Deliver excellent patient care through in-patient and virtual diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information.
Ensure that the activities of the Provider are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA.
Ability to work efficiently in a fast-paced, autonomous environment within both in-patient clinic and virtual settings.
Ability to pursue queue of telehealth patients in a timely fashion to ensure proper patient follow up.
Order, perform, or interpret the results of diagnostic tests, as well as responsibly prescribe medications and educate patients on continued treatment and care of acute and chronic conditions.
Attest and follow clinical practice guidelines by the Office of Medical Affairs.
Attend Mandatory monthly meetings with Supervising physicians and E/M training.
Ability to perform responsibilities within standard NP protocols.
Responsible for learning the aspects of compliance in the company by completing all mandatory compliance training in order to meet and exceed our continued quality of care.

+ Provides primary and urgent health care via telephone and tele video modalities to patients who receive home and community-based services through state Medicaid programs, dual eligible members and other membership as assigned by our MCO partners.
+ Develops and implements clinical plans of care for adult patients facing chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions.).
+ Gathers history and physical exam and diagnostics as needed and then develops and implements treatment plans given the patient's goals of care and current conditions. Identifies and closes gaps in care.
+ Meets the patient's and family's physical and psychosocial needs with support and input from the company's inter-disciplinary team.
+ Educates patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention.
+ Maintains contact with other clinical team members, patients' other physicians and patients' other medical providers to coordinate optimal care and resources for the patient and his or her family in a timely basis and consistent with state regulations and company health standards and policy.
+ Maintains patient medical records and medical documentation consistent with state regulations and company standards and policy.

This is a procedure-oriented environment primarily serving geriatric patients. The successful candidate must possess at least 1 year of procedural background. We seeking to partner with a dedicated and compassionate NursePractitioner to deliver advanced in-home wound care for chronic and complex wounds. The ideal candidate will provide high-quality patient care, perform assessments, and develop treatment plans in the patient's home or potentially in your office. This role requires a strong commitment to patient advocacy and the ability to work collaboratively with other healthcare professionals.
Duties
Conduct comprehensive wound examinations and health assessments for patients upon referral. Debride and graft chronic wounds that have not healed with at least 4 weeks of conservative treatment. Collaborate with physicians and other healthcare providers to ensure coordinated care. Maintain accurate medical records and documentation in compliance with healthcare regulations.
Skills
Independent Medicare Part B credentials are strongly preferred. Experience in wound care is highly desirable. Ability to manage medical records efficiently while ensuring patient confidentiality. Excellent communication skills for effective patient interaction and collaboration with healthcare teams. Strong organizational skills to manage multiple patient cases simultaneously.

Franciscan Health Indianapolis Campus 8111 S Emerson Ave Indianapolis, Indiana 46237 WHO WE ARE LOOKING FOR: Franciscan Physician Network is seeking an Advanced Practice Provider with past or present Oncology work experience to meet the growing demand for our Oncology - Hematology team at Franciscan Physician Network Oncology & Hematology Specialists located inIndianapolis, Indiana. Advanced Practice Provider will collaborate closely with another inpatient APP, as well as a weekly rotation of medical oncologists on the inpatient setting.
WHAT WE NEED:
Ideal candidate will have past or present experience in oncology and/or as a hospitalist. Advanced Practice Provider will have a focus on clinical excellence and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with inpatient clinical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.
WHAT YOU CAN EXPECT:
* Flexible full-time schedule
* Inpatient night call: Will rotate 2 night calls (Monday-Thursday) with fellow APP
* Average inpatient volume: 20 patients per day
* Average patient consults: 5 consults per day
* Experience in Oncology is strongly desired
* Will collaborate with rotating oncologist on assessment and plan
* Common Diagnoses:
* Breast
* Colon
* Lung
* Head and neck
* Rectal
* Benign and malignant hematologic disorders
* Collaborate with rotating oncologists weekly
* Develop heightened inpatient oncology assessment skills
* Engage with clinical staff providing direct patient care
* Oversight of infusion center patients; available for assistance as needed

Contract with our leading clinician-owned and led health company today. The Warrior Vets Medical Services team provides exemplary general medical services. We offer in-person care to current US Military servicemen and women, as well as to veterans.
We pay up to $150 per hour and offer flexible scheduling; you will receive compensation for each evaluation. We typically remit payment within 45 days of encounter documentation in our user-friendly EMR.
Typical Appointments
In-person appointments consist of general medical evaluations. Evaluations are performed once per veteran and typically take 30 to 90 minutes. These evaluations make a difference in the lives of veterans and directly impact their access to the ongoing care they need. There are no ongoing treatment needs for these veterans; each appointment is a one-time encounter.

We Schedule Around Your Availability.
Make your own schedule! Evaluate clients, earn a comfortable income, and be home in time for dinner. We efficiently schedule each series of evaluation appointments, which last no more than a total of eight hours each day. Our current need is for 3-4 days per week.
No Administrative Tasks Required
Our back-office support team handles all administrative work, including contracting with the Veterans Administration, liaising with contractors, and arranging payment. This approach allows you to spend your time where it should be spent: with the patient or your family.
Training and Experience
These evaluations make a difference in the lives of veterans and directly impact their access to the ongoing care they need. Our clinical onboarding team of advanced level practitioners is available to train you on working with the veteran population. They provide excellent ongoing clinical support for any questions or concerns.

Requirements:
Unrestricted state medical licensure without limitations to perform full scope of authorized practice
Education and training in a US-accredited, post-baccalaureate (Masters) course of study
Certification by the National Commission on Certification of Physician Assistants (NCCPA) or certification by the ANCC or AANP
2+ years of post-graduate primary care experience
Established collaborating physician
Part-time or full-time positions available.
