This process includes: Assessment of health status, Diagnosis, Interpreting labs and diagnostics, Development and implementation of treatment plan in accordance and collaboration with interdisciplinary care planning process, Actively develops and implements advanced care planning on all residents Follow-up and evaluation of the patient status as necessary, Completion of all necessary documentation regarding health care of customer in accordance with established policies and procedures and relevant State and Federal regulatory requirements Follows evidence-based practice guidelines 2.
