During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. Clinical Responsibilities
Cross-cover hospitalized patients throughout the night
Support admissions team as needed
Evaluate changes in patient condition and respond accordingly
Communicate with the ICU team for escalations (ICU runs codes)
Document all encounters in EPIC EMR
Collaborate with attending Hospitalists, Intensivists, nursing, and specialty teams
No procedures required:
No lines, no intubations, no paracentesis, no thoracentesis - this is a non-procedural APP Hospitalist role.

Full complement of inpatient subspecialties, including:
Cardiology, Pulmonology, Neurology
General Surgery, Vascular Surgery, Orthopedics

Candidate Requirements
NP or PA with active national certification (NCCPA / NCCNP)
Hospitalist or Inpatient APP experience required
BLS + ACLS
Comfortable managing high census, cross-coverage, and acute changes overnight
OH license preferred; open to licensing candidates

Responsibilities
The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician.
Essential Duties and Responsibilities
Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
Takes history, examines, determines diagnoses.
Provides written documentation of patient visit, per NCQA standards
Takes patient vital signs, as necessary.
Places case management referrals and communicates with PCP as necessary.
Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis.
Performs all clinical duties while observing OSHA Universal Precautions
Maintains patient confidentiality
Attends required meetings and in-services and participates in committees, as requested
Participates in professional development activities and maintains professional licenses and affiliations

Required Knowledge, Skills, and Experience
Active/unrestricted nurse practitioner license to practice in coverage states
Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
Active BLS Certification
Current enrollment in Medicare/Medicaid
Must maintain a valid drivers license and good driving record
