Essential Duties:
Client Services
Engages and builds rapport with primarily medium to high acuity clients, completes assessments, co-creates and implements service plans, facilitates the acquisition of all appropriate services, and monitors individual service plan progression.
Develops a working knowledge of a variety of public and private community resources, serving as a liaison and advocate on behalf of the client. Assists clients with acquiring public assistance and other entitlement programs.
Educates and navigates clients living with HIV through complex health care systems and insurance programs, coordinating with multi-disciplinary teams to reduce barriers to care and prevent gaps in health coverage.
Collaborates with discharge planners to provide transitional care services, when needed.
Department Responsibilities
Develops working knowledge of departmental standards and benchmarks. Strives to reach all quality assurance and improvement markers.
Participates in partnership building with both internal and external resources. Strives to reduce HIV related stigma and isolation through education and understanding.
Works in a rotating schedule to assist walk-in clients, either in office or at a partner location.

Laboratory Operations

* Bachelor's degree in Medical Laboratory Science, science, or other relevant fields.
* Medical Laboratory Science certification with the American Society for Clinical Pathology (ASCP) or equivalent certifying organization; or certification with specialty organization appropriate to the laboratory divisions.
* Six (6) years of progressively responsible clinical technologist experience to include three (3) years in a leadership or supervisory capacity (or equivalent).
* Experience with wet bench procedures, instrumentation, quality management/assurance, and CAP proficiency and accreditation guidelines.
* Track record of effective management of teams and staff to meet group goals, meet deadlines, and set priorities.
* Track record of effective Change Management.
* Extensive knowledge and expertise in clinical laboratory test/system validations and data management.

Assist with admission assessments, orders, and paperwork per DNS direction.
Coordinate admissions, discharges, transfers, and bed management with nursing leadership and admissions teams.
Communicate with primary care providers, hospital partners, and external specialists throughout the resident stay.
Initiate and present NOMNOC (denial) letters as appropriate.
Collaboration & Communication
Work closely with internal partners including Nursing Leadership, Social Work, Rehab Services, MDS, HIM, A/R, Therapists, and Operations.
Collaborate with external stakeholders such as family members, hospitals, discharge planners, HMOs, physicians, home health agencies, and other providers.
Serve as a resident and patient advocate, ensuring informed decision-making and family education.
Utilization, Compliance & Quality
Monitor RUG utilization, PDPM outcomes, and appropriate service use.
Conduct reviews related to falls, wounds, medication errors, readmissions, pharmacy discrepancies, and other quality indicators.
Ensure appropriate testing, timely results, and adherence to regulatory and payer requirements.
Participate in weekly Case Management meetings and continuous quality improvement initiatives.
Leadership & Supervision
Provide day-to-day leadership, guidance, and supervision to Licensed Nurses and NACs.
Participate in hiring, performance management, coaching, and employee development in alignment with organizational values.
Troubleshoot care concerns, complaints, and conflicts, escalating to DNS or Operations leadership as needed.
Additional Duties
Perform medication destruction and disposal oversight.
Assist nursing staff with care activities as needed.
Perform all regular RN or LPN duties per licensure and job description.

Olympic Sports & Spine (OSS) is seeking a full-time Clinic Manager Physical Therapist with at least 2 years of outpatient, orthopedic experience, management and leadership skills, and a keen interest in leading a successful outpatient orthopedic clinic. Candidates with prior clinic management experience are preferred. Clinic Managers are responsible for the day-to-day operations of the clinic, including, but not limited to, patient care, personnel management, physician relations, and financial management.
