Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.

Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.

WHAT IS THIS POSITION RESPONSIBLE FOR?
Clinical supervision of Crisis Services Program, and direct clinical supervision of Outreach and Peer Support Specialists.
Assist with process of screening, interviewing, and recommending applicants for hire.
Provide clinical program development to include use of evidence-based practices, as well as staff clinical training.
Respond promptly when children, families and adults are experiencing a crisis that puts them at great risk for hospitalization or other higher, or less desirable, out of home levels of care.
Assess crisis through risk assessment, management, crisis prevention, intervention and engage as appropriate to help de-escalate crises and promote stability and recovery.
Coordinate referral and transition for engaged clients once stabilization is established.
Promote a culture of inclusion and participation.
Collaborate with psychiatrist, nursing team, and other medical providers, for provision of medical care during crisis recovery.
Engage and collaborate with community partners, to include emergency responders, hospitals, and community social service agencies to coordinate transitions of care for individuals in crisis.
Job Qualifications
MINIMUM QUALIFICATIONS:
Master's Degree in Social Work or related field of study, LCSW preferred.
Experience partnering with community agencies for care coordination.
At least 2 years of direct care experience working with individuals with psychiatric and addiction issues; knowledge of working with homeless and underserved population.
Experience completing risk assessments and creating crisis safety plans.
Supervisory, training, and grant management experience preferred.
Obtain and maintain a valid driver's license.
Obtain and maintain Welle certification.

ESSESNTIAL DUTIES AND RESPONSIBILITIES:

Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 "on-call" process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.

EXPERIENCE, EDUCATION AND CERTIFICATIONS:

Required Qualifications:
Active RN license in good standing.
Bachelor of Science in Nursing (BSN) required.
Minimum of 3 years' experience working with elderly or frail populations in acute, long-term, or community-based care.
Minimum of 3 years of progressive leadership experience in a healthcare setting.
Strong background in patient-centered care, chronic disease management, and elder care best practices.
Demonstrated expertise in staff leadership, interdisciplinary collaboration, and clinical education.
Strong communication, conflict resolution, and team-building skills.
Knowledge of clinical budgeting, KPIs, and financial resource management.
Proficiency in electronic health records and Microsoft Office applications.
Ability to analyze and apply clinical quality improvement data.

JOB SUMMARY
Responsible for overseeing and ensuring 24/7 high-quality clinical care for participants across all settings. This role focuses on delivering participant-centered care in alignment with PACE policies, regulatory standards, and best practices for elder care. As a key member of the PACE program leadership team, this position contributes to policy development, census growth, staff training, and achieving outstanding participant outcomes and satisfaction.
ESSESNTIAL DUTIES AND REPONSIBILITIES:
Integrate organizational values, mission, and vision into program culture, policies, and daily operations.
Oversee clinical care and ensure 24/7 high-quality, participant-centered services.
Monitor participant experience and outcome data, guiding IDT Teams to ensure high-quality care.
Collaborate with HR and management for staff recruitment, training, and performance oversight.
Ensure timely and accurate participant assessments and care plans by interdisciplinary teams.
Establish and oversee an effective 24/7 on-call process with Medical Director.
Collaborate with leadership to maintain efficient, high-quality services.
Ensure clinical staff actively engage in team meetings and care planning.
Develop and monitor budgets, key performance indicators (KPIs), and financial reports for resource utilization for nursing services.
Participate in Quality Improvement programs, track data, and lead improvement initiatives.
Ensure clinical compliance with federal, state, and PACE requirements.
Develop s and assign duties aligned with competencies.
Create policies in line with PACE standards and collaborate with leadership, reviewing these policies and procedures at least annually.
Engage with PACE associations and committees to stay updated on regulations and best practices.
Collaborate with the Medical Director, Operations Director, and program management teams to ensure high-quality, efficient, and effective service delivery.
Participate in family and staff meetings, in-service training, orientation and departmental activities.
Follow all PACE policies, OSHA safety guidelines, and maintain confidentiality.
Lead infection control efforts to ensure best practices in all settings.
Attend continuing education and training as required.
Maintain required certifications and professional affiliations.
A registered professional nurse shall be designated in writing to act in the absence of the director of nursing services.
For NJ: When Director of Clinical Services is operating as an RN, they shall provide care to participants in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record.
Perform other duties as needed.
EXPERIENCE, EDUCATION AND CERTIFICATIONS:
State RN license with leadership skills and experience in healthcare delivery and management.
Bachelor of Science in Nursing Degree required. Master's degree in management or a healthcare related field preferred. Professional Certification preferred.
Minimum 3 years' experience working with the frail and elderly in acute, long-term care or community-based setting.
3 years of progressive management experience required. (For NJ: Nursing supervision and/or nursing administration in a licensed healthcare facility)
Experience in home care, managed care, or long-term care preferred.
PRE-EMPLOYMENT REQUIREMENTS:
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
