POSITION PURPOSE Adobe Population Health is hiring a part-time Nurse Practitioner (NP) to conduct in-home wellness assessments for both pediatric and adult members. This role focuses on improving health literacy and overall quality of life through comprehensive annual visits. The NP will complete in-home annual wellness assessments (which may last up to one hour) and will evaluate medical history, current medications, health status, and social determinants of health. This part-time position requires a minimum commitment of 20 hours per week.
