About our client, the Multi-Cultural Health Evaluation Delivery System (MHEDS)
MHEDS was established 50 years ago as a joint project of the Erie Diocesan Mission Office and the Erie Council of Churches to meet the health needs of African-American and Hispanic Farm Workers and other low-income individuals and their families. Refugee and immigrant health were added in the late 1990s. On January 1, 2019, MHEDS entered the Federally Qualified Health Center system as a Look-Alike.
Our service area is the City of Erie. Our mission is to enable all who wish to achieve complete mental, physical and social well-being by providing equitable, culturally sensitive, patient-centered health care, regardless of their ability to pay. MHEDS has a very diverse panel of patients: 35% are Asian, 29% Black/African American, 29% White, and the remainder mixed race. 6% are Latino/a. As an FQHC Look-Alike, MHEDS must comply with standards that ensure all patients have access to comprehensive health care services. We also offer health insurance enrollment, interpretation, and translation.
MHEDS expanded by opening a second site at the JFK Center in 2020. The second site allowed us to begin to offer women's health and accommodation to serve more new arrivals. In late 2021, MHEDS began receiving Afghan refugees along with its anticipated arrivals. In early 2022 we began receiving Ukrainian refugees. Our anticipated 549 new patients became 1,010. This Federal Fiscal Year, we have been told to expect 750 refugees plus additional Afghans and an unknown number of Ukrainians. Eighty percent of MHEDS' staff are hired from within the New American group, allowing us to be more aligned both linguistically and culturally with those we serve.

Clinical Duties
Triages, prioritizes, responds, and re-routes incoming patient phone calls. Uses critical thinking and clinical reasoning skills to identify health concerns and potential screening identifiers.
Instructs, educates, and provides information to patient and/or designated support persons regarding prescribed medications, test results, provider recommended interventions, and treatment instructions.
Responds timely to requests for medication refills for non-scheduled (controlled) prescriptions within established organizational processes.
Sends all other medication refill requests to appropriate providers.
Conducts pre-visit planning for nurse visits prior to patient arrival. Includes assessing the immunization status of patients.
Administers vaccinations and therapeutic injections or treatments in accordance with current CDC guidelines and organizational standing orders. Ensures compliance to medical supply, medication, and immunization storage regulations.
Responsible for accurate, factual, and complete documents and records. Includes the specific conversations and instructions given to the caller and their perceived understanding. Accurately records any vaccine administrations or therapeutic injections in the electronic health record.
Participates in morning huddles to share relevant information with the team. Collaborates with health care team on patient status and relevant situations.
Manages health information inquiries and offers options/alternatives within the limits of nursing knowledge, approved health information resources, departmental/physician specific guidelines, and established criteria, policies, and procedures.
Assists with access to medications and treatment by obtaining pre-authorization from insurance companies if necessary. Responds timely to other inquiries from insurance companies and referral providers with documented approval from patients. Supports medical providers with DME supply orders or letters as appropriate.
Demonstrates professionalism and promotes a harmonious working environment by communicating clearly and working cooperatively with team members and all internal or external customers.

Assist in Resident and family health teaching and rehabilitation. Interprets philosophy and objectives of the facility and the nursing service to residents, families and other employees when necessary.
Provides nursing care according to the physician orders and conformance with established standards and policies of the facility. Contributes to the nursing process and adds to the resident plan of care. Documents according to the current care plan to reflect resident improvement or decline. Documents incident/accident reports in the computer and determines intervention.
Completes admission and transfer process. Does complete evaluation and body audit within 2 hours of admission. Completes risk evaluation for new admission.
Identifies and evaluates the resident's response to health problems and treatment. Informs the RN Supervisor/DON of all significant signs and symptoms, physical or mental changes of all residents in his/her care. Completes 24-hour nursing report documenting times of admissions and discharges along with resident changes.
May work or assist in Personal Care Department.

Duties/Responsibilities:
Provide direct and indirect nursing care to participants following the Care Plan.
Collect and analyze patient data, set priorities, and identify problems needing follow-up.
Develop, implement, and evaluate plans of care, collaborating with interdisciplinary teams.
Provide education, guidance, and discharge instructions to participants and families.
Respond independently to emergencies and implement advanced care as needed.
Document care, observations, and participant outcomes accurately in the electronic medical record.
Communicate participant changes and updates clearly to the care team.
Participate in Quality Improvement initiatives and clinical process enhancements.
Travel to participant homes or other sites as needed to provide care.

Education and Experience:
Must be a graduate of an accredited nursing school and have a current RN license within practicing state.
Experience in complex nursing care and interdisciplinary collaboration preferred.
Experience working with the frail or elderly population is a plus but not required. We ll provide hands-on training and mentoring to help you grow!
Requirements:
Requires valid State driver s license with a clean driving record.
Successful completion of background check, physical, drug screening, TB Screening
Physical Requirements:
Must be able to move intermittently throughout the workday.
Must function independently, maintain confidentiality, have flexibility, personal integrity, and the ability to work effectively with personnel, regional and corporate consultants, vendors, patients/participants and support agencies.
Is subject to lifting, carrying, and supporting participants.
May include occasional overnight travel for in-service training programs, temporary coverage for another Center, etc.
Must be able to drive a motor vehicle.

Responsible for providing outpatient care to patients showing a change or decline in mental status
Work independently with other members of the medical or behavioral health team, with entitlements to specific clinical privileges approved by the clinic's Board of Directors
Take part in an interdisciplinary team that provides primary care, dental and behavioral health care to employees and patients
Perform patient histories and physicals, risk factor assessment, ordering of therapeutic and diagnostic tests; and attend to consultations and appointments
Prescribe medications following the clinic's policy and relevant regulations of the State
Consult with physicians on cases that are beyond the scope of qualification of PMHNP
Put all relevant patient data in medical records and case presentations

Ensure smooth communication with other health professionals for effective delivery of clinic service to patients
Provide consultation to the clinic's nursing and medical staff for improvement in holistically treating the whole patient, which enhances overall quality service delivery of the clinic

Complete staff employee evaluations, at minimum once yearly.
Manage corrective actions with Chief Clinical Officer as needed.
Provide onsite behavioral health guidance to all staff in the absence of the Chief Clinical Officer.
Education/Skills/Qualifications:
Psychiatric nurse practitioner Masters or Doctorate degree or graduated from accredited physician assistant program with at minimum 3 years post graduate experience in psychiatry.
