    Job Title: Case ManagerJob Description As a RN Care Manager, you will assist patients and their families in developing and executing safe discharge plans from the hospital, utilizing the resources available to each individual patient. You will work closely with durable medical equipment providers, home health agencies, hospices, shelters, and various post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living. Collaboration with community partners and securing authorization from insurance companies will be key components of your role. Responsibilities Coordinate and implement safe discharge plans for patients leaving the hospital. Collaborate with community partners and secure authorizations from insurance companies. Work with Case Management Assistants on tasks such as SNF authorizations, transport arrangements, and DME dispensing. Focus on discharges and new assessments during weekend coverage. Cover any acute departments in the hospital, including ICU, medical, med-surg, telemetry, observation unit, and ED. Essential Skills Active Oregon RN license. BLS certification through the American Heart Association. Minimum of 1 year of case management experience. Recent acute care experience in a hospital setting. Proficiency in Epic charting system. Strong knowledge of discharge planning, care coordination, and community-based services. Excellent communication and interpersonal skills. Additional Skills & Qualifications Case Management Certification preferred, but not required. Knowledge of Utilization Review is helpful. Must demonstrate regular and consistent attendance. Work Environment This position is based at a short-term acute care facility with 108 staff beds. The work schedule is Monday through Friday, 8:00 AM to 4:30 PM, with weekend coverage as needed. The expected patient ratio is 1:15. The facility uses the Epic charting system, and there is no parking fee. Scrubs are provided for OR, Critical Care, ED, PACU, and Maternal Child departments, while business casual attire is required for Case Managers. Job Type & Location This is a Contract position based out of Medford, OR. Pay and Benefits The pay range for this position is $40.00 - $60.00/hr.
