This is a guaranteed-pay 1099 contract with a fast turnaround and fixed schedule - ideal for independent providers who want stability and autonomy without the demands of prescribing or follow-up care.
Position Details
Contract Type: 1099 Independent Contractor
Compensation: $18,600/month (paid $9,300 semi-monthly)
Contract Term: 14 weeks
Start Date: October 1 (partial week), ends December 29 (partial week)
Schedule: Full-time, 40 hours/week (8-hour days)
Fixed schedule options: Mon-Fri, Tue-Sat, or Wed-Sun
No time off permitted during the contract term
Holiday Closures: November 27-30, December 25-28

Primary Coverage: Kent County, MI
Providers must reside in Kent County or an immediately surrounding county
Open to providers outside Kent/neighbors, but no hotel or rental cars provided
Mileage reimbursement: begins from first patient visit of the day (not from home → first visit, or last visit → home)

Active Nurse Practitioner license in Michigan (all specialties except PMHNP)
Availability to work 40 hours/week for the full contract term
Willingness to travel within Kent County (and surrounding areas if required)

The Local Health Guide is a licensed Nurse Practitioner who provides primary and urgent care in-person care (home, street, office setting) for individuals with serious mental illness that firsthand serves. This is a part-time role, estimated at working about 8-14 hours/week. This role is not benefits eligible.
The Local Health Guide is responsible for in-person whole person assessments, chronic care follow-up, and urgent care for our vulnerable population, collaborating closely with specialists including behavioral health providers, as well as an internal team of peer recovery specialists, psychiatric registered nurses, primary care nurse practitioners, and psychiatric nurse practitioners.As a Local Health Guide, you will provide the following::
Work at least three days per week of in-office or home care
Assessing both behavioral and physical health conditions with a thorough history and in-person physical examination
Capturing conditions not yet diagnosed, ordering additional studies where indicated to confirm or rule out the diagnosis
Provide primary and urgent-care treatment to minimize preventable ER visits for conditions such as COPD exacerbations, uncontrolled diabetes, CHF exacerbations, Cold and Flu Symptoms, UTI, pain, anxiety, depression, and suicidal ideation
As part of an interdisciplinary care team, collaborate regularly with certified peer recovery specialists (firsthand Guides) on individualized recovery plans
Making individualized recommendations and/or referrals for programs/care provided by firsthand and/or outside providers, based on the assessed need
Supporting individuals in developing and exercising self management skills and person-centered care around their individual goals and preferences via motivational interviewing and holistic care planning
Organizing clinical information in a manner that supports appropriate clinical documentation and successful disability applications (if appropriate)

SUMMARY OF RESPONSIBILITIES
The Nurse Practitioner is responsible for the palliation and management of the terminal illness for assigned hospice patients when assuming the role of Attending Physician. At all times follows practices approved within the Conditions of Participation of the Medicare and Medicaid Programs. Actively supports the work of the interdisciplinary team.
RESPONSIBILITIES AND DUTIES
Primary Duties
Completes face to face assessments and provides clinical findings to hospice physicians for the completion of the recertification of Hospice patients, ensuring they meet the medical criteria for admission and for continued services in the Hospice program, based upon available diagnostic and prognostic indicators.
Collaborates with attending physicians from the community and other interdisciplinary team members to establish immediate and long-term therapeutic goals. Helps to set clinical priorities and in developing a plan of care.
If the Nurse Practitioner assumes the role of Attending Physician for any patient, she/he will be responsible for meeting the medical needs of the patient.
Shares the agency's patient caseload responsibilities with the Medical Director and any other Hospice Physician(s) including rotation to on-call responsibilities.
Develops ongoing positive and professional relationships with contracted providers and other related bodies.
Monitors, interprets, and reports to the Medical Director clinical indicators to detect trends which could affect quality of care.
Participates in public relations and community activities that promote Hospice Care of Southwest Michigan, as requested.
Promotes Hospice referrals in the community; maintains a good standing in the community.
Provides families and others in the community with appropriate information to make informed decisions regarding Hospice care and services.
Provides ongoing education and consultation on medical issues to staff, to assist them in continually improving all aspects of Hospice services provided.
Stays informed about changes in the field(s) of expertise and hospice care and shares information with the appropriate personnel.
Assists with preceptor/teaching assignments with allied health, medical students, Residents and others while on hospice and palliative care rotations with the agency.
Meets established visit goals for hospice patients residing in their homes, including assisted living, nursing homes and other living environments.
Competent in the use of the electronic medical record for documentation if applicable to assigned work and understands Nurse Practitioner and physician billing codes and their appropriate application that reflects the service provided.
Uses established clinical guidelines for quality and optimal outcomes in making decisions concerning ordering of medications and treatments. Cost effectiveness and the use of hospice standing orders are part of the decision making.
All documentation is completed within timeliness guidelines and is accurate and supports sound medical practice and reimbursement for services provided.
Makes decisions in medication management that consider ordering medications that are formulary, when possible, discontinue medications that are not helpful, assists in discussion with patient and clinical team about appropriateness. Understands and uses the “BUILD” model. Uses the pharmacists at Enclara as resources.

Key Responsibilities
Nurse Practitioners provide primary and/or consultative professional care in a variety of care settings. Assessing health status by obtaining relevant health and medical history.
Performing a physical exam based upon the age and patient history as per the Standard Care Arrangement with a collaborating physician
Ordering, interpreting and evaluating laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needs
Performing diagnostic of therapeutic interventions consistent with certification, law and state medical privileges
Prescribing and monitoring medications as allowed by prescriptive authority through the appropriate State Board Agency
Developing a treatment plan in collaboration with the patient and caregivers
Referring patients to physicians as appropriate for consultation or for specialized health resources and treatment
Ensuring continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas
Providing patients and families an understanding of the diagnosis and disease progression while decreasing unnecessary emergency room and hospital visits
Holding case conferences with families when appropriate to advance the care plan
