In this role, you will be responsible for leading and managing our to provide clinical oversight and Ancora Memory Care Assited Living residents and providing clinical oversight to ensure quality of care for all residents. The Ancora MAnager assists in maintaining compliance of federal and state regulations while ensuring the organizations policies and procedures are implemented and maintained. This position also participates in the community's QAPI Committee.
Essential Functions, Key Duties, & Responsibilities
Supervise the delivery of nursing services to residents including ADLs and medication services.
Assist in interviewing, hiring and orientation of new clinical associates.
Monitor and assist in scheduling staffing to ensure the community is appropriately supported.
Assist with resident assessments for pre-admission, admission, and ongoing assessments and implementation/updating of Resident Service Plans.
Participate in care conferences and maintain regular communication with families and residents.
Routinely make rounds of the Ancora Unit for compliance and ensure proper resident care.
Ensure an attractive, safe, and sanitary environment is provided for residents, associates, families, and outside vendors.
Serve as a member of the QAPI Committee as assigned.
Audit and complete electronic health records.
Member of the Administrative Staff with on-call responsibilities.
Perform other duties as assigned.
Comply with all policies and procedures and any updates.
Position Requirements
Education: Registered Nurse/Licensed Practical Nurse
Licenses/Certifications: Active Nurse license & current CPR certification
Experience: 2+ years of clinical experience/leadership in Assisted Living/ Memory Support.
Skills & Abilities:
Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines pertaining to long-term care.
High degree of integrity and compassion to our residents.
Strong communicator and capable of leading change.
Ability to multi-task and prioritize projects and deadlines concurrently.
Proficiency in electronic health records, Point Click Care and Microsoft experience preferred.

The Medical Support Specialist (Plasma Center Nurse) works under the guidance of the Center Manager (or Assistant Manager as applicable) for operational guidance and under the management of the Center Physician for medical issues. The Medical Support Specialist will be familiar with regulations of the plasma collection industry or a manufacturing environment. The Medical Support Specialist follows guidance provided by BioLife Medical Affairs and provides center level support of environmental, health and safety (EHS).
How you will contribute
You will determine donor eligibility; to include, proper management of informed consent, AIDS education and confidential self-exclusion, medical history interview and acceptance requirements, physical examination, blood testing, and reviewing Serological Protein Electrophoresis (SPE).
You will evaluate donor reaction(s), which occurs at the facility as outlined in the SOPs. Follow applicable SOPs for medical emergencies including the development of Center Physician's standing orders and donor transport to emergency care facilities.
You will refer to the Center Physician or Medical Affairs (as applicable) when in need of providing unacceptable findings to donors, or guidance concerning medical or technical issues, including donor safety and eligibility.
You will support the Hepatitis B and Seasonal Flu vaccination programs for employees as applicable.
You will manage employee incidents and determine whether further evaluation is required by occupational health/ER. Refer to EHS guidance regarding employee incidents.
You can be a Pandemic Coordinator when authorized by EHS and support investigations associated with pandemic threats within the local community as indicated by EHS or Medical Affairs.
What you bring to Takeda:
High school diploma or equivalent to including graduate equivalent of a recognized educational nursing program with state requirements
Currently licensed or certified in the state where responsibilities will be assigned: Registered Nurse (RN), Nurse Practitioner (NP) or Physician Assistant (PA)
Current Cardiopulmonary Resuscitation (CPR) and AED certification
Fulfill state requirements (in state of licensure) for basic IV therapy
Satisfactorily complete the FDA approved training requirements for BioLife Medical Support Specialist
Two years in a clinical or hospital setting

KNOWLEDGE & SKILLS:

Experience as a RN, Medical Case Manager is ideal, or a clinical background in orthopedics, neurology, or rehabilitation is preferred.
Ability to meet with the patient, their physicians, other healthcare providers, attorneys, and advisors/clients and coworkers.

Manage an assigned caseload of client with HIV/AIDS requiring comprehensive Ryan White Medical Case Management Services.
Conducts initial intake and ongoing assessments, medication and treatment adherence assessments, treatment/service plans and medical assessments
Monitors progress toward improving/stabilizing health status
Development of comprehensive care plans with clients that integrates medication and treatment adherence into services
Work in conjunction with clinical providers (e.g. case conferencing, huddles); ensuring all clients are case conference at least twice per year.
Provides and tracks referrals and linkages to services with special emphasis on core clinical services, access to medical, income benefit programs and access to emergency financial assistance.
Monitors treatment and medication adherence for all core clinical services
Maintains client file, records all activity, and maintains appropriate documentation of eligibility
Inputs all data into the client level database.
Completes the Case Management Competencies training with a passing grade as determined by the AIDS Foundation of Chicago training policy
Attends at least 12 trainings during the contract year to enhance skills and HIV knowledge; including at least four clinical training
Submits monthly/quarterly/annually reports on program/client activities in a timely fashion.
Attends monthly team meetings and all other relevant program meetings as scheduled.
Conducts presentations and/or educational workshops on behalf of CCHC.
Conducts peer chart reviews to ensure continued quality of services.
Completes at least one telephone contact with each client monthly.
Meets with clients at other agency clinic locations as necessary.
Follows standard operating procedures and AIDS Foundation of Chicago
Performs other duties as required or assigned

SUMMARY
The Medical Case Management (MCM) Care Coordinator ensures timely access and assignment of youth to ongoing Medical Case Management services. This role reviews interim case activities, monitors performance benchmarks, prepares case materials, manages data entry, and supports the Interim Care Coordination Unit as needed. The Care Coordinator plays a key role in ensuring accurate documentation, compliance with timelines, and effective coordination of health information across systems.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Ensure timely access and assignment of youth for ongoing Medical Case Management (MCM).
Assist in final review of completed interim critical case activities.
Review and monitor daily, weekly, and monthly reports to identify cases approaching funder benchmarks.
Assign and reassign youth to MCM services within required timeframes, including:
New custody cases within 5 business days after the 45-day interim period (95% benchmark).
Reassignments within 14 days of receipt from the Ongoing Medical Case Management Agency.
Conduct outreach to PCP/Dental providers for transfers involving youth ages 0-5 and pregnant youth.
Prepare and forward complete MCM assignment packets-including health information, placement details, and contact information-according to contractual timelines.
Generate and distribute written correspondence informing caregivers, youth, caseworkers, and providers of assigned MCM services.
Work with the Medical Records Clerk to ensure closed cases are properly prepared for storage and long-term retention.
Produce standard and custom reports using information system tools as directed by the HealthWorks supervisor.
Gather and maintain data for reporting and analytics needs within HealthWorks.
Complete all assigned data entry in the HealthWorks Database/SACWIS and Athena systems.
Review and complete tasks associated with SACWIS reports and other DCFS reporting requirements.
Use system reports to support data integrity and sanitization of the information system.
Submit daily production reports to the immediate supervisor detailing assignment status, pending items, and issues.
Perform all other duties as assigned.
QUALIFICATIONS Education and/or Experience
Bachelor's or Associate's degree in Social Work, Computer Science, Human Services, or related field; OR equivalent education and experience.
Three years of professional experience in social services preferred .
