Develops, implements, evaluates, and alters treatment plan of care as needed
Makes recommendations and/or adjustments to medications as needed
Demonstrates and maintains accurate, complete, and timely documentation in electronic health record to include but not limited to:
Correct diagnosis code(s)
Status of all acute and chronic conditions identified
Plan to address all diagnosis identified at time of visit
Demonstrates understanding and working knowledge of key elements of Risk Adjustment Factor (RAF) and Hierarchical Condition Categories (HCC) model within 30 days of hire
Demonstrates understanding and working knowledge of key elements of Quality STAR/HEDIS measures within 30 days of hire
Rotation of telephonic on call coverage after hours
Qualifications and Skills:
Current and valid State of Illinois RN license required
Bachelor's degree in Nursing and Illinois Advanced Practice Nursing license required; Master's degree in Nursing preferred
Board Certification from a nationally recognized certifying body for Nurse Practitioners required
Unrestricted prescribing and dispensing privileges with DEA licensure required
Minimum of 1 year experience in an ambulatory or community care setting or a combination of relevant work/clinical experience with strong assessment skills

Document clinical findings accurately and attest F2F encounters in patient medical records.
Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements.
Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
Provide clinical recommendations to the IDG based on F2F assessment findings.
Conduct assessments in the patient's place of residence, including home, skilled nursing, or other settings.
Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
May provide F2F coverage for multiple provider numbers with approval.

1+ years of experience in advanced wound care highly preferred

Attention all Nurse Practitioners, we are calling you to partner with us as independent contractors providing services to our Military Service members.
We primarily work weekends, with some occasional weekdays under the Reserve Health Readiness Program (RHRP) in the State of _________ on an "as needed basis".
We provide health readiness services to meet the medical and dental requirements to maintain a deployable military force for the following:
U.S. Army Reserve (USAR)
Army National Guard (ARNG)
U.S. Navy Reserve (USNR)
U.S. Marine Forces Reserve (MARFORRES)
U.S. Coast Guard Reserve (USCGR)
Air National Guard (ANG)
U.S. Air Force Reserve (USAFR)
Responsibilities:
Support Physical Health Assessments of military personnel.
Review health history, service member medical records, and medical questionnaires.
Interview military personnel to determine current and prior medical concerns.
Identify any concerns that need to be addressed immediately, inclusive of Behavioral Health.
Document interview results for readiness classification in the service member's medical record using the proprietary system.
