The Community Health Centers of Lane County (CHCLC) is a dynamic, growing organization that has been providing comprehensive primary care and behavioral health services to the citizens of Lane County since 2004. We operate eight primary care clinics in which we provide a broad array of services to adults and children that are organized around care teams who work collaboratively to support our patients. Our integrated care teams include primary care providers, nurses, pharmacists, medical assistants, patient care coordinators, community health workers, and behavioral health specialists. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics. Our care teams also help patients reach their optimal health status by addressing social determinants of health barriers to care such as transportation, food insecurity, or housing. The CHCLC provide affordable primary care for the whole family - infants, children, adolescents, adults, and seniors - as well as alternative services such as acupuncture, and preventative dental services. At the Community Health Centers of Lane County, we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness, but also health education and behavioral health services.

Responsibilities:
Clinical Competency
Provide comprehensive primary care to patients in a professional manner, leading to trust, engagement, and positive health outcomes.

Clinical Responsibility

Complete patient charts within 72 hours of providing a visit to a patient.
Be available for consultation, assistance with medical/mental health emergencies and referrals by Orchid staff while at the clinic and as mutually agreed upon in terms of after-hours consultation.
Respond to patient queries and cases daily when scheduled to be in the clinic.

Follow Clinical Guidelines for the management of patient health issues in a competent manner.

Work Schedule

Compensation:
$139,000 - $170,000/year depending on experience and skills
$8,000 sign-on bonus! First $4,000 paid after 30 days of employment, second $4,000 paid after 12 months of employment.

Located at the foot of the Cascade Mountains, Orchid Health's Oakridge Clinic is in the center of one of the best recreational areas in the state and is just 45 minutes east of Eugene. The area is known for year-round sunny weather, having over 500 miles of hiking and biking trails, and being surrounded by waterfalls, hot springs, and numerous other outdoor adventure opportunities.
Responsibilities:
Clinical Competency
Provide comprehensive primary care to patients in a professional manner, leading to trust, engagement, and positive health outcomes.
Treat all patients equally and to the best of your ability.
Maintain a calm and professional demeanor.
Model actions that demonstrate respect for all individuals, treating patients without judgment or prejudice.
Practice empathy, using a Trauma Informed Care approach with all patients.
Maintain licensure and certifications and follow all necessary measures to maintain and demonstrate competency as required by your Licensing Board.
Clinical Responsibility
Create and maintain thorough patient records including health history, present illness, physical findings, assessment, medical decision making, and treatment plans. Orders for prescription drugs, diagnostic testing, and referrals should be documented. Interpretation of test results should also be documented.
Maintain chart notes comprehensively, with proper management of Problem Lists and Medication Lists, and active involvement in the development and maintenance of Patient Care Plans.
Respond to patient lab, imaging, and referral results within 24 hours on days scheduled on duty.
Complete patient charts within 72 hours of providing a visit to a patient.
Be available for consultation, assistance with medical/mental health emergencies and referrals by Orchid staff while at the clinic and as mutually agreed upon in terms of after-hours consultation.
Respond to patient queries and cases daily when scheduled to be in the clinic.
Work to meet productivity targets as agreed upon by the leadership team.
Follow Clinical Guidelines for the management of patient health issues in a competent manner.
Adhere to policies in managing patients prescribed Controlled Substances.

Work Schedule
Scheduled hours: 1.0 FTE = 40 scheduled hours or 0.6 FTE = 24 scheduled hours
Patient care hours: Orchid PCP providers are generally scheduled to have ~ 75% of their scheduled hours available for direct patient care. The remaining 25% shall be made available for care coordination, team huddles and/or meetings, community health initiatives, charting, and other project-based work.
Required Qualifications:
Current certification as a Nurse Practitioner or Physician's Assistant
Must be licensed to practice medicine in the State of Oregon prior to beginning employment
Must have and be able to maintain current DEA certification, licensure, and board certification
Work Environment:
While in the clinic, team members may be exposed to viruses, disease, and infection from patients and specimens in the working environment. Team members may be required to work for extended periods of time at a computer. Team members use standard office equipment, such as computers, phones, and e-fax.

While performing the duties of this job, the employee is regularly required to talk and hear.
