Summary:
The Rehabilitation Nurse (Rehab RN) provides comprehensive care to patients recovering from injuries, surgeries, or illnesses that require rehabilitative treatment. This role involves providing comprehensive nursing assessments, implementing care plans, monitoring patient progress, and collaborating with a multidisciplinary team to help patients regain optimal physical, functional, and emotional well-being. The Rehab RN demonstrates advanced clinical skills, excellent communication, and a commitment to patient-centered care and safety.
Essential Work Functions:
Complete nursing assessment of patients on the unit and documents appropriately into patient records
Prepare patients for surgery, including educating them about procedures and post-operative care
Coordinate and provide holistic patient care, including mobility support, rehabilitation exercises, and activities of daily living (ADLs), while ensuring adherence to individualized care plans
Monitor and evaluate patients' responses to therapeutic interventions, documenting progress and communicating any deviations or concerns to the interdisciplinary team
Safely administer prescribed medications and treatments ensuring proper documentation and compliance with safety protocols
Maintain a clean, organized environment and ensure rooms are adequately stocked with necessary supplies and equipment to support rehabilitation care
Assist with safe patient transport to and from therapy sessions, diagnostic procedures, or other care areas as needed
Perform other duties as assigned within the scope of practice
Adhere to hospital safety protocols, infection control guidelines, and regulatory standards
Required Essential Skills:
Critical thinking, service excellence, and good interpersonal communication skills
The ability to read, write, and communicate in the English language
Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend
Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment
Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail

Essential Duties and Responsibilities:
Work directly as a member of the Arkos Health Interdisciplinary Team
Implement and participate in the patient's plan of care; update and revise as needed
Assist in coordination of comprehensive care; including obtaining referrals to other interdisciplinary services (In Home Wellness, Care Coordination, Population Health, Care Management to help prevent further need for acute care utilization
Maintain case load based on the needs of the client and/or contractual obligation
Conduct face to face visits at a minimum of every 2 weeks for assigned patients enrolled in Intensive Case Management. May conduct increased face to face or telephonic visits based on the needs of the patient and or contractual obligation
Complete Clinical and Social Assessments (CSA) to develop care plan upon initial start of care visit with patient
Provide communication with patient's providers and medical specialists regarding patient status and any changes in condition
Engage with Arkos Health leadership and contracted providers to resolve changes in patient status, complaints, or complications though participation in weekly ICT meetings
Complete medical reconciliation for all transitional care patients within 30 days of initiation of TCM contact
For this field-based position, individuals must be able to travel up to 80% of the time in their own personal vehicle
Qualifications and Skills:
Current, unencumbered, Registered Nurse License and graduation from an accredited Registered Nursing program required
Current BLS/CPR certification required
Current TB test and maintain a negative test annually required
Hep B vax/titer proof (Company does offer/or may sign a declination) required
Current flu shot (or sign waiver/follow mask policy) required
Must have valid state driver license and appropriate liability insurance
Must have their own reliable vehicle to drive out in the field to patients homes

DEFINITION:
Performs work as a registered nurse, as a member of primary care medical teams to provide care management throughout a multiple county region for the Partnership Health Center (PHC).
REPRESENTATIVE EXAMPLES OF WORK:
Maintains an active patient caseload. Develops strategies with the care team consistent with a patient-centered medical home to manage complex medical needs for clients with complex medical conditions.
Works within the PHC model for chronic disease self-management to provide patient and family education and develop goals that result in improved health outcomes. Develops comprehensive care plans and provides documentation as appropriate or required.
Makes visits to clients in their homes or at other locations outside the PHC as necessary.
Establishes a quality measurement system for all clients in the individual caseload to objectively measure client outcomes, compare and analyze disease burdens, and predict medical expense.
Coordinates hospital discharges with client, family, and hospital care team.
Participates in multi-disciplinary conferences, serving as a clinical resource for medical providers, dental, mental health and psycho-social case managers.
Provides clinical work directions for Licensed Practical Nurses and Medical Assistants, serves as a clinical resource, and contributes to performance management efforts.

WORKING RELATIONSHIPS: Works closely with Partnership Health Center medical care teams, hospital case managers or social workers, other case managers (home health, public health, mental health etc.); and medical providers in the community who are working with clients on his or her caseload.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
KNOWLEDGE: Working knowledge of quality management processes. Working knowledge of client confidentiality, and the ethical and legal requirements of the profession. Working knowledge of nursing practices related to acute and chronic complex medical conditions such as cancer, cardiac disease, pulmonary disease, diabetes, and chronic pain conditions. Working knowledge of chronic disease self-management strategies. Working knowledge of the principles and practices of HIPAA compliance. Working knowledge of the PHC Corporate Compliance Guidelines.
SKILLS: Skill in the use of a personal computer including electronic medical records, word processing, spreadsheets, email and data base software.
ABILITIES: Ability to coordinate complex medical care plans consistent with the chronic disease self-management model. Ability to gather and analyze data and make appropriate recommendations to the medical care team. Ability to communicate in the English language, orally and in writing. Ability to establish and maintain effective working relationships with diverse individuals and groups.
MINIMUM QUALIFICATIONS:
EDUCATION: Requires graduation from an ACEN (Accreditation Commission for Education in Nursing) approved program.
EXPERIENCE: Requires one year of experience in providing chronic disease management and interventions with clients.
SPECIAL REQUIREMENTS: Requires current license to practice as a registered professional nurse in Montana. Subject to passing a criminal background investigation that will require fingerprinting. Requires a current Healthcare Professional BLS certification or the ability to obtain it within 90 days of hire. Requires immunizations or proof of immunity to certain infectious diseases and a TB test. New employees will be asked to volunteer vaccination status for required vaccines upon hire and will be offered assistance during hiring to receive necessary immunizations. Schedule is part-time job-share with flexible hours Monday - Friday, no weekends or evenings
In this position you will:
Welcome patients and prepare them for their infusion appointment
Complete intake assessments including medical history and vital signs
Monitor, document, and administer IV medications per protocols
IV skills, phlebotomy and point of care laboratory testing
Manage and prioritize patient portal (MyChart) and telephone communications
Participate in and support patient education
Coordinate ongoing care including referrals and prescriptions
Triaging and manage complex patients
Play an essential role in fulfilling our mission
Heighten the patient experience by providing extraordinary care

Job Summary
Partners in Home Care is seeking a compassionate and skilled Registered Nurse (RN) - Case Manager to join our team. This position is responsible for delivering high-quality nursing care and managing the overall coordination of home-based services for assigned patients. Using an interdisciplinary team approach, the RN will assess, plan, implement, and evaluate individualized care, ensuring continuity, quality outcomes, and optimal patient and family support.
This position may begin as a Visiting RN, transitioning into a Case Manager role once a half caseload is assumed, in accordance with the Collective Bargaining Agreement with Montana Nurses Association Local #32. Both roles share the same clinical responsibilities.
Essential Duties and Responsibilities
Provide skilled nursing care and coordinate services for patients in their homes based on comprehensive assessments and physician-authorized care plans.
Continuously observe, assess, and evaluate patient conditions-physical and psychosocial-and adjust care interventions accordingly.
Supervise and evaluate care provided by Home Health Aides and CNAs, ensuring adherence to the patient's care plan.
Maintain open and timely communication with physicians, interdisciplinary team members, and external community resources to ensure continuity of care.
Educate and support patients and families through counseling, symptom management, health teaching, and referrals, promoting quality of life and optimal outcomes.
Accurately document all patient care in accordance with agency standards and regulatory requirements (OASIS/HIS/HOPE), with working knowledge of current reimbursement rules and compliance guidelines.
Participate in interdisciplinary scheduling and rotate into the evening/weekend on-call schedule, assessing and addressing patient needs via phone triage or in-person visits as needed.
Engage in agency quality improvement activities and assume responsibility for achieving relevant quality metrics. Attend required in-service training sessions.
Meet caseload and productivity expectations aligned with agency standards.
Perform other duties as assigned by supervisor.
Minimum Qualifications
Current Registered Nurse (RN) license in the State of Montana (BSN preferred).
Valid driver's license, auto insurance, and access to reliable transportation.
Preferred: Two years of recent clinical experience in acute care, home health, or hospice, including skills such as venipuncture, catheter care, wound management, and parenteral therapy.
Schedule
Typical workdays are Monday through Friday, with evening/weekend on-call rotation. Schedule may vary depending on FTE status and patient care needs, as directed by the supervisor.
