**PURPOSE AND SCOPE:**

Job Summary
The Bilingual HIV Medical Case Manager (Spanish/English) provides comprehensive case management services to a caseload of approximately 35-40 HIV-positive clients, including pregnant women, in both clinical and office settings. This role is essential in supporting clients' medical, psychosocial needs while promoting health equity and reducing HIV transmission.
Essential Functions
Conduct client intakes in alignment with Uniform Access procedures.
Perform in-person needs assessments, gathering medical, demographic, and consent information.
Develop individualized care plans in collaboration with the client and support team, ensuring strict confidentiality.
Provide ongoing case management and advocacy for a caseload of individuals symptomatic with HIV.
Offer flexible service delivery, including home, hospital, clinic, and office visits as appropriate.
Support clients in navigating social service systems, arranging and securing linkages to appropriate resources.
Deliver information and referrals, conducting monthly follow-ups, monitoring related activities, and reassessing needs as necessary.
Educate and inform members of the client's support network when appropriate and with documented informed consent.

Accurately document a minimum of 450 service units (15-minute increments) each month.
Maintain accurate and timely documentation in accordance with agency requirements.
Record the minimum required client contacts within the timeframes specified in the Direct Service Policy and Procedure Manual.

* Formally / informally supervises daily &/or house functions of charge nurses, professional & frontline staff (multidisciplinary team) of assigned department.

* May be accountable for the staff coordination, scheduling & assignment, or throughput.
* May participate in & contribute to the performance management / review process.
* Contributes to the daily performance, daily adherence to regulatory requirements & drives results of team within assigned department.
* Implements departmental plans & priorities identified by accountable nurse leaders.
*Minimum Qualifications: *
* Graduation from an accredited school of nursing.
* Current Registered Nurse license by Pennsylvania Board of Nursing or Compact State.
* Baccalaureate of Science in Nursing (BSN) degree from an accredited school of nursing.
*Preferred Qualifications: *
* Master of Science in Nursing (MSN) degree from an accredited school of nursing.
* Prior leadership experience & / or other experience / licensure / certifications aligned with area of responsibility & assignment.

Responsibilities
