Process OASIS assessments, verify SOC dates, and correct documentation discrepancies. Follow up on missing documentation and ensure completion by Licensed Professionals. Manage referral intake and ensure timely scheduling of assessment visits. Lead weekly Case Conferences and guide recertification/discharge decisions. Review and approve Case Conference coordination notes within 48 hours. Communicate with physicians to obtain, confirm, or update clinical orders. Order Management Review, approve, or decline orders per protocol. Ensure modifications, corrections, and medication updates are completed accurately. Track unsigned orders and follow up as needed. Ensure frequency-related orders reflect correct scheduling. Quality & Patient Safety Monitoring Review vital-sign alerts and wound-score deviations; notify physicians as appropriate.
