What you'll be working on:
Treating patients via tele-health visits, including telephonic triage calls, video visit appointments, and email follow-ups
Continuous learning during weekly Clinical Rounds and through other modalities

Education, licenses, and experiences required for this role:
Completed an accredited FNP program with a national certification
In the past 5 years, practiced as an Advanced Practitioner for at least:

Currently licensed in NY with ability to obtain additional state licenses

The base hourly range for this role is $59.10 to $65.50 per hour (plus evening and weekend differential) based on a full-time schedule. Position Overview

Complete comprehensive histories and physicals on all new admissions including documentation of problem list, diagnosis, orders (e.g. labs, imaging and referrals) and ordering appropriate medications where applicable.
Evaluate patients requesting sick call, schedule follow-ups and update medication orders. Update problem lists and reconcile patient orders at all visits.
Implement plans for patient care utilizing protocols approved by the Medical Director and/or treatment plans reflecting the current standard of care.
Request radiology exams, lab tests, EKGs when clinically indicated and interpret these results based on clinical findings and in consultation with supervisors where appropriate.

Perform chart reviews and summaries for patients transferring facilities including updating problem lists, rewriting medication orders, and reconciling orders and consults as needed.

Teach patients about their medical conditions and treatments; counsel on risks and benefits of different treatment decisions; witness, sign, and document patient refusals of care.
Ensure that all progress notes and orders are signed before the end of the shift.
Respond to emergencies in a timely and professional manner.
Notify the staff MD and Urgicare about 3 hour runs and EMS activation.
Complete special housing rounds when assigned.
Be familiar with quality of care and population health indicators. Take appropriate action to meet or exceed standards.
Maintain clinical competency by participating in all CME and CHS training and in-service requirements.
Maintain your schedule as directed with particular attention to punctuality and timely notification of absences.
Adhere to policies and procedures of CHS and be familiar with them by reviewing them as needed.

We are seeking an experienced, patient-focused Dermatology NursePractitioner or Physician Assistant to join our dynamic outpatient clinic and aesthetics practice. This role blends traditional dermatologic care with advanced aesthetic procedures, ensuring our patients receive comprehensive, high-quality treatment for both medical and cosmetic skin concerns.
Key Responsibilities:
Patient Evaluation & Management:
Conduct thorough patient histories, physical examinations, and skin assessments.
Diagnose and treat a variety of dermatologic conditions including acne, eczema, psoriasis, skin infections, and other common skin disorders.
Aesthetic Procedures:
Perform cosmetic treatments such as Botox injections, dermal fillers, chemical peels, laser therapies, and other minimally invasive procedures.
Collaborate with the dermatology team to develop individualized aesthetic treatment plans.
Clinical Procedures & Interventions:
Assist with or perform minor dermatologic surgeries and procedures under the supervision of a dermatologist as needed.
Order, interpret, and act on diagnostic tests (e.g., skin biopsies, cultures) to guide patient treatment.
Patient Education & Follow-up:
Provide comprehensive patient education on skin health, disease prevention, and post-procedure care.
Develop and maintain individualized care plans, ensuring proper follow-up and continuity of care.
Collaboration & Documentation:
Work closely with dermatologists, nurses, and administrative staff to ensure integrated care delivery.
Maintain accurate and detailed patient records in compliance with regulatory standards and clinic protocols.
Quality Assurance & Professional Development:
Stay current with the latest developments in dermatology and aesthetic treatments by participating in ongoing education and professional training.
Ensure adherence to all clinical guidelines, safety protocols, and regulatory requirements.
Qualifications:
Education & Certification:
Master's or Doctoral degree in Nursing (NP) or completion of a Physician Assistant program (PA).
Current state licensure and board certification or eligibility in Dermatology, Family Medicine, or a related specialty.
Experience:
Proven experience in outpatient dermatology practice with a strong background in medical dermatology and cosmetic/aesthetic procedures.
Demonstrated proficiency in performing aesthetic treatments and minor dermatologic procedures.
Skills & Competencies:
Excellent diagnostic, clinical, and procedural skills.
Strong interpersonal and communication skills with a compassionate approach to patient care.
Ability to work independently as well as collaboratively in a multidisciplinary team environment.
Detail-oriented with strong organizational and documentation skills.
Preferred:
Additional certifications or specialized training in aesthetic medicine and laser procedures.
