The NP will complete in-home annual wellness assessments (which may last up to one hour) and will evaluate medical history, current medications, health status, and social determinants of health. Conduct in-home assessments on identified patients, following established guidelines. Visit types may include: In-home Medicare and Medicaid Assessments. In-home Well-Child Visits (WCV), for children ranging from 6 months to 21 years of age. In-home Child and Adolescent Well-Care Visits (WCV). In-home Annual adult Wellness Visits (AWV). In-home Health Risk Assessments (IHRA). Telephonic Adult Annual Preventative (AAP) Assessments. Telehealth assessments, as needed. Understand the importance of preventative health evaluations focused on monitoring physical, developmental, behavioral, emotional health, and the assessment of social determinants of health (SDOH). Assess psychosocial and clinical needs of patients and families. Coordinate and communicate care plans with primary care and community providers. Support appropriate utilization of healthcare resources. Ensure timely and accurate documentation across the continuum of care. Formulate a list of current and past medical conditions using clinical knowledge, judgment, and current findings. Identify urgent and emergent situations for proper intervention. Educate members on topics related to health conditions and the continuation of care. Identify diagnoses to be used in care management and active medical management. If client requires further ongoing chronic medical management, PCNP is expected to work with residential staff so that clients can be referred to a primary care provider in the community. In the future, this care may be integrated within the Prototypes program in which case the PCNP would follow up with these patients and provide direct primary care. PCNP is also expected to help prescribe bridging medications as needed for clients until they can be connected to a specific primary care provider (30-60 days). PCNP should also feel comfortable providing first line management for patients with uncomplicated stable mental health diagnosis such as depression, anxiety, bipolar disorder or schizophrenia. Under the supervision of the Supervising or Collaborating Physician with a specialty in HIV Medicine, the Advanced Practice Provider will: • Provides HIV, TB and other infectious disease care in accordance with standard medical proactive protocols. • Provide medical services, within scope of practice as determined by licensure and/or certification. Provide additional medical services, as determined by Chief Medical Officer, within your scope of practice, such as Medication Assisted Therapy (MAT) or Pre-Exposure Prophylaxis (PrEP). • Accurately complete all necessary charting according to clinic protocols and mandated by funding source guidelines and standard medical practice. • Records medical history and provides physical examinations and follow-up care. • Provides individual patient instruction in accordance with the prescribed treatment plan. Works closely with HIV Case Managers in order to provide appropriate education to the patients. • Prescribes appropriate medication to patients as needed according to medical protocols and standard medical practice. • Identify physical, nutritional and/or lifestyle problems and refers to appropriate on-site and off-site resources. • Identify psychosocial and emotional problems/issues and refers to on-site or off-site resources. • Must be in compliance with all clinical and administrative quality measures such as HEDIS metrics and patient productivity. • Consults with supervising/collaborating physician or Chief Medical Officer for patient evaluation as needed. • Perform timely documentation/action within Electronic Health Record (EHR) including but not limited to, diagnosis, treatment plan, orders, medications, etc. • Take timely action on pending EHR PAQ items. • Issue timely responses to E-Consult specialist reviewers. • Able to provide medical services on telehealth platform provided by EVCHC. • Work closely with Registered Dieticians/Health Educators in order to provide appropriate education to patients. • Participate in monthly Provider Meetings as required. • Adheres to Quality Assurance procedures and participates in Quality Assurance reviews. • Participates in team meetings and case conferences for HIV patients. • Serves as a preceptor instructor to Nurse Practitioner, Physician Assistant, or medical students as assigned. • Attends conferences and seminars when requested and will comply with continuing education requirements.
