What You Will Do
Reviews and utilizes completed medical and mental health assessments from the ResCARE clinical team when initiating the wellness plan for clients on the temporarily unable to work track. . Confirms acuity level of identified client and tailors services plan accordingly.
Develops and monitors wellness plan on behalf of clients with untreated or unstable medical and/or mental health conditions adversely affecting the level of employability. Coordinates and integrates a written, coordinated wellness plan in cooperation with the client the client's family, and/or other providers serving the client.
Performs and maintains effective care management for a caseload of clients, as assigned, from wellness initiation to wellness completion. Meets with assigned clients to monitor progress and compliance with the wellness plan. Tracks/ monitors client progress and produces/maintains detailed, accurate and timely case notes. Reviews cases for completeness of documentation.
Develops inventory of resources that will meet the clients' needs as identified in the assessment process. Becomes familiar with service providers in the community where the clients resides in order to mitigate barriers to wellness plan compliance such as transportation, childcare etc.
Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers. Participates in meetings with service providers to coordinate service and follow up to ensure client's compliance with and timely completion of the wellness plans and required documentation.
Works collaboratively with team members to provide outreach (Via Phone calls, Emails, Texts and Field visits) to clients who have failed to comply with the process of the initiated wellness plan and wellness care management services.
Provides information and assistance through advocacy and education to client/family on availability and eligibility of entitlements and community services. Assists with arranging escorts and transportation for clients to appropriate facilities/agencies, as necessary.
Participates in initial and ongoing trainings as necessary to maintain basic level of knowledge related to serious physical ailments as defined by HRA. Collaborates with the wellness health team to develop psycho-educational plans for client's wellness plan process and medication compliance.
Maintains updated clients' case records through the WeCARE wellness care management and HRA platforms, and coordinates effective electronic communication throughout all provider databases, as needed. Maintains case records in accordance with the wellness care management policies/procedures, agency standards and regulatory requirements.
Participates and consults with team supervisor in case conferences, staff meetings, and discharge planning meetings to determine if client requires an alternate level of care or is appropriate for discharge.
Participates in special projects and performs other duties as assigned.

Serve as the clinical director for Complex Care, providing clinical supervision to clinicians (1 NP, 1 RN) who supporting care delivery and coordination for the more medically complex patients in the jails.
This role encompasses the following responsibilities:
Establishing clinical criteria for inclusion.
Defining the overarching vision and program objectives.
Attending weekly Complex Care rounds to ensure the effective delivery of care.
Consulting on Complex Care patients and offering clinical guidance and support to front-line providers within jail facilities.
Providing direct patient care at least 50 percent of schedule
Serving as a clinical point of contact for colleagues from other departments as needed.
Delivering patient education related to complex diagnoses.
Ensuring that patients are assigned to medically appropriate housing areas on Rikers Island.

RN Administrator for Admissions, Intake & Outreach is responsible for overseeing the functions and activities associated with Community Outreach Services, Admitting, and Intake. Reports directly to the Vice President for Admitting, Intake and Outreach Services and maintains an open dialogue with the senior leadership of the hospital and referring partners.
Responsibilities:
Continuously monitors and assesses the delivery of services provided by the Community Outreach, Admitting and Intake implementing changes when appropriate.
Acts as liaison between Hospital and referring institutions and providers.
Actively engages in the ongoing marketing of programs and services provided by the Hospital.
Works cooperatively with other departments, provider referrers and managed care organizations to facilitate the admission of patients to the various programs provided by the Hospital.
Interfaces with managed care organizations to facilitate reimbursement for services provided by the Hospital. Oversees the appeals process for those cases for which reimbursement has been denied.
Conducts interdepartmental meetings related to the quality and utilization of clinical services.
Serves on various Hospital committees as assigned.
Attends meetings and seminars as appropriate.
Ensures all related manuals are kept current.
Works within legal, ethical practice standards to his/her position as well as the Hospital's Code of Conduct. Complies with appropriate policies and procedures and brings potential compliance matters to the supervisor or compliance officer.
Oversees and participates in the budget process for his/her assigned departments.
Completes Introductory and Annual Performance Appraisals for all direct reports. Reviews Introductory and Annual Performance Appraisals for other assigned staff.
Oversees the interface with the Finance Office regarding pertinent insurance information, e.g. Medicare, Medicaid and Commercial Insurance.
Elicits recommendations regarding continuous improvement of patient safety from staff.
Encourages reporting of occurrences without fear of reprisal.
Participates in the development of recommendations for improvement in patient safety based on staff recommendations.
Participates in the implementation and monitoring of patient safety programs.
Maintains current knowledge of regulatory requirements which may impact on the delivery of patient services.
Integrates the services of the departments supervised into the day-to-day activities of the Hospital in support of its philosophy and mission.
Develops appropriate goals and objectives for departments supervised.
QUALIFICATIONS:
Master of Nursing Degree required.
NYS unencumbered Registered Nurse License required.
Ten (10) to fifteen (15) years of management experience, five (5) of which must be at the department head level. Three (3) years' experience as an Administrator or Vice President preferred.
Excellent interpersonal skills.
Ability to interface with all levels of personnel.
Excellent written and verbal communication skills.
Ability to analyze detailed statistical and other pertinent data.
Knowledge of and experience in Quality Assessment and Improvement, Utilization Review.

Job responsibilities for the Registered Nurse:
· Determines types of reconsiderations, redeterminations, action appeals, grievances, and grievance appeal
· Investigation of Provider, Subscriber, Membership and Pharmacy Appeals to include: Reimbursement, Coding, Benefits, Pre-existing Conditions, Prior Approval, Medical Necessity, Precertification, Continued Stay, Reduction, Termination, and Suspension of services.
· Investigate and respond to written and/or verbal appeals and grievances submitted by members and providers in accordance with Elderplan Contract Benefits, CMS, and DOH regulations.
· Documents all relevant information in the Appeals and Grievances determination process in the appropriate designated areas.
· Tracks departments' compliance with requested action as a component of the Appeals and Grievance and determination process.
· Presents cases to Appeals & Grievances File Review.
· Attend and represent A&G at NYS Fair Hearings/OTDA/ALJ Hearing
Qualifications needed for Registered Nurse:
· Graduate of accredited nursing program.
· BSN or equivalent experience required.
· 2 years of managed care or hospital-based utilization/ case management experience
· 2 years minimum of clinical experience
· Licensed Registered Nurse in New York State
· Must be proficient in MS Word, Excel and desktop software knowledge preferred.
