Responsibilities:
The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The Advanced Practice Provider collaborates on-site with the Center Leadership Team for center specific goals and priorities. The Advanced Practice Provider collaborates with the Medical Director and/or Attending Physician for patient specific medical care.
The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. The Advanced Practice Provider works collaboratively with Center Leadership to achieve AlignMed Partners mission of improving the lives we touch through the delivery of high quality health care and everyday compassion.
Qualifications:
The Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a NursePractitioner by being a nursing graduate from an accredited NursePractitioner Program with a Master's Degree in Nursing and have a concentration and specialty track of Older Adult and/or Family NursePractitioner (FNP). Will consider other concentrations/specialty tracks based on experience and business need.
A minimum of two years experience working with adult or geriatric population is preferred.
In states where practice allows; APRN with an unrestricted professional license and DEA, for each state in which they practice.

As a NursePractitioner, you will:
Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care.
Take responsibility for patient palliative care from the initial assessment through discharge. Assessment at a minimum should include: disease-specific changes, decline or change in function, nutrition and/or cognition, inadequately controlled pain or other distressing symptoms, falls, infections or similar events, the impact of the disease burden on the patient's condition, caregiver and/or family's quality of life.
Establish the palliative plan of care, individualized for each patient, in consultation with the patient, family and other members of the Palliative Care Team.
Facilitate obtaining care for patient when a need is identified in accordance with the Plan of Care.
Participates in evening/weekend call as required, conducting on-call services in a clinically competent and responsive manner.
Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law. Order and interpret laboratory and other diagnostic tests within the scope of professional practice. Order treatments and durable medical equipment as indicated.
Communicate clinical findings to the Palliative Medical Director as needed, but no less than bi-weekly. Re-assess effectiveness of care plan on a regular basis and modify as needed.
Maintain effective working relationships with supportive care team (e.g., Social Worker, Spiritual Care Coordinator), as applicable, and involve team when patient's care plan warrants team participation to achieve desired outcome.
Establish and maintain effective working relationships with those contacted in the course of work.
Communicate and document clinical findings, treatment plan and care provided in the patient's medical record in a manner consistent with acceptable standards in order to support sound medical practice and reimbursement for services provided.
Knowledgeable of Palliative Medicine Quality Measures collection, documentation and reporting.

Meets mandatory continuing education requirements of Gentiva and licensing board.
Must be able to work efficiently with point of care software.
Education/Experience:
Minimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state law

Plan, direct and evaluate resident and employee health care in long-term care
Daily Travel to various facilities. Regions can vary based on scheduling needs.

Graduate of an accredited NursePractitioner program.
Licensure/Certification
Current unrestricted license to practice as Registered Nurse in North Carolina and approved to practice as a NursePractitioner by the North Carolina Medical Board and the North Carolina Board of Nursing. BLS certification required.
Experience
Minimum 2-3 years' experience working with adolescents in a pediatric unit, office, or health clinic preferred.
Knowledge, Skills and Abilities
- Knowledge of adolescent development processes. Knowledge of adolescent nursing, adolescent and human development, common childhood infectious diseases and chronic and acute health conditions.
- Excellent interpersonal skills. Able to build rapport with adolescents.
- Ability to read, write and communicate effectively in English.
- Proficient with MS Office and EHR, with the ability to learn new software rapidly.
Valid NC Driver's License: No
