS certifications; annual review of applicable policies
• Maintains strict confidentiality in alignment with HIPAA (Health Insurance
Portability and Accountability) guidelines and policies.
• Consistently adheres to applicable industry standard guidelines (i.e. AORN,
CDC, APIC)
• Assumes responsibility for assigned role and seek/performs other job-related

Hospice of Southern Maine is seeking a per diem On-Call/Triage Nurse to work in our Home Program. The On-Call/Triage Nurse is the first line contact for all agency communications and patient care needs outside of normal operating hours to ensure access to and delivery of 24/7 nursing services per agency policy. The On-Call/Triage Nurse handles inbound after-hours phone calls from Hospice patients and families, assesses the needs of the patient and family, provides education about interventions, and coordinates or performs required care and follow-up in person, electronically, and by telephone in homes, facilities, and hospitals. The On-Call/Triage Nurse conducts symptom management and emergent nursing care; continuous home hospice care visits; referral management, admissions, and transfers to the Gosnell Memorial Hospice House; and Time of Death, post-mortem, and RN bereavement visits. The Triage Nurse also supports RN Case Managers by following up on physician orders, prescription refills, and test results; providing targeted teaching to home patient caregivers; coordinating care with the Hospice Interdisciplinary Team; and making proactive outreach calls to patients with rapidly changing conditions. All services provided to the patient and families are pursuant to a physician's orders and consistent with the patient's plan of care.
The on-call/triage hours are 4pm-8:30am and the incumbents are paid their hourly rate of pay for the entire on-call shift, regardless of the number of calls received or if they go out and make visits. On-call shift days vary based on the needs. Must have flexibility to cover various days of the week.
The ideal candidate would also have availability to cover day shift per diem visit RN hours if there are no on-call/triage shifts needed.
Graduate of an NLN accredited nursing program; BSN or CHPN preferred. Minimum of two to three years of RN experience required. Home care, hospice and/or oncology nursing experience preferred. Pain management, symptom controls, crisis intervention and family support experience required. Registered Nurse licensure by the State of Maine; valid driver's license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order. BLS certification or the ability to obtain certification within the first 90 days of employment required. Certification must be kept active during the course of employment. During initial and ongoing consultations, serve as a patient liaison between the patient, their family, and the pharmacy to ensure a seamless therapy process.
Perform comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care.
Use health assessment data, and input from the agency team, the physician, the patient, and the family to determine patient needs.
Manage patient and family expectations regarding services, outcomes, and discharge goals.
Establish primary and secondary diagnoses based on patient assessment and home health care focus.
Develop care plans with skilled interventions, medical supplies, and necessary services to meet goals.
Regularly evaluate patient progress, in collaboration with team members, and revise care plans as needed.

Qualifications
Graduate of an accredited nursing school with a current RN state license.
5+ years of experience in home infusion and infusion centers.
3+ Experience with IV Immunoglobulins and IV Biologics. CRNI certification(Certified Registered Nurse Infusion) is preferred.
Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred
Effective interpersonal, time management, and organizational skills.
Strong written and verbal communication skills.
Organizational ability to maintain consistently accurate records.
Ability to direct information to the appropriate healthcare provider.
Proficient in computer skills, including word processing, and efficient use of the internet and email.

The Infusion Nurse will contribute to the company's growth by providing direct patient care at one or several of our centers in Portland. The Infusion Nurse will make sure our patients receive the highest level of care possible in infusion therapy.
The ideal candidate has experience working autonomously, and experience starting IVs and working with biologic drugs. The Infusion Nurse role is perfect for those wishing to remain at the forefront of healthcare innovation, while having predictable hours, autonomy, and a manageable workload (always a 3:1 patient to clinician ratio).
In this role, you will:
Assess and document the health status of the patient
Deliver infusions or injections in accordance with prescribed protocols and organization guidelines
Assist in maintaining medical inventory and supplies
Openly communicate with referring/prescribing providers, such as order clarification, patient status, and plan of care
Provide onsite supervision for all patients
Here's what we're looking for:
Current RN licensure in Maine
ACLS & PALS Certification (preferred)
Current BLS CPR certification
Excellent IV skills (infusion therapy experience a plus)
Experience and strong familiarity utilizing Electronic Health Records (EHR) systems
Ability to work 1 to 2 Saturday's a month
