About the Role:
We are hiring experienced NursePractitioners who are passionate about and who have recent experience in obesity medicine to provide longitudinal telehealth care for patients with obesity. In this role, you would collaborate with physicians and registered dietitians to deliver exceptional team-based care. This will be a full-time, remote position.
We seek applicants who:
Have a NursePractitioner degree, with experience practicing Obesity Medicine, including effective evidence-based lifestyle counseling and pharmacotherapy
Are licensed to practice and prescribe legally in FL, with preference for additional licenses in the following states: MA, IL, NY and/or TX
Have 2+ years of experience in outpatient or obesity medicine, endocrinology, or bariatrics, and/or 2 + years of obesity medicine experience in a primary care/internal medicine setting
Have excellent interpersonal skills for patient and team interactions
Are committed to the highest quality of medical care and an evidence-based approach to obesity management
Have prior experience conducting telemedicine visits and who have a great "web-side" manner
Bilingual candidates with medical fluency in Spanish are highly preferred

Overview
The NursePractitioner provides clinical care and consultation to hospice patients and families across multiple care settings. Responsibilities include face-to-face encounters, routine and urgent symptom management, participation in hospice certification and recertification processes, and clinical education, all in accordance with state regulations and organizational protocols.
Key Responsibilities
Conduct comprehensive face-to-face encounters for patients entering or continuing the third and subsequent hospice benefit periods
Perform evaluation, consent, and admission visits; assess patient eligibility and collaborate with the interdisciplinary team to initiate hospice services
Provide routine, urgent, or symptom management visits based on patient condition and plan of care
Assess, plan, and evaluate care in collaboration with the hospice physician and attending providers
Participate in interdisciplinary group meetings and contribute to the development and revision of patient plans of care
Educate patients, families, and caregivers regarding disease progression, medications, and end-of-life expectations
Provide clinical consultation and mentorship to team members and participate in internal education activities
Document all patient encounters and clinical interventions accurately and timely in the electronic medical record
Provide on-call coverage including evenings or weekends as assigned to support continuity of care
Maintain current clinical knowledge in palliative care, symptom management, pharmacology, and applicable state practice regulations
Ensure compliance with HIPAA, hospice policies, and regulatory standards in all documentation and communication
Collaborate with physicians and clinical leadership to ensure appropriate prescribing practices and care planning
Practice within the scope authorized by the state, including physician collaboration where required
Perform other duties as assigned
Qualifications
Graduate of an accredited Bachelor of Science in Nursing program and an NP-focused Master's or Doctoral Nursing Program
Current Registered Nurse and Advanced Practice Nurse license in the state of practice
Active prescriptive authority and DEA registration as required by state law
Minimum of two (2) years of experience as a NursePractitioner; hospice or palliative care experience preferred
Strong knowledge of palliative care, pharmacology, and end-of-life symptom management
Effective communication and interpersonal skills with patients, families, and interdisciplinary team members
Palliative care certification such as CHPN or ACHPN preferred but not required
Reliable transportation, valid driver's license, and current automobile insurance

Plans patient care based on in-depth knowledge of the specific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonly encountered including the consideration of the patient's cultural background, level of understanding, personality and support systems. Patient management includes the following:
Writing admission, transfer and discharge orders.

Typical Appointments
In-person appointments consist of general medical evaluations at one of our partner facilities. Evaluations are performed once per veteran and typically take 30 to 90 minutes. There are no ongoing treatment needs for these veterans; it is a one-time encounter.
Our current need is for 3 days per week.
We efficiently schedule a series of evaluation appointments that last no more than a total of eight hours on each of those days.
No Administrative Tasks Required
Our back-office support team handles all administrative work, including contracting with the Veterans Administration, liaising with contractors, and payment. This approach allows you to spend your time where it should be: with your patients.
Note: The evaluations are general medical evaluations, not disability evaluations.
