The Local Health Guide is responsible for in-person whole person assessments, chronic care follow-up, and urgent care for our vulnerable population, collaborating closely with specialists psychiatric registered nurses, primary care nursepractitioners, and psychiatric nursepractitioners.
As a Local Health Guide, you will provide the following:
* Work at least three days per week of in-office or home care
* Assessing both behavioral and physical health conditions with a thorough history and in-person physical examination
* Capturing conditions not yet diagnosed, ordering additional studies where indicated to confirm or rule out the diagnosis
* Provide primary and urgent-care treatment to minimize preventable ER visits for conditions such as COPD exacerbations, uncontrolled diabetes, CHF exacerbations, Cold and Flu Symptoms, UTI, pain, anxiety, depression, and suicidal ideation
* As part of an interdisciplinary care team, collaborate regularly with certified peer recovery specialists (firsthand Guides) on individualized recovery plans
* Making individualized recommendations and/or referrals for programs/care provided by firsthand and/or outside providers, based on the assessed need
* Supporting individuals in developing and exercising self management skills and person-centered care around their individual goals and preferences via motivational interviewing and holistic care planning
* Organizing clinical information in a manner that supports appropriate clinical documentation and successful disability applications (if appropriate)

Wound Care NursePractitioner Job Responsibilities:
Plan, direct and evaluate resident and employee health care in long-term care
Daily Travel to various facilities. Regions can vary based on scheduling needs.
Examine and treat chronic and acute episodic wound-related illnesses and accidents, minor injuries, and accidents.
Interpretation of diagnostic tests and recommendation of appropriate follow-up.
Refer to and update physicians, write prescriptions, and perform bedside procedures associated with wound care.
Work with the medical director in the care and treatment of residents and staff.
Debridement of wounds by sharp, enzymatic, autolytic, or surgical methods is able to be safely completed in the place of patient visit and all associated procedures to complete the debridement.
Application and management of topical wound care and associated wound management products.
Direction and recommendation of associated services to improve wound healing outcomes.
Educate patients and Primary Care Physicians on current wound management practices.
NP Qualifications and Requirements:
Proof of Covid vaccination.
Must be currently licensed as a registered nurse in the state.
Current Active, unrestricted state APRN license or ability to obtain prior to hire.
Successful completion of an approved nursepractitioner program.
Current approval by the Board of Nursing to perform medical acts.
Master's degree of Nursing.
Excellent organizational skills and attention to detail.
Strong analytical and problem-solving skills.
Physical Requirements

Key Responsibilities
NursePractitioners provide primary and/or consultative professional care in a variety of care settings. The NursePractitioner will function in collaboration with physicians according to a Collaboration Arrangement. The NursePractitioner practice will focus on caring for geriatric and seriously ill populations to assist patients and families with comfort and quality of life.
In this role you will be responsible for:
Conducting medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment
Determining follow up assessments according to patient need and best practice guidelines
Communicating significant findings, problems, and changes in condition or environment to the clinical coordinator, the physician, family, facility and other personnel involved with patient care
Assessing health status by obtaining relevant health and medical history.
Performing a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician
Ordering, interpreting and evaluating laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needs
Performing diagnostic of therapeutic interventions consistent with certification, law and state medical privileges
Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency
Developing a treatment plan in collaboration with the patient and caregivers
Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment
Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits
Holding case conferences with families when appropriate to advance the care plan
Meeting minimum productivity requirements based on practice standard.
Required Qualifications
Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
Master's or doctoral degree as a Board-Certified NursePractitioner
Current State License as a Registered Nurse and NursePractitionerNursing skills as defined by accepted nursing standards
Sit, stand, bend, lift, and move intermittently
Must be able to lift at least 35lbs and bear the weight of an average adult effectively
