YOUR ROLE
As an Oncology Navigation NursePractitioner, you will be a critical clinical team member caring for our members. You will have three primary responsibilities:
First, using a telemedicine platform, you will evaluate and manage members identified by our nursing and provider team who have urgent symptoms, complications related to their treatment, or exacerbations of their medical conditions. You will assess presenting symptoms, develop comprehensive care plans, triage to the appropriate level of care, educate patients, and ensure follow-up with relevant primary treating clinicians, including oncology and primary care. You will also complete annual clinical intake visits for select members.
Second, you will develop care plans for our members, in collaboration with our onboarding RNs, to address clinical and non-clinical needs and barriers. You will prioritize members' goals and develop concrete plans for our Oncology Nurse Navigators and Community Health Workers to reach these goals. To do this, you will review available medical records, work with external providers, give clinical insights to our care team, and adhere to best documentation practices to drive high-quality care for our members.
Third, you will be responsible for the oversight and direction of interdisciplinary case conferences for our members, in which individualized care plans are created and monitored by our team of Community Health Workers and Oncology Nurse Navigators. You will provide clinical oversight to the case management team and direct clinical care, as needed.
In addition to these primary responsibilities, you may be asked to participate in some holiday, night and weekend emergency coverage as needed.
This role reports to our NursePractitioner Team Lead and can be remote or hybrid based in our Nashville or New York City offices. All patient interactions will be virtual via telephone, video, text, or our proprietary virtual care platform.
Within your first three months, you will:
Complete training and be up to speed on Thyme Care's systems, tools, technology, partners, and clinical expectations.
Be adept at developing evidence-based, member-centered care plans that will guide our Community Health Workers and Oncology Nurse Navigators in effectively prioritizing our members needs and goals.

This position is with a mission-driven healthcare organization that has been open for over 30 years and focuses on the underinsured, uninsured, and low-income patients in over 5 counties in the greater Denver area.
Salary and Benefits
The salary range is based on experience ($108,000 - $130,000/year)

Monday - Friday (8:00 AM - 4:30 PM)
On-call by phone after 90 days - 3-5 days per quarter (earns PTO)
Up to 18 patients per day
Mobile unit school-based - runs independently from the school system
Sole provider with support from an MA
Outpatient, primary care providing medical screenings to patients of all ages
EMR - Epic
Requirements

Board Certified in the State of Colorado

Responsibilities
Responsible for annual health assessment of high-risk patients that captures a comprehensive picture of the patient's medical condition
Enhances communication and collaborative relationships with multidisciplinary healthcare team members
Screen patients for potential participation in CCS's care management program
Acts as a patient advocate protecting privacy and confidentiality issues
Provides patient education, emphasizing importance of consistent monitoring of health needs, and recommending community resources
Facilitates patient empowerment and quality of life by promoting educated, independent patient choice on all aspects of care
Identifies opportunities for health promotion and illness prevention
Demonstrates accountability for own professional practice by participating in educational programs that increase clinical knowledge base and skill set
Complete all documentation within 48 hours from visit
Communicate clearly and timely with Team Manager when a clinical escalation is found. Document clinical escalations appropriately
Qualifications
Active un-encumbered license to practice nursing;
NursePractitioner Certification from either the ANCC or AANP; in Family, Adult, Gerontology or Emergency Medicine.
Board Certified through the AANP or ANCC
Maintains current CPR certification
Compliance, prior to hire, with recommended Healthcare Personnel Requirements for vaccinations and preventive testing:
Hepatitis B
Influenza
MMR: Measles, Mumps and Rubella
Pertussis, Tetanus and diphtheria and acellular pertussis (Tdap)
Varicella
Tuberculosis
Home Health exp a plus
Must be able to effectively communicate with elderly and chronically ill patients and families;
Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial;
Ability to multitask;
Excellent customer service skills;
Bi-lingual or multi-lingual a plus.
