Responsibilities:
About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member's current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:
Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
Administer and order point of care and lab-based testing as needed
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
Provide services at the member's place of residence which can be in their homes or Skilled Nursing Facilities

Flexibility - Visits are scheduled based on YOUR availability. We conduct assessments Sunday - Saturday from 8 am - 7 pm. You can work during the day, evening, and/or weekend based on your schedule.
Competitive Pay - Earn income based on the number of assessments you complete. Those completing a monthly average of 20 - 100 visits earn $1,800 - $11,500 per month. This is based on the per assessment completion rate range of $90 - $115 depending on your state of residence.

Qualifications:
Skills & Experience That Shine
Master's Degree or commensurate experience and satisfactory completion of NP licensure
Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
Current BLS, ACLS or CPR certification

What You'll Do
As an integral part of our behavioral health team, you will:
Conduct comprehensive psychiatric evaluations
Diagnose and treat a wide range of mental, emotional, and behavioral health disorders
Develop individualized treatment plans in coordination with an interdisciplinary team
Make treatment recommendations for psychotherapeutic interventions and medications
Collaborate with facility staff on behavioral strategies and care planning
Communicate effectively with patients, families, referring physicians, and other healthcare professionals

As a CRNP with Traditional Home Health & Hospice, you are able to give each patient your undivided attention and the time they deserve as you care for them. Our office is located in Dunmore, PA
Job responsibilities include, but not limited to the following:
CRNP performing face to face encounters should document the encounter in the medical record and communicate findings of the encounter to the certifying physician so that he/she can certify/sign that the required face to face encounter occurred.
CRNP will need to provide the required documentation for each face-to-face visit.
Determination of a patient's eligibility to receive Medicare home health services with their home bound status being clearly established and documented during the encounter.
Broad knowledge of medical treatment practices - Hospice patients specifically
Knowledge of healthcare regulatory agency standards for hospice/ home health care organizations.
Knowledge of The Joint Commission (TJC) and CMS principles and standards.
Familiarity with Part A Hospice Medicare and insurance coverage processes preferred.
Requirements:
Current license as a Certified Registered Nurse Practitioner (CRNP) in Pennsylvania
Excellent assessment skills, documentation skills and ability to meet deadlines
Must have a current driver's license
Ability to travel to case assignments as assigned.
Strong interpersonal skills.
Demonstrates good verbal and written communication, and organizational skills

Physical Medicine and Rehabilitation (PM&R) or Physiatry is a branch of medicine that aims to enhance and restore functional ability and quality of life to those with physical impairments and disabilities, with a focus typically on the neuromusculoskeletal system. Responsibilities:
Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant
Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
Collaborate with PCP teams to help with the management of conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
Work alongside your PT, OT, and SLP teams to improve patient function and quality of life
Follow therapy patients to track their progress in therapy
Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and much more!
Become an integral part of the SNF team by attending multidisciplinary meetings, and bridging the gap between therapy and internal medicine
Engage in culture building opportunities and ongoing education including monthly Didactic lectures, virtual meetings with your Regional Team, and annual, regional dinners to meet your nearby colleagues
