Conduct in-home visits that focus on a person's overall health and well-being and identify potential barriers to care
Earn up to $2,500 to $4,000 each week (Average 6-9 visits daily, base rate of $80 per completed assessment)
Earn additional compensation via our pay-per-test model (for visits where the health plan member qualifies to receive additional diagnostic and preventive tests)
Shifts available Monday - Sunday (no nights/weekends required)

Pre-scheduled visit in-home w/ health plan members

These visits include a medication and medical history review and a physical evaluation performed in a health plan member's home. Our NP contractors can earn a competitive wage (or supplement their current income) while managing other personal and clinical commitments. You will be provided a pre-populated, patient-specific form for each evaluation and do not prescribe medicine, order lab tests, or alter people's current treatment regimen.

* Communicate significant findings, problems, changes in condition or environment to the clinical manager/attending physician and other members of the interdisciplinary team.
* Complete face to face encounters and assesses the following: disease related changes, decline in function, adherence and effectiveness of pain and symptom regimen, the patient and family coping mechanisms and quality of life.

Full-time Description
Purpose:
The primary purpose of the NursePractitioner is to maintain patient care within the
Center by providing MAT services within the parameters of policy, state, and federal
regulations and procedures. The NursePractitioner is an essential part of creating a safe and
well-balanced environment for ReVIDA's patients. The position creates an atmosphere of warmth, personal interest, positive emphasis, and a calm attitude when visiting with staff, patients, and visitors.
Duties:
The NursePractitioner will work effectively within a multidisciplinary treatment team, assist in
complex problem solving with patients and work autonomously. They will demonstrate
excellent observation, good judgment, and communication skills. Must have the ability to
provide well-written documentation promptly. They are skilled in conducting assessments,
utilizing ASAM criteria for appropriate patient placement and continued to stay justification,
treatment plan development and implementation, service coordination, and crisis
intervention. Knowledge of local community drug trends, the effects on the body and cognitive
functioning of drugs of abuse, signs, and symptoms of opioid overdose, psychosocial
implications of substance use disorders, the process of addiction and treatment, social
service, economic, legal, and medical systems, HIV transmission-health, behavioral, and
emotional implications of addiction.
Physical, Emotional Demands, and Work Conditions:
Work is sedentary and ambulant with occasional physical exertion (lifting 30 or more pounds, walking, standing, etc.) ability to support patient weight in case of emergency or disability requiring assistance. Must be able to see, stoop, sit, stand, bend, reach, and be mobile (whether natural or with accommodation).
Quality of hearing (whether natural or with accommodation) must be acceptable. Must be able to communicate both verbally and in writing. Must be able to relate to and work with mentally and physically ill, disabled, emotionally upset, and hostile patients. Must be emotionally stable and exhibit the ability to display coping skills to deal with multiple situations.
Risk of exposure to infections, bloodborne pathogens, and other potentially infectious materials or contagious diseases. For this reason, "Universal Precautions" must always be followed. Provides backup call coverage;
Serves in a teaching capacity for the training and development of other personnel;
Responsible for securing physical exams and history from the patient, reviewing medical records, making medical evaluations, requesting appropriate diagnostic testing, prescribing medications and treatments, assisting in the management of behavioral or mental health problems of patients, maintaining current medical records in accordance with federal or state regulation, and clinic or corporate policies and procedures; and,
Refer patients to PCP and/or specialist when necessary; Discuss patient's care as part of the multidisciplinary treatment team
Collaborate with referral sources, internal team members, and external service providers to ensure appropriate patient care and continuity of care
Interpret test results to diagnose health issues; provide feedback to patients
Complete patient evaluations and physical exams with thorough, timely documentation to include coordination of care
Complete full and appropriate documentation to support professional billing for services
Perform follow up visits to include detailed, timely documentation, including patients' progress with medication and/or treatment
Participates in provider meetings and staff meetings as determined by the Medical Director
Facilitates leadership regarding new findings in the field of addictions treatment
Demonstrate use of evidence-based practices

Requirements
Minimum Requirements:
Education: Master of Nursing
Experience: Have experience working with individuals with substance use and other co-occurring mental disorders.
License/certifications: Licensed in the State of TN or VA and must have or be able to obtain a DATA2000 waiver to prescribe buprenorphine
Background check: ability to pass routine state/federal criminal background check and be eligible for credentialing under CMS and commercial health plans
Special Requirements: Required/Maintained state trainee status, intern, registration,
training standing, and associate level clinical licensure. Knowledge of applicable State Code
of Regulations and 42 Code of Federal Regulations (CFR) Section 8.12. Compliance with
accepted professional standards and practices; Adherence to the NAADAC Code of Ethics and
any other Codes of Ethics for the individual's respective field.
Continuing Education & Professional Licensing/Certification Requirements: Employee is
expected to participate in appropriate continuing education as may be requested and
required by their immediate supervisor. In addition, the employee is expected to accept
personal responsibility for other educational activities to enhance job-related skills and
abilities. The employee must attend mandatory educational programs and maintain current
professional certifications as delineated above in their state, in good standing.
