Nurse Practitioner, Nurse Educator Diabetes Key Responsibilities: * Coordinating patient self-management initiatives in a family centric program for pediatric and adolescent patients in Boston Children's Hospital's Division of Endocrinology's ADA recognized outpatient diabetes self-management education program. The program uses a multi-disciplinary approach in the management of children and adolescents with diabetes (Type 1, Type 2, steroid induced and cystic fibrosis related) and their families. * Working with patients diagnosed with diabetes and their families, to facilitate improving self-management age specific goals for all patients; providing lifestyle coaching, and conducting one-on-one patient visits and group visits in a hospital clinic setting. * Providing patient and family education; collaborating with outside community resources including but not limited to school and insurer-based case coordination services. * Providing case management including oversight and interpretation of laboratory results for patients with diabetes; ordering of therapeutics, durable medical supplies and refills for prescription medications; as well as coordination of referrals and diagnostic studies as needed in consultation with the patient's physician. * Working 40 hours per week, day shift M-F.
